Despite the ominous prognosis of severe left ventricular (LV) dysfunction from coronary artery disease, coronary artery bypass grafting (CABG) in this setting remains controversial because of concerns over high operative risk and low likelihood of functional or survival benefit. We analyzed 135 consecutive patients (113 men, 22 women; age 42 to 87 years, mean 66.5) with LV ejection fraction (EF) ≤30% undergoing isolated CABG by 1 surgeon over an 8-year period. LVEF ranged from 10% to 30% (mean 23.6%). Preoperatively, 63% of patients had angina, 61% had heart failure (23% with pulmonary edema), and 24% manifested severe ventricular arrhythmia. The mean number of grafts was 2.7 per patient. The internal mammary artery was used in 103 of the 120 grafts (86%) to the left anterior descending coronary artery. Seven patients (5.2%) died in hospital. Only 2 of 99 patients (2%) not in intensive care preoperatively died in hospital. Angina class improved by 2.0 categories and congestive heart failure class by 1.5 categories. LVEF (assessed in 104 of 128 hospital survivors) improved from 24% preoperatively to 34% postoperatively (p <0.0001). At 1, 3, and 4.5 years respectively, all-cause survival was 87%, 81%, and 71%, and freedom from cardiac death was 90%, 85%, and 80%. CABG in patients with coronary artery disease and advanced LV dysfunction: (1) can be performed relatively safely, (2) achieves good long-term survival, (3) improves LVEF, (4) improves quality of life, and (5) can safely utilize the internal mammary artery as a conduit. The use of CABG is encouraged for patients with advanced LV dysfunction and may provide a viable alternative to transplantation in selected patients.

Experience with coronary artery bypass grafting in 135 patients with ejection fraction ≤30% indicates that operation can be performed safely, that survival and quality of life are improved, and that ejection fraction increases (24% to 34%, p <0.0001). Use of coronary bypass grafting is encouraged in these patients and may provide a viable alternative to transplantation.  相似文献

目的观察并比较主动脉内气囊反搏(intra-aortic ballon counterpulsation,IABP)对原位及冠状动脉旁路移植术后的乳内动脉血流特性的影响,为冠状动脉旁路移植术后应用IABP提供客观依据。方法对7例冠状动脉旁路移植术后应用IABP病人应用二维多普勒法在胸骨旁左右第1肋间分别测定左乳内动脉与右乳内动脉的直径、时间流速积分、心输出量和乳内动脉血流量,比较这些指标在应用及撤除IABP后的改变。结果应用IABP可使原位及旁路移植术后的乳内动脉的时间流速积分,总血流量和心脏舒张期血流量明显增加(P<0.05),使乳内动脉在心脏收缩期血流量下降,这些改变以原位乳内动脉更为朋显;尽管IABP对心输出量并无明显影响,但可明显增加乳内动脉血流量与心输出量的比值。结论IABP可明显增加原位及旁路移植术后乳内动脉的血流量,改善冠状动脉旁路移植术后心肌的血液供应。  相似文献

The left internal mammary artery (LIMA) is currently used in most coronary artery bypass graft (CABG) surgeries due to excellent long-term patency. Left subclavian artery stenosis (SAS) proximal to the LIMA origin can cause a steal syndrome leading to myocardial ischemia or LIMA failure. We retrospectively evaluated the records of 608 consecutive patients referred for CABG at our institution between October 1, 2004 and October 1, 2006 and identified 226 patients (37%) who underwent left subclavian angiography immediately after diagnostic coronary angiography. Significant left SAS was found in 6 of those 226 patients (2.7%). Subclavian angiography did not result in any complications. All left SAS lesions were successfully stented, followed by CABG surgery (using the LIMA artery) after 22+/-7 days. Left subclavian angiography in patients referred for coronary artery bypass surgery has low risk and may identify a small proportion of patients with significant proximal left SAS. Stenting of proximal left SAS can be accomplished before CABG with low risk and excellent short-term outcomes.  相似文献

