The potential of magnetic resonance (MR) imaging for the evaluation and staging of bladder tumors was analyzed in 15 patients (11 cases of transitional cell carcinoma, two adenocarcinomas, one leiomyosarcoma, and one leiomyoma). Neoplasms were characterized by size, site, and growth pattern, and the accuracy of the staging was compared with the results of computed tomography and pathologic study. Malignancies were accurately detected and staged by MR imaging in 12 of 14 patients (85%). Tumor site and degree of bladder distention did not adversely affect detection; tumors greater than 1.5 cm were detected easily. In situ carcinoma (stage Tis) was not detected on MR images. Imaging in both sagittal and transverse planes was needed for optimal bladder evaluation. Bladder carcinoma was best displayed with a short echo delay time (TE) of 28 msec and repetition (TR) times of 1.0-2.0 sec: TR = 1.0 gave 34% contrast and TR = 2.0 gave 36% contrast between tumor and surrounding urine. Bladder-wall invasion by tumor was best evaluated with long TR (2.0 sec) and long TE (56 msec) (82% contrast). For assessing tumor extension into perivesical fat, short TR (0.5 sec) and TE (28 msec) were optimal (58% contrast). MR imaging offers an increased sensitivity for tumor detection and promises to greatly improve the staging of bladder neoplasms.  相似文献

To better understand the morphologic appearance of avascular necrosis (AVN) of the femoral head on magnetic resonance (MR) images (1.5 T) and computed tomographic (CT) scans, the records of 21 lesions were reviewed retrospectively. All MR imaging studies included T1-weighted images (T1WI) (repetition times [TR] of 400-1,000 msec, and echo times [TE] of 20-25 msec), and 15 included T2-weighted images (T2WI) (TR = 2,000-2,500 msec; TE = 60-80 msec). MR signal features of the lesions were compared with features on the corresponding CT scans. Abnormalities in the superoanterior aspect of the femoral head were noted on both image types in all 21 lesions but were more obvious on MR images in two. A characteristic margin of peripheral sclerosis seen on CT scans in 95% (20 of 21) of lesions corresponded to a line of low intensity on MR images. Fractures complicating AVN were seen in eight lesions at CT scanning. On T1WI, fractures were not clearly delineated. On T2WI, fractures were of high intensity but were depicted less clearly than on CT scans. Central signal intensity of the lesions on T1WI correlated with the presence or absence of fracture: 88% (seven of eight) of the lesions with fractures appeared less intense than fat, compared with only 8% (one of 13) of lesions without fractures (P less than .005). While MR imaging is a sensitive method for early diagnosis of AVN, CT scanning can more accurately identify fractures and is thus important for staging.  相似文献

The role of magnetic resonance (MR) imaging in the detection of gallbladder disease was evaluated in 39 individuals (16 healthy, five with asymptomatic gallstones, and 18 with clinical symptoms of gallbladder disease). MR imaging was performed after they fasted for 12 hours. Imaging sequences included a combination of repetition times (TR) of 0.5 and 1.5 sec and echo times (TE) of 28 and 56 msec. On the images obtained at TR = 0.5 sec and TE = 56 msec, gallbladder bile was hyperintense compared with the liver in all healthy and asymptomatic subjects and was hypointense (n = 9), isointense (n = 4), or hyperintense (n = 5) in symptomatic patients, eight of whom had surgical confirmation of cholecystitis. Comparison of normal versus pathologically proved cases for the presence of gallbladder disease yielded a specificity of 100%, sensitivity of 75%, and a significant difference of P less than .01. Thus, with a pulse sequence of TR = 0.5 sec and TE = 56 msec, MR was sensitive in detecting gallbladder disease. However, the role of MR in the radiologic workup of gallbladder disease will be determined by more experience with this modality.  相似文献

