This systematic review aimed to examine the caregivers' knowledge with burned children and related factors towards burn first aid. A comprehensive, systematic search was performed in different international electronic databases, such as Scopus, PubMed, Web of Science, and Persian electronic databases such as Iranmedex, and Scientific Information Database using keywords extracted from Medical Subject Headings such as “Knowledge”, “First aid”, “Caregiver”, “Burn”, and “Child” from the earliest to the December 1, 2022. The quality of the studies included in this systematic review was evaluated by using the appraisal tool for cross-sectional studies (AXIS tool). A total of 11 763 caregivers of children with burns were enrolled in 14 studies. Of the participants, 78.81% were female and 41.15% had a university education. The mean score of caregivers' knowledge with burned children towards burn first aid was 51.44 out of 100. The knowledge of caregivers of burned children towards burn first aid had a significant positive relationship with the level of education, first aid training, age of caregivers, history of burn, number of children, monthly income, social status, and attitude. In addition, caregivers' knowledge had a significant negative relationship with the number of children. Furthermore, there was a significant relationship between caregivers' knowledge and level of education, monthly income, smoking, and previous knowledge of first aid. The level of caregivers' knowledge with burned children towards burn first aid was moderate. Therefore, health managers and policymakers can improve the knowledge of caregivers of burned children towards burn first aid by creating suitable platforms for face-to-face training as well as online training using websites.  相似文献

This study evaluates the relationship between spirituality, resilience, anger and health status, and posttraumatic symptom severity in trauma survivors. A community sample (N = 1,200) completed an online survey that included measures of resilience, spirituality (general beliefs and reincarnation), anger, forgiveness, and hatred. In survivors of violent trauma (n = 648), these measures were evaluated with respect to their relationship to physical and mental health, trauma-related distress, and posttraumatic symptom severity. Using multivariate regression models, general spiritual beliefs and anger emerged in association with each outcome, whereas resilience was associated with health status and posttraumatic symptom severity only. Forgiveness, hatred, and beliefs in reincarnation were not associated with outcome. The importance of these findings to treating trauma survivors is discussed.  相似文献

Caring for an offspring with an autism spectrum disorder (ASD) has been related to high stress levels and health disturbances. However, a protective effect against these negative health outcomes has been described in high‐resilient caregivers. In this context, the main aim of the present study was to assess the association between resilient coping and cortisol response to acute stress in caregivers of people with ASD. Furthermore, the study aimed to explore the mediating role of anger in this association. We exposed 40 caregivers of people with ASD to an acute psychosocial stressor in the laboratory. Salivary cortisol samples were obtained before, during, and after the stressor. Resilient coping, anger, and socio‐demographic variables were also assessed. Resilient coping was negatively correlated with cortisol response. Specifically, cortisol release was lower in high‐resilient than low‐resilient caregivers. Anger was positively correlated with cortisol response, mediating the association with resilient coping. The observed associations of resilient coping and anger with cortisol response indicate that these variables may affect health outcomes, resilience being protective and anger harmful. Psychotherapeutic interventions focused on strengthening resilience and anger management could benefit caregivers, improving their health status and quality of life.  相似文献

This systematic review aimed to examine the knowledge of caregivers regarding pressure ulcer (PU) prevention. A thorough, methodical search was conducted from the earliest date to February 1, 2023 using keywords extracted from Medical Subject Headings such as “Caregivers”, “Knowledge”, and “Pressure ulcer” in various international electronic databases such as Scopus, PubMed, Web of Science, and Persian electronic databases such as Iranmedex and Scientific Information Database. The quality of the studies included in this systematic review was evaluated using an appraisal tool for cross-sectional studies (AXIS tool). In total, 927 caregivers participated in the eight studies. The average age of the participants was 40.50 (SD = 12.67). Among the participants, 61.87% were women. The average caregiver's knowledge of PU prevention was 53.70 (SD = 14.09) out of 100, which suggests a moderate level of knowledge. Factors such as level of education, age, occupation, information about PUs, attitude, and practice had a significant positive relationship with caregivers' knowledge related to the prevention of PUs. Knowledge had a significantly negative relationship with age. In addition, marital status, type of relationship, age, gender, occupation, level of education, and inpatient wards had a significant relationship with caregivers' knowledge regarding PUs prevention. Therefore, managers and policymakers in the medical field can help increase caregivers' knowledge by providing an online or in-person educational platform relevant to PU prevention.  相似文献