目的:非体外循环冠状动脉旁路移植术(OPCABG)是治疗冠心病主要手段之一。本文拟通过回顾性Logistic分析1 516例施行OPCABG男性患者,术中应用主动脉内球囊反搏(IABP)的独立危险因素,为此类患者提供手术依据和前瞻性判断。方法:采用麻醉科2007年11月至2009年2月OP-CABG围术期数据库,回顾性分析了1 516例记录齐全的男性患者OPCABG围术期资料。以术中应用IABP为因变量,将有统计学意义的单因素进行Logistic回归分析并评价各影响因素的作用大小。结果:术前射血分数(EF)<40%(OR=4.946,P=0.001)、搭桥数>3支(OR=2.340,P=0.007)、左主干病变(OR=2.857,P=0.001)、急诊(OR=4.816,P=0.001)和术中发生心房颤动(P=0.001,OR=12.188)为围术期应用IABP的独立危险因素。结论:术前EF<40%、旁路移植血管数>3支、左主干病变、急诊和术中发生心房颤动为术中应用IABP的独立危险因素,提示在遇到这类患者时,及早在术前应用IABP,对于OPCABG患者是有益的。  相似文献

目的 乳内动脉 (IMA)是当前国际上公认的最好的搭桥移植物 ,而在老年患者中使用较少 ,因为人们觉得IMA的使用会增加手术并发症发生率和死亡率 ,而且亚洲人比欧美人IMA细小 ,手术有一定难度 ,据此事实 ,作者总结了老年患者常规使用IMA搭桥的近中期效果、技术要点及手术指征。方法 自 1 997年 4月至 1 999年 9月 ,冠心病 1 68例接受了冠状动脉旁路移植术 (CABG)治疗 ,其中老年患者 1 1 2例 (男 1 1 0例 ,女 2例 ) ,年龄 60~ 75岁 ,平均 66 .5岁。原位IMA使用率占单纯CABG手术的 92 % ,同时行双瓣置换 2例 ,单瓣置换 8例 ,室壁瘤切除、左室成形 1 0例。结果  1 1 1例痊愈出院 ,心绞痛消失 ,围手术期发生心肌梗死 1例 ,术后死亡 1例。近中期随访效果满意。结论 老年冠心病患者搭桥也应首选IMA ,而且可以常规使用  相似文献

BACKGROUND: Uncertainty exists regarding the frequency of early occlusion when the left internal mammary artery (LIMA) is anastomosed to the left anterior descending artery (LAD) through a sternotomy with conventional coronary artery bypass grafting (CABG). The issue has gained importance for comparison with less invasive surgical approaches in which operative exposure may be limited and graft anastomosis more difficult. METHODS AND RESULTS: Data were analyzed from the International Multicenter Aprotinin Graft Patency Experience (IMAGE) trial in which 617 patients underwent conventional CABG of the LAD with a LIMA between April 1993 and May 1995. Coronary angiography was performed a mean of 10.8 days postoperatively. Patients were randomized to receive intraoperative aprotinin, an inhibitor of several serine proteinases, or placebo. Because no differences existed in patency rates of LIMA grafts between patients who received aprotinin and placebo, both groups were analyzed collectively. On coronary angiography, the LIMA was widely patent (<50% stenosis) in 561 patients (91%), had > or = 50% and <99% stenosis in 48 patients (7.8%), and was occluded in 8 patients (1.3%). Therefore, the LIMA was patent in 609 patients (98.7%). Conclusions-In the IMAGE trial, the largest and most contemporary early angiographic analysis of CABG available, early patency of the LIMA was >98% when anastomosed to the LAD. These data provide an important benchmark for less invasive surgical approaches in which the LIMA is anastomosed to the LAD.  相似文献

