目的探讨轴性高度近视合并原发性闭角型青光眼的临床特点及发病机制。方法结合文献对一例(2眼)轴性高度近视合并原发性闭角型青光眼患者的临床资料进行分析。结果根据术前检查结果,右眼行虹膜周切术,左眼行复合式小梁切除术,术后矫正视力右眼0.4,左眼0.3;左眼上方结膜滤过泡扁平弥散;眼压:右眼14.57mmHg,左眼10.24mmHg。结论轴性高度近视发生的闭角型青光眼,可能与眼局部解剖结构的变异、脉络膜的神经血管因素以及高度近视患者的悬韧带松弛及晶状体的稳定性差等因素有关;常规抗青光眼手术可获得较好的治疗效果。  相似文献

PURPOSE: We describe a 39-year-old Japanese woman with chronic angle closure glaucoma secondary to spherophakia and frail zonular fibres. The patient was 143 cm in height with short fingers and had no family history of eye problems. High intraocular pressure, total optic disc cupping and severe visual field constriction were found in the right eye. METHODS: The patient was treated successfully with trabeculectomy in the right eye and laser iridotomy in the left eye. CONCLUSION: The clinical findings imply that this was a borderline case of Weil-Marchesani syndrome.  相似文献

BACKGROUND: We report a new case of oculo-dento-digital syndrome. METHODS: Case report. RESULTS: We saw a 34-year-old women with oculo-dento-digital syndrome. Visual acuity was no perception of light (RE) and 1/35 (LE). Biomicroscopy revealed a flat anterior chamber and an advanced cataract. The intraocular pressure (IOP) was 60 mmHg in both eyes. Ultrasonographic biomicroscopy demonstrated a closed chamber angle in both eyes. Measurements of the axial length and of the diameter of the lens were 18.7 mm/6.0 mm for the RE and 18.7 mm/5.8 mm for the LE respectively. In the time following we conducted a cyclodestructive procedure in the RE and a cataract extraction with implantation of an intraocular lens in the LE. This led to a considerable reduction of the IOP in the RE and combined with local therapy to IOP regulation in the LE. CONCLUSION: In this patient nanophthalmos, cataract and spherophakia led to angle-closure glaucoma in both eyes. We recommend early monitoring of IOP, axial length and lens diameter. This case demonstrates that an early cataract extraction might beneficially influence the natural course of the disease.  相似文献

BACKGROUND: Sturge-Weber syndrome has been known to be frequently associated with facial cutaneous angioma and ipsilateral glaucoma. However, as far as we know, no cases accompanied by acute angle-closure glaucoma have been reported in patients with Sturge-Weber syndrome. CASE: A 14-year-old boy with unilateral acute angle-closure glaucoma secondary to posterior scleritis associated with Sturge-Weber syndrome is described. OBSERVATIONS: Slit-lamp examination revealed diffuse episcleral venous hemangioma in the right eye. With ultrasound biomicroscopy, a forward shift of the lens-iris diaphragm, a swelling of the ciliary body, and an anterior rotation of the ciliary processes with annular choroidal effusion were detected. The patient responded well to treatment with systemic corticosteroids and cycloplegics. CONCLUSIONS: In our patient, inflammatory changes of the sclera, including swelling of the ciliary body, choroidal effusion, an anterior rotation of the ciliary processes at the scleral spur, and swelling of the lens, leading to closure of the anterior chamber angle, were suggested to be the major mechanisms of intraocular pressure elevation.  相似文献

A poor general condition of patients with hemorrhagic fever and elevated creatinine should raise suspicion of a systemic infection with Hantavirus. This can often also cause ocular changes. Above all, changes of intraocular pressure, in our case bilateral acute angle-closure glaucoma, due to edema and hemorrhage in the ciliary body result in anterolateral rotation of the iris-lens diaphragm with closure of the chamber angle. It can also cause a transient myopia, and intraretinal hemorrhages can also occur. Hantavirus infection is mostly transmitted through red-backed voles in Europe. The predominant type in Europe is the Puumala virus that can lead to an epidemic nephropathy with high fever, headache, gastrointestinal complaints, and an impaired renal function to the point of renal failure. No specific treatment is needed, but adequate symptomatic therapy is of utmost importance. When the infection has been overcome, complete recovery is possible. Measures for prophylaxis should be taken with respect to contact with mice and their excreta.  相似文献

