For two reasons, communication is one of the major tools in the fight against any Ebola epidemics. Firstly, because Ebola is one of the most easily preventable of all infectious diseases and the thorough application of health-protection measures by the community of the sick persons is the best tool to fight any Ebola epidemic. Secondly, because during the two dozens of known Ebola epidemics health care workers have often met with people’s skepticism, or even hostility. However, our review of Ebola communication, as defined by WHO since 2013, shows that it has been marked by a series of errors, as well from a strategic perspective as in its concrete deployment. The same communication messages and tools have been used in non-epidemic and epidemic countries. A general ban on hunting has been promoted, while only 2% of sub-Saharan Africans live in areas inhabited by the bats that are the reservoir of the Ebola virus and while it is not proven that hunting is a major risk of infection. Erroneous or inappropriate messages have contributed to doubts and created anxiety. To be effective, Ebola communication should be based on education about the disease, meaning explanation of its cause, its transmission and its prevention.  相似文献

Developed countries issue recommendations regarding healthcare that aren't constantly appropriate for emergent countries. We suggest some remarks concerning rheumatology in Madagascar, taking account of scientific data, medical ethics, equality and equity. We have studied the minimal cost of care of medical conditions found in our hospital department if we were to follow international recommendations for their management. Then, we have estimated treatment expenses as a percentage of the SMIC (Malagasy minimum monthly salary). Out of 517 patients examined yearly, we have found 62.8% osteoarthritis cases, 6.3% rheumatoid arthritis (RA), and 4,2% septic arthritis. Therefore, the first month of treatment for an arthritis of the knee would absorb 147.3% of the SMIC; diagnosis and treatment of a case of septic arthritis would take up 1762.8% of the minimum wage, and a case of RA without biotherapy would require 175%. According to the American College of Rheumatology criteria which are used as a reference, the treatment of an arthritis of the knee would take only 23% of the SMIC. Caring for septic arthritis would demand 57.5% of the SMIC and while it would yield more arguments for diagnosis such as clinical examination, CRP, and Gram coloration on joint liquid aspiration. We can proceed to RA diagnosis with an acceptable security through precise clinical examination, blood cell count, ESR, CRP, rheumatoid factor and radiography. This means 56% of the SMIC. From this 517 patients, our suggestions would reduce the expense by 35,850% of the SMIC per year. The allocation of such funds onto the treatment of complicated forms of rheumatism would be fair. By refining and evaluating these suggestions, we would come up with appropriate recommendations for emergent countries.  相似文献

The aim of this study was to describe the malaria morbidity and the frequencies of molecular markers of resistance to chloroquine and sulfadoxine-pyrimethamine in pregnant women at delivery in Mali. Two hundred pregnant women have been included at the delivery clinic in Fana. The age group of 14–19 years was predominant. Fifty two per cent (52.3%: 104/200) were malaria slides positive in their peripheral blood and 15% (30/200) of the women carried parasite in their placenta. The prevalence rate of anemia was 44.5% (89/200). PCR technique was successfully performed on 16 paired samples. The frequency of the Pfcrt K76T mutants in Plasmodium falciparum infections in peripheral blood was 68.8% (11/16) and 100 % (16/16) in the placenta (p = 0.004). The frequency in peripheral blood of the DHFR N51I mutation was 12.5% (2/16) and 18.8% (3/16) in the placenta (p=0.12). The frequencies of the DHPS A437G mutants were similar in both sites 25% (4/16). No DHPS K540E and DHFR 164L mutations were found in the Fana pregnancy women samples.  相似文献

As a follow-up to the first AfroREB (Africa Rabies Expert Bureau) meeting, held in Grand-Bassam (Côted’Ivoire) in March 2008, African rabies experts of the Afro-REB network met a second time to complete the evaluation of the rabies situation in Africa and define specific action plans. About forty French speaking rabies specialists from Northern, Western and Central Africa and Madagascar met in Dakar (Senegal), from March 16th to 19th, 2009. With the participation of delegates from Tunisia, who joined the AfroREB network this year, 15 French speaking African countries were represented. Experts from the Institut Pasteur in Paris, the Alliance for Rabies Control, and the Southern and Eastern African Rabies Group (SEARG, a network of rabies experts from 19 English speaking Southern and Eastern African countries) were in attendance, to participate in the discussion and share their experiences. AfroREB members documented 146 known human rabies cases in all represented countries combined for 2008, for a total population of 209.3 million, or an incidence of 0.07 cases per 100,000 people. Even admitting that the experts do not have access to all reported cases, this is far from the WHO estimation of 2 rabies deaths per 100,000 people in urban areas and 3.6 per 100,000 in rural Africa. It was unanimously agreed that the priority is to break the vicious cycle of indifference and lack of information which is the main barrier to human rabies prevention.  相似文献

Although systematically denied, that obligation is one of the main sources of sociability.