Multiple randomized trials support the treatment of patients with multivessel coronary artery disease (CAD) and relatively normal left ventricular (LV) ejection fraction (EF) by either percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG). However, there has been a paucity of trials in the recent literature that have compared the outcomes of patients with multivessel CAD and low EF who undergo PCI or CABG. This review examines some of the clinical trials and series in this subgroup of patients and also compares the outcome of patients undergoing either procedure in the absence and presence of LV dysfunction. These trials and series support the notion that PCI can be successfully performed in patients with low EF with relatively low mortality, but that CABG is associated with greater freedom from repeat revascularization and from angina or congestive heart failure symptoms. In addition, most of the data published thus far indicate a long-term survival advantage among patients with ventricular dysfunction who have undergone CABG. Further studies, including randomized trials incorporating the evolving techniques of CABG and the recent advances in PCI, will be needed to assess the proper role and outcome of these two interventions.  相似文献

目的总结左主干急性闭塞或狭窄所致急性心肌梗死(AMI)患者急诊经皮冠状动脉介入治疗(PCI)的经验,探讨急诊PCI在此类患者中的安全性和有效性。方法从1995年1月至2004年12月,在1343例急诊PCI中,共有11例梗死相关血管为左主干。11例患者均为男性,年龄43~70岁,平均(56·4±9·2)岁,其中6例(54·5%)入院时即存在严重心原性休克。所有患者均在主动脉内球囊反搏支持下接受急诊PCI治疗。结果8例急诊置入支架,余3例仅行球囊扩张,后者有2例术后接受急诊冠状动脉旁路移植术。住院期间死亡5例(45·5%),存活的6例患者均完成3个月随访,其中4例随访超过2年,1例于术后4年猝死,1例患者术后5年重复造影检查结果良好。对比分析提示术前存在良好的侧支循环可能是影响此类患者急诊PCI术后疗效的因素。结论左主干急性闭塞或严重狭窄所致的AMI患者病情凶险,急性期死亡率高。侧支循环形成与否,治疗策略和预后不同,主动脉内球囊反搏支持下急诊PCI治疗可以挽救部分患者的生命和改善预后,对于侧支循环不良的患者,部分再灌注策略可能为一种有效手段之一,值得进一步研究。  相似文献

Twenty-seven consecutive patients underwent surgery for ischemic mitral regurgitation (MR): papillary muscle rupture (1), papillary muscle dysfunction (11) and annular dilatation (15). The grade of MR was moderate or severe, and the ejection fraction (EF) was less than 30% in 8 patients (mean, 43%). Three cases were reoperation and 3 were emergencies. Under ventricular fibrillation (VF) and intermittent aortic cross-clamping at moderate hypothermia, coronary artery bypass grafting (CABG) was performed first, followed by the mitral procedure through a right-sided left atriotomy (repair 21, replacement 6) performed under VF with the heart perfused through the native coronary arteries and CABG grafts. Concomitant procedures were CABG (23), Dor's procedure (5), and tricuspid annuloplasty (3). In one reoperative case with cardiogenic shock, CABG was impossible because of dense adhesions and the patient died just after surgery (hospital mortality, 3.7%). Five patients required intra-aortic balloon pump (IABP) support intraoperatively, but none required prolonged (> or =7 days) inotropic support or IABP use, although the serum concentrations of creatine kinase and its myocardial fraction were elevated remarkably. Other morbid events were refractory ventricular arrhythmia in one case and stroke in another. Median duration of mechanical ventilation and intensive care unit stay was 8 h and 3 days, respectively. Mean EF at hospital discharge was 48%. The extended period of VF was not associated with unfavorable clinical outcomes. Noncardioplegic surgery for ischemic MR was carried out with acceptable mortality and morbidity, and can be a good alternative, especially when clamping the aorta is undesirable.  相似文献