Radiographically negative avascular necrosis: detection with MR imaging   总被引:7,自引:0,他引:7  
To correlate the morphologic appearance on magnetic resonance (MR) images of radiographically negative avascular necrosis (AVN) of the femoral head with that on computed tomographic (CT) and radionuclide scans, the radiographic and clinical records of 24 patients were reviewed retrospectively. In 18 patients the MR signal intensity features were monitored by means of serial imaging. All MR studies included T1-weighted (short repetition time [TR], short echo delay time [TE] ) imaging and T2-weighted imaging (long TR, long TE). Thirty-one hips were determined with MR to be involved by AVN; 27 were staged on the basis of signal intensity characteristics within the low-intensity rim. Core decompression was performed on 18 hips. Afterward, progression of disease occurred in only one hip. Fourteen of the 16 asymptomatic patients (88%) had early-stage focal lesions. CT scans were obtained in 15 patients and radionuclide scans in 21. Ten hips at radionuclide imaging and five at CT appeared normal when MR results were distinctly abnormal. MR can depict early radiographically negative AVN in asymptomatic individuals. At this early stage, the lesions in this series appear to be nonprogressive after treatment.  相似文献

The authors evaluated the use of magnetic resonance (MR) imaging in diagnosis of avascular necrosis (AVN) of carpal bones by examining 21 patients with wrist pain and two healthy volunteers. MR images were compared with conventional radiographs in every case and with bone scintigrams in 18 cases. MR imaging was slightly less sensitive than bone scintigraphy in depicting AVN, but in patients who were imaged with long repetition time (TR)/long echo time (TE) sequences in addition to short TR/short TE sequences, MR imaging was found to be more specific. While the authors believe that bone scintigraphy remains the screening test of choice for patients with wrist pain and normal plain radiographs, MR imaging promises to add significant diagnostic information in cases in which bone scans are abnormal.  相似文献

Ten cases of hepatic focal nodular hyperplasia (FNH) were studied with magnetic resonance (MR) imaging. Proof of diagnosis was by needle biopsy or follow-up of as long as 5 years. Both short TR/TE and long TR/TE images were obtained. The most common finding was isointensity of the lesion relative to liver parenchyma with all pulse sequences. A central scar was seen in only one patient and was hyperintense on long TR/TE images. Slight hyperintensity of the lesion on short TR/TE and long TR/TE images was seen in two cases. Textural heterogeneity was present in only one of these. The most common presentation in our series was a visually isointense lesion relative to liver on short TR/TE images and an isointense or slightly hyperintense lesion on long TR/TE images. A review of the literature and the present findings suggest that FNH has a variable appearance on MR images.  相似文献

Magnetic resonance (MR) images of the pelvis in 63 women (40 healthy and 23 with various adnexal diseases) were assessed retrospectively. When imaged with contiguous sections without gaps, adnexa were demonstrated bilaterally in 13 of the 15 healthy women of reproductive age, but in only seven of the 15 healthy postmenopausal women. Normal adnexa demonstrated low to medium signal intensity on images obtained with short repetition time (TR) (0.5 sec) and echo delay time (TE) (28 or 30 msec). Their signal intensity approached that of fat on images with a long TR (2.0 sec) and TE (56 or 60 msec). The adnexal origin of the pelvic masses was correctly identified in every case. Lesions containing fluid with little or no protein, fat, or blood content (simple fluid) had characteristically long T1 and T2 relaxation times and low signal intensity on images obtained with a short TR (0.5 sec) and TE (28 or 30 msec); they could be readily differentiated from all the other types of lesions.  相似文献

Intraspinal synovial or ganglion cysts are uncommon lesions associated with degenerative lumbosacral spine disease. CT usually reveals cystic lesions adjacent to a facet joint, and they may show calcification. MR imaging of four surgically confirmed cases of intraspinal synovial cysts revealed subtle signal changes compared with CSF. Short TR/TE images showed the lesions to be slightly hyperintense in three cases and isointense in one case. Long TR/TE sequences revealed a hyperintense appearance in two cases and a hypointense appearance in the others. A peripheral rim of decreased signal on long TR/TE images probably reflects fine calcification or hemorrhage in the margins of the cysts. The multiplanar and contrast characteristics of MR make this technique well suited to the diagnosis of herniated disk, degenerative facet disease, and synovial cyst.  相似文献