目的探讨老年慢性病患者家庭照顾者应对方式现状及对虐待老人倾向的影响。方法便利选取老年慢性病患者家庭照顾者402人,采用简易应对方式问卷和中文版照顾者虐待老年人评估量表进行调查。结果照顾者虐待倾向得分中位数为3分,四分位数间距为4分;分值在2分及以下者有146人(36.32%),3分及以上者有256人(63.68%)。照顾者积极应对维度得分低于国内常模,消极应对维度得分高于常模(均P0.01);照顾者积极应对方式与虐待倾向呈显著负相关(P0.01),消极应对方式与虐待倾向呈显著正相关(P0.01);照顾者应对方式是虐待倾向的影响因素,积极应对方式是虐老倾向的保护因素,消极应对方式是虐老倾向的危险因素,应对方式能解释照顾者虐老倾向总变异的29.0%。结论老年慢性病患者家庭照顾者应对方式趋于消极且其虐老风险较高。护理人员应鼓励照顾者多采取积极应对方式,避免长期采用消极应对方式,以减少虐老风险,提高照顾质量。  相似文献

目的了解社区失智老人照顾者积极感受现况并探究其影响因素,为制定有效的社区护理干预方案提供指导。方法应用照顾者积极感受量表对80名社区失智老人照顾者进行测评。结果80名社区失智老人照顾者积极感受得分(28.53±5.37)分,得分率63.40%;回归分析结果显示,失智老人患病严重程度及是否患有其他慢性疾病是影响照顾者积极感受的主要因素(均P0.01)。结论社区失智老人照顾者积极感受水平中等,生活展望态度不佳。建议完善失智症患者的社区医疗服务,减轻失智症家庭的疾病负担,同时可开展社区失智症照顾指导干预,提高社区失智老人照顾者应对失智症的信心,提升居家照顾者的积极感受。  相似文献

Childhood gender nonconformity (CGNC) seems to be associated with more mental health problems in adulthood. Previous research has suggested that this link might be mediated via the increased risk for aversive childhood experiences (ACEs) as a negative social reaction to CGNC. However, no study yet examined the role of resilience factors in this relationship. The present study aims to address this gap by examining the potential buffering effect of sense of coherence (SOC). In a German sample of 371 cisgender men, we used mediation models to investigate the relationship between CGNC, ACEs, and mental health problems in adulthood, that is, depressive symptoms, loneliness, and suicidal behavior. We then employed moderated mediation models to examine the buffering effect of SOC on the association ACEs and mental health problems. The results showed that higher levels of CGNC were associated with more severe adult mental health problems, with this link being partially mediated by higher levels of ACEs. For depressive symptoms and suicidal behavior in the last 12 months, we found evidence of a buffering effect of SOC. Higher levels of SOC were associated with a weaker association between ACEs and mental health problems. In contrast, this effect was absent for loneliness and lifetime suicidal behavior. Our study provides evidence that ACEs partly account for the relationship between CGNC and mental health in adulthood. Moreover, we found support for SOC having a buffering effect on this link. Future studies need to examine whether SOC might be an important target for resilience training in those experiencing CGNC. However, sustainable interventions may rather address the negative social reactions to CGNC.  相似文献

Objective

The burden placed on caregivers can negatively affect the functional recovery of patients with hip fractures. It is therefore essential to consider caregivers' well-being during the hip fracture care pathway. The aim of this study is to evaluate caregivers' quality of life and depression status during the first year after hip fracture treatment.

Methods

We prospectively enrolled the primary caregivers of patients with hip fractures admitted to the Faculty of Medicine Siriraj Hospital (Bangkok, Thailand) between April 2019 and January 2020. The quality of life of each caregiver was evaluated using the 36-Item Short Form Survey (SF-36), EuroQol 5-Dimensions 5-Levels (EQ-5D-5L), and EuroQol Visual Analog Scale (EQ-VAS). Their depression statuses were assessed using the Hamilton Rating Scale for Depression (HRSD). The outcome measures were collected during admission as baseline data and 3, 6 months, and 1 year after hip fracture treatment. The repeated measures analysis of variance was used to compare all outcome measures from baseline to each indicated time point.

Results

Fifty caregivers were included in the final analysis. The mean SF-36 physical and mental component summary scores decreased significantly from 56.6 to 54.9 (p = 0.012) and 52.7 to 50.4 (p = 0.043), respectively, during the first 3 months after treatment. The physical and mental component summary scores returned to baseline values 12 and 6 months posttreatment, respectively. Although the mean EQ-5D-5L and EQ-VAS scores significantly declined at 3 months, they returned to baseline values within 12 months. As for HRSD, 6%, 56%, 36%, and 6% of the caregivers reported mild depression symptoms at baseline and 3, 6, and 12 months posttreatment, respectively.