The left internal mammary (LIMA) is an ideal conduit for bypassing the left anterior descending artery (LAD) during coronary artery bypass surgery (CABG). In this study, we evaluate the frequency of significant disease in the LIMA prior to CABG and describe associated significant lateral costal branches (> 1.5 mm), subclavian disease and vertebral disease. In 115 consecutive patients referred to CABG, 101 patients met the inclusion criteria. All patients who underwent routine visualization of the LIMA vessels were reviewed and quantitatively analyzed by an independent reader. Disease in the LIMA, left subclavian and vertebral arteries were graded and considered significant if lesions were > 50%. Lateral costal branches of the LIMA were also noted for their presence and size. Of the 101 patients, 87.1% of the LIMAs prior to surgery were without any disease. One LIMA had a 25-50% narrowing and the remaining LIMAs showed 0-25% disease. Of the LIMAs, 48.5% had lateral costal branches with diameters > 1.5 mm. The incidence of significant left vertebral disease and proximal subclavian disease was 37.6% and 5%, respectively. Cardiolite stress imaging post-CABG (n = 75) showed that when ischemia persisted in the anterior wall in patients with a LIMA to the LAD, this was not associated with the presence of significant LIMA disease, lateral costal branches, or subclavian disease identified pre-operatively. We conclude that the LIMA rarely shows significant disease on routine visualization prior to CABG. Asymptomatic subclavian artery disease and the presence of lateral costal branches pre-operatively did not correlate with post-CABG ischemia in the LAD territory.  相似文献

目的:总结重症患者在浅低温不停跳体外循环(extracorporeal circulation,ECC)下行冠状动脉旁路移植手术的效果。方法:2010年1月至2011年5月对34例择期行冠状动脉旁路移植术(coro-nary artery bypass grafting surgery,CABG)的重症患者(左主干病变、左心室舒张末期内径>60 mm、左心室射血分数<40%、术前Euroscore评分>6分、术前心源性休克、急性心肌梗死)施行浅低温ECC下不停跳CABG,术前均存在不稳定型心绞痛。其中10例(28%)合并糖尿病。3例(44%)患者术前应用了主动脉内球囊反搏(intra-aortic ballon pump,IABP)。分析总结重症CABG患者体外循环管理经验。结果:平均ECC时间68 min(42~123 min),机械通气时间平均18 h(6~84 h),ICU滞留时间平均2 d(14h~14 d),住院时间平均22 d(10~34 d)。12例患者术后应用IABP。无与ECC相关的神经系统并发症,肾功能不全6例(18%),并应用肾替代疗法。死亡2例(6%)。结论:对严重心功能不全或血管条件不好的重症CABG患者,采用浅低温不停跳ECC是一种安全可靠的手术方法。  相似文献

We describe coronary-subclavian steal restricting flow to the left internal mammary artery (LIMA) associated with critical aortic stenosis treated with combined percutaneous transluminal stenting and minimally invasive aortic valve replacement (AVR). An 86-year-old patient had coronary artery bypass graft placement (CABG) seven years prior with the LIMA anastomosed to the left anterior descending coronary artery (LAD). At the time of CABG, the patient had mild aortic stenosis and normal left ventricular function. By the time of re-presentation with refractory angina and heart failure, the patient had developed critical aortic stenosis. Because repeat CABG with median sternotomy risked damaging the LIMA, pre-operative revascularization was planned to minimize the likelihood of peri-operative ischemia. Stenting of the subclavian artery was performed prior to minimally invasive AVR.  相似文献

Preventing left ventricular (LV) remodeling after coronary artery bypass graft surgery (CABG) is important to avoid long-term congestive heart failure. The present study evaluated the effects of angiotensin converting enzyme inhibitors (ACEIs) and beta-blockers on LV remodeling. Twenty-three patients with angina pectoris and 36 with old myocardial infarction underwent CABG. We assessed end diastolic volume index (EDVI), end systolic volume index (ESVI), and ejection fraction (EF) using left ventriculography before and after CABG. Changes in EDVI, ESVI, and EF were studied in the ACEI, beta-blocker, and control groups. Although EDVI was reduced in the ACEI group, ESVI and EF improved only slightly, whereas in the group given beta-blockers, ESVI was reduced, EF improved, and EDVI was minimally reduced. These results indicate that ACEIs and beta-blockers both protect against LV remodeling, although through different mechanisms.  相似文献