A 55-year-old woman was referred to our clinic because of a one-week history of visual loss and raised intraocular pressure in the left eye followed 4 days later by visual loss in the right eye. Slit-lamp examination showed bilateral conjunctival hyperemia, slight diffuse corneal edema, shallow anterior chamber and fixed and dilated pupil in both eyes. Splitting of the anterior layers of the iris with fibrillar degeneration extending for approximately one quadrant inferiorly was presented in each eye. Fundus examination showed optic disc edema with no vascular tortuosity and no cup in both eyes. The condition was treated as bilateral acute angle-closure glaucoma in a patient with irisdoschisis. After medical treatment and improvement of visual acuity, perimetry revealed a significant visual field defect especially in left eye; this case represents a rare concurrence of acute angle-closure glaucoma and bilateral nonarteritic ischemic optic neuropathy. Although most cases of elevated intraocular pressure, including acute angle-closure glaucoma, do not result in optic disc edema and irreversible vision loss, variations in the vascular supply of the nerve optic head along with others ocular systemic risk factors, may predispose certain individuals to nonarteritic ischemic optic neuropathy during periods of elevated intraocular pressure.  相似文献

Diabetes mellitus contributes to an earlier and a quicker development of opacities of the lens and worsening of the vision in affected people, in comparison with healthy people in similar age. The main source of energy in the human lens is glucose, which is metabolised through the glycolytic process. When the level of glucose increases in blood, its content also increases in aqueous humour and in the lens. It causes a saturation of the glycolytic process and the activation of the sorbitol pathway with participation of aldose reductase. The accumulation of sorbits responsible for the growth of osmotic pressure inside the lens, inflow of water and gradual swelling of its fibres. In extreme cases a swelling of the lens can even lead to occurrence of cataracta intumescens and secondary glaucoma. The primary treatment of cataracta intumescens is to remove lens surgicaly, preferably after lowering intraocular pressure. In that report is presented a particular case of such, fulminating course of ophthalmological complications in patient with diabetes mellitus type 1.  相似文献

Purpose

To report a case of bilateral acute angle-closure glaucoma after oral administration of cabergoline for the treatment of galactorrhea.

Methods

A diagnosis of secondary drug-induced angle-closure glaucoma was made in a patient with elevated intraocular pressure (IOP) and myopic refractive shift, which was confirmed by ultrasound biomicroscopy (UBM) of the ciliary body and anterior segment, sonography, and optical coherence tomography. The treatment included the discontinuation of the culprit drug and the administration of topical anti-glaucoma drops. The treatment course was followed with serial measurements of the IOP and refraction, and with performing UBM.

Results

Five hours after he received a single 0.5-mg oral cabergoline tablet, the patient suffered from acute secondary angle-closure glaucoma and myopic refractive error. UBM demonstrated both effusion of the ciliary body and an anterior rotation of the iris-ciliary body. IOP was reduced 8 h after cessation of the causative agent and administration of anti-glaucoma drops. Refractive errors returned to normal levels after 8 days.

Conclusion

Secondary acute angle-closure glaucoma has been reported to occur after the administration of some drugs. In this report, an attempt has been made to describe this adverse reaction after oral cabergoline intake.Key Words: Bilateral angle-closure glaucoma, Cabergoline, Galactorrhea  相似文献

PURPOSE: To study the safety and efficacy of immediate argon laser peripheral iridoplasty (ALPI) as initial treatment of acute phacomorphic angle-closure (phacomorphic glaucoma) before cataract extraction. METHODS: In all, 10 consecutive patients with acute phacomorphic angle-closure and intraocular pressure (IOP) > or =40 mmHg were recruited into the study. Each patient received topical atropine (1%) and timolol (0.5%), and immediate ALPI as initial treatment. The IOP at 15, 30, 60, and 120 min, and then 1 day, after ALPI were documented by applanation tonometry. Systemic IOP-lowering drugs were only started if IOP remained above 40 mmHg at 2 h after ALPI. Cataract extraction was subsequently performed as definitive treatment. RESULTS: In total, 10 patients (five male, five female), with a mean age+/-SD of 73.1+/-10.3 years were recruited. Mean duration of symptomatic attack was 128+/-232 h. After ALPI, the mean IOP was reduced from 56.1+/-12.5 to 45.3+/-14.5 mmHg at 15 min, 37.6+/-7.5 mmHg at 30 min, 34.2+/-9.7 mmHg at 60 min, 25.5+/-8.7 mmHg at 120 min, and 13.6+/-4.2 mmHg at 1 day. In one patient, systemic acetazolamide was administered, because the IOP remained above 40 mmHg at 2 h after ALPI. All 10 patients had uncomplicated cataract extraction performed within 4 days after ALPI. No complications from the laser procedure were encountered. CONCLUSION: Immediate ALPI, replacing systemic antiglaucomatous medications, appeared to be safe and effective as first-line treatment of acute phacomorphic angle-closure.  相似文献

