A case of gallstone obturation of the duodenal bulb is presented. The general topic of gallstone ileus is briefly discussed and a review of the reported cases of Bouveret's syndrome is presented. The radiographic criteria for diagnosis of these entities are emphasized.

The case of an asymptomatic individual with nodular lymphoid hyperplasia and Ig-A deficiency is presented. His other serum immunoglobulins were in the high normal range and his saliva contained high concentration of Ig-M in the absence of Ig-A, suggesting a compensatory immunological mechanism.

Recent advances in immunology have permitted recognition of a group of patients who have gastrointestinal manifestations as part of an immunoglobulin deficiency syndrome. Such immunoglobulin deficiency may be primary or may be secondary to a variety of diseases. We have classified and described the small bowel roentgen features associated with the various immunoglobulin deficiency syndromes as follows: 1. the sprue pattern, as seen in hypogammaglobulinemic sprue and in Ig-A deficient sprue; 2. multiple nodular defects; 3. inflammatory changes secondary to giardiasis, associated with immune deficiency diseases; 4. thickening of the small intestinal folds, as seen in the plasma cell dyscrasias, lymphoma, intestinal lymphangiectasia and amyloidosis.

This case serves to illustrate the multiple routes which may occur in the dissemination of carcinoma of the stomach of the large bowel. Spread along the gastrocolic ligament involves the superior aspect of the transverse colon with limitation laterally by the phrenicocolic ligament. Once ascites has developed there is spread along the superior border of the sigmoid and lateral margin of the right side of the colon.

Radiographic manifestations of the colon on barium enema examination in two cases of antibiotic-induced pseudomembraneous colitis are presented. The radiographic findings are ulcerations, shaggy edematous mucosa and excessive mucus secretion. Although these roentgenorgraphic findings could not be differentiated from other inflammatory bowel disease, the antibiotic-induced enterocolitis must be included in the differential diagnosis whenever there is a clinical history of recent antibiotic therapy.