Objective : To determine the feasibility of a hybrid coronary revascularization (HCR) approach for the treatment of left main (LM) coronary artery stenosis. Background : The recommended therapy for significant LM stenosis is coronary artery bypass grafting (CABG). Percutaneous coronary intervention (PCI) of unprotected LM lesions is reserved for patients at high risk for complications with CABG. HCR in LM disease has not been studied. Methods : Twenty‐two consecutive patients with LM stenosis >70% underwent staged HCR. Following a robotic or thoracoscopic‐assisted minimally invasive left internal mammary artery (LIMA) to left anterior descending artery (LAD) coronary bypass, PCI of the LM, and non‐LAD targets was performed after angiographic confirmation of LIMA patency. Intravascular ultrasound confirmed optimal stent deployment. Thirty‐day adverse outcomes and long term follow up was obtained. Results : In the 22 patients with LM lesions, 6 were ostial, 5 mid, and 11 distal. LIMA patency was FitzGibbon A in all cases. LM stenting was successful in all patients with drug‐eluting stents (DES) placed in 21 of 22 cases. Three patients underwent stent implantation in the right coronary artery. There were no 30‐day major adverse cardiac or cerebrovascular events. At a mean of 38.8 ± 22 months postprocedure, 21 patients were alive without reintervention; one death occurred at 454 days. Conclusions : HCR for LM coronary disease is a feasible alternative to CABG and unprotected LM PCI. This approach combines the long‐term durability of a LIMA‐LAD bypass with the less invasive option of PCI in non‐LAD targets with DES. © 2011 Wiley Periodicals, Inc.  相似文献

The aim of this study was to evaluate the effects of preoperative and postoperative statins on coronary artery bypass grafting (CABG) for extensive coronary artery disease as well as left main coronary stenosis (LMS). The data of 626 cases of extensive coronary artery disease as well as LMS patients in Anzhen Hospital between January 1998 and March 2008 for CABG procedure were retrospectively analyzed, and were classified as preoperative statin therapy group (Group A, n = 320) or preoperative no statin therapy group (Group B, n = 306). Propensity scores were estimated to determine the probability of inclusion into statin therapy group, resulting in the successful matching of 267 pairs. The incidence of in‐hospital death, and atrial fibrillation or flutter and disabling stroke was higher in Group B than in Group A. The actuarial freedom from late events at 5 yrs were 98.75%± 0.73% for the postoperative statin therapy group and 88.33%± 3.71% for the postoperative no statin therapy group respectively, P= 0.000. The logistic regression revealed that CRP (>5.0 mg/L), and elevated Troponin I, and emergent procedure, and preoperative IABP support, and EF < 40% were the independent risk factors, and preoperatively statins was the protective factor for the perioperative death; and the Cox proportional hazard also revealed that preoperative IABP support and preoperative cardiac arrest, and EF < 40% were independent risk factors, and postoperatively statins were the protective factor for the late cardiac events. Preoperative statin therapy could provide protective effect in the perioperative period. Postoperative statin usage could provide protective effect on the late cardiac events.  相似文献

BACKGROUND: To determine whether a coronary artery bypass graft (CABG) is patent, we examined the flow of the left internal mammary artery (LIMA) to the left anterior descending artery (LAD) by transthoracic Doppler echocardiography (TTDE). PATIENTS AND METHODS: Eighty-seven patients with CABG (LIMA to distal LAD) were enrolled in the study. The flows from each subject were analyzed by three criteria: mosaic flow at the anastomosis site, distal anterograde flow (ante flow), and proximal retrograde flow (retro flow). RESULTS: On angiography, 79 grafts were patent and eight were not. TTDE study of 79 patent grafts demonstrated mosaic, ante, and retro flow in 63 (79.7%), 74 (93.7%), and 35 grafts (49.4%), respectively. The averaged diastolic peak velocity of ante flow was 26.3 +/- 11.0 cm/sec, significantly higher than that (4.8 +/- 7.1 cm/sec, P < or = 0.0001) in eight patients without patent grafts. These eight patients had no mosaic or retro flow and only three had ante flow. The accuracies to predict patency were 81.6%, 90.8%, and 49.4% for mosaic, ante, and retro flows, respectively. CONCLUSIONS: The existence of mosaic, retro, or sufficient ante flows strongly indicated the patency of LIMA to the LAD. When symptoms are possible to be derived from the occlusion of CABG to LAD, TTDE is a promising method to examine whether a LIMA to LAD bypass is patent.  相似文献