Ten patients with intracerebral metastases from malignant melanoma were evaluated with magnetic resonance (MR) imaging performed at 1.5 T using spin-echo techniques. On the basis of histopathologic findings in three of 10 cases and CT appearances in all 10 cases, three patterns were identified on analysis of MR signal intensities in both short repetition time/echo time (TR/TE) and long TR/TE spin-echo scans. In comparison to normal cortex, nonhemorrhagic melanotic melanoma appeared markedly hyperintense on short TR/TE images and isointense, mildly hypointense on long TR/TE images. Nonhemorrhagic, amelanotic melanoma appeared isointense or mildly hypointense on short TR/TE and isointense or mildly hyperintense on long TR/TE images. Hemorrhagic melanoma varied in appearance, depending on the stage of hemorrhage. Melanotic, nonhemorrhagic melanoma can be distinguished from early and late subacute hemorrhage by its signal intensity on long TR/TE images. Spin-echo MR appears to be the method of choice for diagnosing melanotic metastases.  相似文献

Using a 0.35-T superconducting magnet and spin echo imaging, we prospectively evaluated 11 patients who had proved hepatic cavernous hemangioma. Magnetic resonance (MR) identified more lesions than either contrast-enhanced CT, or ultrasonography. The MR appearance was consistent; hemangiomas were homogeneous and generally isointense at short TR and TE intervals but were hyperintense at long TR intervals and greatly hyperintense at long TR and long TE intervals. However, the MR appearance of hemangioma was not specific; 2/14 other focal hepatic masses had similar features. The calculated relaxation times (T1, T2) were not useful in lesion characterization, although the intensity ratio of hemangioma to normal liver at the TR = 2.0 sec TE = 56 msec pulse sequence was useful in diagnosis since hemangiomas always had a ratio greater than 1.4.  相似文献

The MR features of 20 extracranial hematomas studied on a 1.5-T system and imaged with both short repetition-time/echo-time (TR/TE) and long TR/TE pulse sequences were reviewed. In four of five acute hematomas (those less than 7 days of age), signal intensity was markedly decreased on long TR/TE images and was either intermediate or slightly decreased on short TR/TR images. Fourteen subacute hematomas (7 days to 7 weeks of age) and one chronic hematoma (9 months) were studied. The appearance of the subacute lesions varied from intermediate to high intensity on short TR/TE sequences, but all demonstrated increased signal on long TR/TE sequences. A low-signal rim was noted at the margin of nine subacute lesions. In one patient with this finding, pathologic examination showed that the low-signal margin corresponded to a region containing hemosiderin-laden macrophages at the periphery of the hematoma. These results correlate well with those reported for intracranial hematomas examined at this field strength. We conclude that analysis of signal-intensity patterns at 1.5 T is useful in staging the evolution of hematomas.  相似文献

The magnetic resonance appearance of three lesions of focal nodular hyperplasia observed in two patients is reviewed. All three lesions demonstrated isointensity with the liver on the short repetition time (TR) and echo time (TE) spin echo images. One lesion also showed a central area of low signal intensity that pathologically corresponded to a scar. The long TR and TE images had a varied appearance, from mixed high signal intensity to predominantly isointense with the liver. The histopathology for each lesion was carefully reviewed. The primary microscopic difference between the lesion that was predominantly isointense with liver on the long time TR and TE images and the other two, which were of mixed high signal intensity on the long time TR and TE images, was the former's relative lack of fibromuscular obliteration of septal blood vessels in the scar. Focal nodular hyperplasia can have at least two possible appearances at 1.5 T; in these three lesions there was consistent isointensity with the liver on the short TR and TE images.  相似文献

Magnetic resonance imaging of the adrenal glands was performed in 9 patients with Addison's disease to evaluate the role of magnetic resonance (MR) in this entity. All patients had bilateral adrenal masses demonstrated by computed tomography (CT); etiologies included adrenal hemorrhage (2 patients), granulomatous disease (1 patient), adrenal lymphoma (3 patients), and adrenal metastases (3 patients). Spin-echo axial images were obtained at repetition times (TR) 0.5, 2.0 s and TE 28, 56 ms, using a Diasonics superconducting magnet operating at 0.35 T. In the patients with lymphoma, metastases, and granulomatous disease, the adrenal masses appeared hypointense or isointense with liver on the T1-weighted images (TR 0.5 s, TE 28 ms). In cases of adrenal hemorrhage, areas of hyperintensity were seen on TR 0.5, TE 56 ms sequences, due to shortening of T1 values. In both groups of patients the masses were hyperintense on T2 weighted sequences. Mean calculated T1 of the hemorrhagic glands was 449 ms, compared with a mean of 782 ms for metastases and lymphoma. While MR is not capable of distinguishing between acute inflammatory and metastatic diseases of the adrenal glands, it may be equally efficacious as CT in suggesting the diagnosis of adrenal hemorrhage in patients with Addison's disease.  相似文献

