Sleep apnea (SA) is a common breathing disorder. It is associated with a myriad of medical conditions including increased cardiovascular morbidity and mortality. Recent studies have shown that cardiac arrhythmias and conduction disorders are common in patients with SA. Sleep apnea has also been also linked to heart failure, hypertension, coronary artery disease, and stroke. The purpose of this brief review is to analyze the available information that links SA with different cardiac arrhythmias and conduction disorders and the role of intracardiac devices for the diagnosis and management of this condition. 相似文献

Arousal from sleep produces transient increases in systemic blood pressure, leading to the suggestion that repeated arousals are associated with a sustained increase in daytime blood pressure. Using data from the Wisconsin Sleep Cohort Study, a population-based study, we tested the hypothesis that sleep fragmentation is associated with elevated awake blood pressure. Sleep, breathing, and seated blood pressure measurements from 1,021 participants (age 42 +/- 8 yr; 590 males) were analyzed. Sleep fragmentation was defined as the total number of awakenings and shifts to Stage 1 sleep divided by the total sleep time (sleep fragmentation index: SFI). To reduce the confounding influence of sleep-disordered breathing, which is related to both increased daytime blood pressure and sleep fragmentation, all participants with an apnea-hypopnea index (AHI) > or = 1 were analyzed separately. Accounting for the influences of sex, age, body mass index, and antihypertensive medication use, the SFI was significantly associated with higher levels of awake systolic blood pressure in people with an AHI < 1; a 2 standard deviation increase in the SFI was associated with a 3.1 mm Hg rise in awake systolic blood pressure. In participants with an AHI > or = 1, there was no independent association between the SFI and awake blood pressure after controlling for the influence of the AHI. 相似文献

The objective of this study was to evaluate the effects of nocturnal sleep, partial night sleep deprivation, and sleep stages on circulating concentrations of interleukin-6 (IL-6) in relation to the secretory profiles of GH, cortisol, and melatonin. In 31 healthy male volunteers, blood samples were obtained every 30 min during 2 nights: uninterrupted, baseline sleep and partial sleep deprivation-early night (awake until 0300 h). Sleep was measured by electroencephalogram polysomnography. Sleep onset was associated with an increase in serum levels of IL-6 (P < 0.05) during baseline sleep. During PSD-E, the nocturnal increase in IL-6 was delayed until sleep at 0300 h. Sleep stage analyses indicated that the nocturnal increase in IL-6 occurred in association with stage 1-2 sleep and rapid eye movement sleep, but levels during slow wave sleep were not different from those while awake. The profile of GH across the 2 nights was similar to that of IL-6, whereas the circadian-driven hormones cortisol and melatonin showed no concordance with sleep. Loss of sleep may serve to decrease nocturnal IL-6 levels, with effects on the integrity of immune system functioning. Alternatively, given the association between sleep stages and IL-6 levels, depressed or aged populations who show increased amounts of REM sleep and a relative loss of slow wave sleep may have elevated nocturnal concentrations of IL-6 with implications for inflammatory disease risk. 相似文献

Bronchial asthma, allergic rhinitis and chronic obstructive pulmonary disease (COPD) are chronic diseases affecting millions of people around the world. Sleep plays an important modulatory role for these diseases. Circadian variations of respiration and pulmonary function may cause hypoventilation in COPD patients and may worsen asthmatic symptoms during sleep. Sleep disordered breathing is highly prevalent in all of these diseases. Finally, asthma, COPD and allergic rhinitis can contribute to the development of insomnia via multiple mechanisms. This article focuses on the complex relationships between these diseases and sleep and reviews the implications for diagnosis and therapy. 相似文献