The left internal mammary artery (LIMA) has been the conduit of choice in coronary artery bypass grafting (CABG). Atresia of the LIMA is a common finding after CABG in patients with less than critical disease of the native vessel. However, it is extremely rare for an atretic LIMA to restore its normal caliber after progression of native coronary artery disease. We report a patient in whom the LIMA was occluded 2 years after CABG, but was found to be completely patent 5 years later, after worsening of the left anterior descending artery disease.  相似文献

BACKGROUND: Cardiopulmonary bypass may exacerbate myocardial damage in compromised left ventricles. Early and mid-term outcomes of off-pump coronary artery bypass grafting (OPCAB) vs on-pump coronary artery bypass grafting (On-pump CABG) were compared in patients with poor left ventricular dysfunction, using an analysis of a propensity score matching. METHODS AND RESULTS: Between December 2000 and November 2005, 1,473 patients underwent isolated coronary artery bypass grafting in our institute and 153 patients who had a left ventricular ejection fraction (LVEF) lower than 35% were enrolled. The OPCAB group contained 100 patients and the On-pump CABG group contained 53 patients. Preoperative risk factors were compared and 50 patients in each group were matched. The mean follow-up time was 35.5+/-17.3 months. Three deaths (3.0%) occurred in the matched cohort, with no significant difference between 2 groups. The operation time, ventilation time, intensive care unit admission time and occurrence of respiratory failure were significantly lower in the OPCAB group. The mean LVEF of the 2 groups improved significantly. The overall 6-year actuarial survival rates of the OPCAB and On-pump CABG group were 88.2% and 72.4% (p=0.2), respectively, and there were no significant differences in 6-year rates of freedom from major adverse cardiac and cerebrovascular events (p=0.97). CONCLUSIONS: Coronary artery bypass grafting in patients with poor left ventricular dysfunction improved myocardial function. Postoperative respiratory failure was significantly related to the cardiopulmonary bypass for surgical myocardial revascularization. Off-pump and On-pump surgical revascularization resulted in equivalent mid-term outcomes.  相似文献

OBJECTIVE: To compare the efficacy of drug eluting stents (DES) compared with bypass surgery (CABG) with left internal mammary artery (LIMA) in patients with single vessel disease suffering from chronic stable angina. BACKGROUND: There are a limited number of studies investigating this group of patients. METHODS: We included 257 consecutive patients with isolated lesion in the proximal segment of left anterior descending artery (LAD). All patients suffered from chronic stable angina or from stress-induced ischemia. Of 257 patients, 147 underwent DES implantation and 110 CABG with LIMA. All patients were followed-up clinically for major adverse cardiac events. RESULTS: The baseline demographic and angiographic characteristics were similar between the two groups. In the DES group we used sirolimus-, paclitaxel-, and ABT-578-eluting stents. The mean duration of hospitalization after CABG was 7.86 +/- 3.84 days vs. 1.02 +/- 0.19 days after PCI (P < 0.01). The incidence of MACE was 2.72% in the DES and 2.72% in the surgical group during a mean follow-up period of 18.71 +/- 6.27 months for PCI and 18.70 +/- 7.31 months for CABG (P = 0.99). There was one cardiac related death in the DES group and two in the surgical group (P = 0.58). There were three reinterventions in the DES group versus none in the surgical group (P = 0.26). Recurrence of angina was observed in 4.08% of pts in the DES group versus 6.36% in the CABG group (P = 0.57). CONCLUSIONS: The present study demonstrated that patients suffering from chronic stable angina with isolated lesion in the proximal segment of LAD have excellent long-term outcome in both surgical and DES treatment.  相似文献