AIM—To evaluate histomorphometrically the optic nerve head in highly myopic eyes with absolute secondary angle closure glaucoma.
METHODS—Optic disc sections of 16 highly myopic eyes with an axial length of more than 26 mm and of 19 eyes with an axial length of less than 26 mm were histomorphometrically evaluated. All eyes had been enucleated due to painful absolute secondary angle closure glaucoma.
RESULTS—In the highly myopic eyes compared with the non-highly myopic eyes, mean optic disc diameter was significantly larger (mean 2.33 (SD 0.55) mm versus 1.77 (0.50) mm; p=0.01), and the optic cup was significantly shallower (optic cup depth 0.34 (0.29) mm versus 0.63 (0.23) mm; p=0.03). The peripapillary scleral ring was significantly broader (0.58 (0.65) mm versus 0.08 (0.06) mm; p=0.001), and the β zone (0.83 (0.74) mm versus 0.28 (0.25) mm; p=0.006) of the parapapillary chorioretinal atrophy was significantly larger in the highly myopic eyes.
CONCLUSIONS—The results of the present study agree with biomorphometric data of the optic nerve head in highly myopic eyes with glaucoma. In the highly myopic group, a markedly enlarged peripapillary scleral ring characterised by absence of Bruch's membrane and choriocapillaris contributes in addition to α and β zone to the parapapillary atrophy.

The following is a case report of a 19-year-old male who presented with unilateral acute angle-closure glaucoma. Posterior scleritis was diagnosed based on clinical features and ultrasound B-scan. An 83-year-old man who had phacoemulsification and ciliary sulcus fixation of a posterior chamber intraocular lens developed capsular block syndrome with secondary glaucoma 1 year after surgery. The glaucoma resolved, and vision returned immediately after a neodymium:YAG laser capsulotomy was performed. Capsular block syndrome with secondary angle-closure glaucoma should be considered in pseudophakic patients presenting with increased intraocular pressure and a narrow angle.  相似文献

PURPOSE: To evaluate spontaneous reports of ocular side effects associated with topiramate use. DESIGN: Retrospective case series. METHODS: One hundred fifteen case reports, primarily of a specific ocular syndrome (acute secondary angle-closure glaucoma), were collected from spontaneous reporting systems: the Drug Safety section of Ortho-McNeil Pharmaceuticals, Inc. (Raritan, NJ), the Food and Drug Administration (Rockville, MD), the World Health Organization (Uppsala, Sweden), the National Registry of Drug-Induced Side Effects (Casey Eye Institute, Oregon Health & Science University, Portland, Oregon), and the world literature. MAIN OUTCOME MEASURES: The data were evaluated using the World Health Organization Causality Assessment Guide to the certainty of a suspected adverse drug reaction. RESULTS: Eighty-six cases of acute-onset glaucoma (83 bilateral and 3 unilateral), 17 cases of acute bilateral myopia (up to 8.75 diopters), 9 cases of suprachoroidal effusions, 3 cases of periorbital edema, and 4 cases of scleritis were reported. In those cases for which management was reported, 38% had laser or surgical peripheral iridectomy (21 cases). CONCLUSIONS: In the "certain" category of the World Health Organization classification system, the following are caused by topiramate therapy: abnormal vision, acute secondary angle-closure glaucoma, acute myopia, and suprachoroidal effusions. All findings are reversible if recognized early and if the drug is discontinued. The first presenting symptom of acute secondary angle-closure glaucoma in many patients was blurring of vision. Peripheral iridectomy is ineffective for this type of angle-closure glaucoma.  相似文献
