Intracranial arachnoid cysts are usually non-symptomatic. Intracystic and subdural haematomas induced by even minor head injury may turn an asymptomatic AC into a symptomatic one, necessitating surgical treatment. We present a case of a previous asymptomatic AC, spontaneously complicated with subdural hygroma and development of intracystic and subdural haematoma. Clinical follow-up and control CT regime of patients with AC are recommended. 相似文献

Post-traumatic or spontaneous rupture of an arachnoid cyst resulting in a subdural haematoma is rare. Much more rarely, a ruptured arachnoid cyst may be present with a subdural CSF collection without evidence of haemorrhage. These are most commonly seen in the middle cranial fossa, where arachnoid cysts occur most frequently. In this paper, five teenage patients (four male, one female) are reported with post-traumatic or spontaneous arachnoid cyst ruptures, resulting in subdural haematomas in four patients and a subdural CSF collection without haemorrhage in one patient. Possible pathogenesis of the condition is discussed. 相似文献

Arachnoid cysts are collections of cerebrospinal fluid within the arachnoid membrane. They are benign lesions and most are congenital. They are usually asymptomatic; however, they can bleed and become symptomatic. Although arachnoid cyst associated to intracystic hemorrhage and spontaneous subdural hematoma is an uncommon complication, it is a well-known indication for emergency neurosurgery. We present the findings in a ten-year-old boy diagnosed with arachnoid cyst complicated by intracystic hemorrhage and spontaneous subdural hematoma diagnosed by magnetic resonance. 相似文献

subdural haematomas following minor trauma. The aetiology, pathogenesis and differential diagnosis are discussed.

Described are 26 cases of arachnoid cysts. Of these cysts, 54% were in the sylvian cistern, three (1.2%) were in the suprasellar cistern, and the rest were over the cerebral sulci. There were symptoms of seizures at presentation in 30.8%, and in 23% there were symptoms of hydrocephalus and compression of the brain. A total of 61.5% presented with mental retardation and/or cerebral palsy. Trauma and infection accounted for 35% of these cysts. Associated porencephaly were present in five cases. There was a male preponderance. Left-sided lesions accounted for 61.5% of the lesions. The radiologic appearances, especially of suprasellar cysts, and the differential diagnoses of these lesions are discussed. Their recognition and the role of surgery are stressed. 相似文献

14 patients with subdural haematoma (SDH) diagnosed by ultrasonography (US) and confirmed by computerized tomography (CT) were reviewed. We describe the morphology of SDH on US and discuss the ultrasound findings which may suggest outcome. SDH appeared as a crescentic mass overlying the cerebral hemisphere and producing medial displacement of the Sylvian fissure, midline shift, ventricular compression, sutural diastasis, changes in the sulcal pattern and echogenicity of the ipsilateral side of the brain. Hydrocephalus was the most common complication. It was the presence of cerebral complications and not the midline shift or ventricular compression which correlated with poor outcome. 相似文献

We report a patient with calcification of the convexity dura mater and an acute epidural haematoma. CT revealed a calcified layer between the haematoma and brain parenchyma, which mimicked acute bleeding into a calcified chronic subdural haematoma. The appearance of a calcified haematoma does not always mean a chronic lesion, and that emergency operation should not be foregone, when there is a history of acute head trauma and progressive impairment of consciousness. 相似文献

Two cases of chronic subdural haematoma following a snowboard head injury are reported. Although such cases are rare in sport, the risk in snowboarders is higher than expected. Evaluation of a snowboarder with a history of head injury, albeit mild, who complains of headaches should include computed tomography or magnetic resonance imaging to allow rapid identification of any intracranial pathology. 相似文献

The authors report on 117 consecutive hemorrhagic breast cysts cases. All patients underwent cytologic examination of cystic fluid content and US or pneumocystography. Ten cancers (9 infiltrating, and 1 lobular in situ) and 22 papillomas had histologic confirmation--out of 45 cases undergoing surgical biopsy. Seventy-two cases exhibiting no intracystic growth have been followed for 5.4 years on the average and no cancer has developed so far. Pneumocystography or US had 100% predictive value for intracystic lesions; we observed that surgical biopsy could be safely avoided when both examinations were negative, that is in about 3/4 cases. No differential diagnosis of cancer and papilloma was possible on pneumocystography or US. Even though positive cytology was highly predictive for cancer, cytology exhibited poor sensitivity and surgical biopsy was necessary in all cases presenting with intracystic growth. 相似文献

A 16-year-old male high school student with a history of fever (38-39 degrees C) for 4 days lost consciousness following the end of a three round boxing match. He was transferred to a neurosurgical unit located 48 km away 1 h 36 min after injury. Clinical assessment at the unit revealed a comatose patient with a left acute subdural haematoma, but because of advanced brain oedema surgical management was deemed futile. At autopsy an acute subdural haematoma and a severe brain oedema were confirmed. Evaluation of physical conditions before the fight should have been more rigorous in our patient. The observations in our case support the published literature that boxing matches should be held only where neurosurgical expertise is readily available. 相似文献

We present MRI findings in three patients with acute spontaneous subdural haematomas of the spine. Acute haematomas (1–3
days) were isointense or gave slightly high signal on T1- and heterogeneous signal on T2-weighted images. MRI precisely defined
the level and extent of the haematoma preoperatively. The MRI was prospectively correctly interpreted as acute subdural haematomas
in all patients. As a specific, noninvasive modality, MRI is the preferred imaging technique in this rare clinical entity.

The authors report a 39-year-old woman who presented with intermittent, excruciating nuchal and occipital headache. Magnetic resonance imaging and magnetic resonance angiography scans showed bilateral subdural haematomas with veno-occlusive disease of the superficial and deep venous systems. There were bridging collaterals with scalp veins, bleeds from which could explain the subdural haematoma. There was acute on chronic veno-occlusive disease with an acute rise in intracranial pressure and a bleed from the vein of Galen. 相似文献

Bilateral subdural haematoma is not a rare disorder, but only a few cases diagnosed at brain scintigraphy have been reported. Bilateral subdural haematoma was demonstrated at scintigraphy in a 66-year-old man. Subsequent computerized-tomography scanning of the brain also demonstrated the lesion. After bilateral burr-hole aspiration, the patient made a good recovery. 相似文献