The Tolosa-Hunt syndrome consists of painful ophthalmoplegia caused by cavernous sinus inflammation, which is responsive to steroid therapy. The MR features of 11 patients with the clinical diagnosis of Tolosa-Hunt syndrome were studied. Two patients had normal MR studies of the orbit and cavernous sinuses. In nine patients, abnormal signal and/or mass lesions were seen in the cavernous sinuses; in eight cases, the abnormality was hypointense relative to fat and isointense with muscle on short TR/TE images and isointense with fat on long TR/TE scans. Extension into the orbital apex was seen in eight cases. In six of nine cases the affected cavernous sinus was enlarged; in five of nine it had a convex outer margin. One patient had a thrombosed cavernous sinus and superior ophthalmic vein in addition to a cavernous sinus soft-tissue mass. The signal intensity of Tolosa-Hunt syndrome in this limited series was similar to that of orbital pseudotumor and is confined to a limited differential diagnosis, which includes meningioma, lymphoma, and sarcoidosis.  相似文献

The MR images and CT scans of 14 patients with surgically verified pyogenic cerebral abscesses were reviewed. The MR findings correlated well with those seen on CT and were believed to be sufficiently characteristic to allow early and accurate diagnosis with MR alone. These features include (1) peripheral edema producing mild hypointensity on short TR/short TE and marked hyperintensity on long TR/intermediate to long TE scans; (2) central necrosis with abscess fluid hypointense relative to white matter and hyperintense relative to CSF on short TR/short TE scans and hyperintense relative to gray matter on long TR/intermediate to long TE scans (the fluid had concentric zones of varying intensity in seven cases, a finding not previously identified in other lesions); (3) extraparenchymal spread (intraventricular or subarachnoid), which was detected more easily on MR than on CT and was manifested by increased intensity relative to normal CSF on both short TR/short TE and long TR/intermediate TE scans; and (4) visualization of the abscess capsule, which was iso- to mildly hyperintense relative to brain on short TR/short TE scans and iso- to hypointense relative to white matter on long TR/intermediate to long TE scans. On the long TR scans, the relative hypointensity of the rim allowed for visualization of the typical morphologic features of the capsule, which in turn aided in differentiation of abscesses from other lesions (as it does on CT). To investigate the cause of the capsular intensity, pathologic studies of the capsules were reviewed when available (10 cases). Fibrosis was identified in all mature abscess capsules, but the combination of the intensities seen on short TR/short TE and long TR/intermediate to long TE scans as well as the temporal changes in intensity were believed to be incompatible with fibrosis as a cause of the capsular changes. Intensity patterns were suggestive of hemorrhage, but neither acute nor chronic hemorrhage was identified on routine H and E stains, while iron stain revealed scant hemorrhage in only two of the eight patients in whom these stains were used. We believe the capsular intensity (in particular the hypointense rims on long TR scans) may reflect paramagnetic T1, and to a greater extent T2, shortening, possibly due to the presence of heterogeneously distributed free radicals that are products of the respiratory burst produced by actively phagocytosing macrophages in the capsule wall. Distinctive MR features of pyogenic abscesses should afford early and accurate diagnosis.  相似文献

Multiple sclerosis: serial study of gadolinium-enhanced MR imaging   总被引:4,自引:0,他引:4  
Thirteen patients with definite multiple sclerosis (MS), studied 16-24 months previously with magnetic resonance (MR) imaging with and without enhancement by intravenously administered gadolinium diethylenetriaminepentaacetic acid (DTPA) dimeglumine, were reexamined with a similar protocol. Assessment of enhancement and clinical activity in both studies revealed that enhancement was observed in 13 of 14 cases in which clinical activity had changed within 4 weeks of the study and thus appeared more sensitive than clinical examination in determining active disease. The 3-minute postinjection, short repetition time image (TR) was the most efficient for depicting enhancement. Enhancing lesions (active plaques) arose from previously hyper- or isointense regions on long TR images. Previously active lesions reverted to areas of iso- or hyperintensity on long TR images. Serial comparison of long TR images in this population reveals a decrease in high-intensity lesions on long TR images in some cases and an increase in others. The findings of high-intensity regions on long TR images and previously enhancing lesions both becoming isointense suggests that transient inflammatory changes with concomitant edema without demyelination and/or with significant remyelination may occur in some MS lesions. MS lesions are dynamic; both active and inactive lesions may show dramatic change on longitudinal MR imaging studies.  相似文献

Combined gadolinium-enhanced and fat-saturation MR imaging of renal masses   总被引:4,自引:0,他引:4  
Combined gadopentetate dimeglumine enhancement and fat-saturation (FS) spin-echo (SE) magnetic resonance (MR) imaging for the detection and characterization of renal masses was evaluated in 43 patients with a total of 71 lesions (28 solid masses and 43 cysts). SE MR sequences compared were the following: short repetition time (TR)/echo time (TE), conventional SE, short TR/TE FS SE, long TR/TE conventional SE, gadolinium-enhanced short TR/TE conventional SE, and gadolinium-enhanced short TR/TE FS SE techniques. MR findings were compared with findings of contrast-enhanced computed tomography (CT) and with pathologic findings in all patients. The sensitivities for detection of renal masses with gadolinium-enhanced FS (71 of 71 lesions) and with gadolinium-enhanced short TR/TE conventional (65 of 71 lesions) SE sequences were significantly (P less than .01) greater than with any unenhanced (short TR/TE conventional [40 of 71 lesions], or long TR/TE [39 of 71 lesions]) SE sequence. Lesion characterization was also best with the gadolinium-enhanced FS SE sequence (65 of 71 lesions correctly classified). When combined pre- and postcontrast short TR/TE FS SE images were analyzed with both qualitative (visual) and quantitative (region-of-interest measurements) assessment, lesion characterization improved even further (70 of 71 lesions were correctly characterized). All lesions detected with CT were visualized with the gadolinium-enhanced FS SE MR sequence, which in addition depicted seven cysts and two small renal cell carcinomas. In summary, the use of gadopentetate dimeglumine, especially when combined with the FS technique, was superior to unenhanced MR imaging for detection and characterization of renal lesions.  相似文献

Pigmented villonodular synovitis: a report of MR imaging in two cases   总被引:3,自引:0,他引:3  
Although magnetic resonance (MR) imaging has been used to evaluate many musculoskeletal lesions, the MR appearance of pigmented villonodular synovitis (PVNS) has not been described in detail. The authors describe two cases of PVNS in the knee imaged with both computed tomography and MR. In both cases parts of each lesion had very low signal intensity on both short repetition time (TR)/echo time (TE) sequences and long TR/TE sequences. Other portions of both lesions had intermediate signal intensity (equal to or higher than that of muscle but lower than that of fat) on short TR/TE sequences and increasing signal intensity on longer TR/TE images. In one case, the lesion also had a cystic component that showed MR changes typical of complex fluid. The authors propose that the MR signal characteristics demonstrated in these cases may be explained by the unique tissue components of the lesion, particularly hemosiderin and fat.  相似文献

We retrospectively reviewed the routine spin-echo MR studies of the brain in 12 patients with 13 angiographically demonstrated occlusions and in 14 patients with 16 high-grade stenoses of the carotid arteries. Intraluminal signal that was isointense with adjacent brain on long TR/short TE and long TR/long TE images was 100% specific for atherosclerotic occlusion. Of the 13 proved occlusions, six (46%) had significant degrees of hyperintense intraluminal signal indistinguishable from that observed consequent to slow flow distal to high-grade stenoses. MR detected only five (31%) of the 16 proved high-grade stenoses. Normal flow void does not exclude significant extracranial carotid stenosis. Occlusion cannot always be distinguished from high-grade stenosis when hyperintense intraluminal signal is encountered. However, a reliable diagnosis of atherosclerotic occlusion can be made when isointense intraluminal signal is observed.  相似文献