Conclusions

The quality of life and depression status of hip fracture patients' caregivers worsen substantially in the first 3 months and return to baseline 1 year after hip fracture treatment. Specific attention and support should be given to caregivers, particularly during this difficult period. Caregivers should be regarded as “hidden patients” who need to be integrated into the hip fracture treatment pathway.  相似文献

Posttraumatic stress disorder (PTSD) is related to dysfunctional emotional processing, thus motivating the search for physiological indices that can elucidate this process. Toward this aim, we compared pupillary response patterns in response to angry and fearful auditory stimuli among 99 adults, some with PTSD (n = 14), some trauma‐exposed without PTSD (TE; n = 53), and some with no history of trauma exposure (CON; n = 32). We hypothesized that individuals with PTSD would show more pupillary response to angry and fearful auditory stimuli compared to those in the TE and CON groups. Among participants who had experienced a traumatic event, we explored the association between PTSD symptoms and pupillary response; contrary to our prediction, individuals with PTSD displayed the least pupillary response to fearful auditory stimuli compared those in the TE, B = ?0.022, p = .077, and CON, B = ?0.042, p = .002, groups, but they did not differ on angry auditory stimuli, B = 0.019, p = .118 and B = 0.006, p = .634, respectively. It is important to note that within‐group analyses revealed that participants with PTSD differed significantly in their response to angry versus fearful stimuli, B = ?0.032, p = .015. We also found a positive association between PTSD symptoms and pupillary response to angry stimuli. Our findings suggest that differential pupil response to anger and fear stimuli may be a promising way to understand emotional processing in PTSD.  相似文献

The present study aimed to explore the association between mental health condition of caregivers and mental health of burn injury patients. Totally 300 burn injury patients and 300 caregivers were enrolled. These two cohorts of patients were randomly allocated to study group and control group (150 patients and 150 caregivers in each group). The mental health condition of patients and caregivers was evaluated both before and after psychological interventions. There was a significant reduction of the self-rating depression scale (SDS) and self-rating anxiety scale (SAS) of patients in the study group and control group after intervention (28.23 ± 4.98 vs 32.21 ± 5.01, P < 0.001; 28.18 ± 5.01 vs 31.18 ± 5.04, P < 0.001). The corresponding indexes of caregivers showed similar results before (SDS: 47.03 ± 4.41 vs 46.98 ± 4.39, P = 0.922; SAS: 47.01 ± 4.31 vs 46.93 ± 4.35, P = 0.873) and after intervention (21.76 ± 4.23 vs 38.98 ± 4.09, P < 0.001; SAS: 21.02 ± 4.09 vs 38.65 ± 4.04, P < 0.001). The SDS score of patients in study group was positively correlated with the SDS and SAS score of caregivers (r = 0.418 and 0.218, P = 0.003 and 0.012). The SAS score of patients in the study group was also positively correlated with the SDS and SAS scores of caregivers (r = 0.107 and 0.761, P = 0.029 and 0.018). Multiple linear regression showed that age, education and time of care per day were the independent variables associated with mental health condition of caregivers in the study group (P < 0.05). Mental health condition of caregivers was closely related to the mental health of patients. We examined social information processing factors that could represent pathways through which posttraumatic stress disorder (PTSD) symptoms relate to anger expression and intimate partner violence (IPV) perpetration in returning U.S. veterans. The sample included 92 male Operation Enduring Freedom/Operation Iraqi Freedom veterans, primarily Caucasian (77.4%), with smaller numbers of African American, Asian, Hispanic or Latino, American Indian or Alaskan Native, and other minority participants (9.7%, 2.2%, 2.2%, 3.2%, and 5.3% respectively). The average age was 40.37 (SD = 9.63) years. Data were collected through self‐report questionnaires (PTSD Checklist, State‐Trait Anger Expression Scale, Revised Conflict Tactics Scales) and the Articulated Thoughts in Simulated Situations experimental protocol. Laboratory‐based assessment of cognitive biases and hostile attributions were tested as mediators of associations between PTSD symptoms and anger expression and IPV. Among the PTSD symptom clusters, hyperarousal symptoms were most strongly associated with anger expression (r = .50) and IPV perpetration (r = .27). Cognitive biases mediated associations between PTSD total scores and 3 of 4 PTSD cluster scores as well as anger expression. Hostile attribution biases were also associated with IPV perpetration (r = .23). We discuss the implications of these findings for understanding social information processing mechanisms for the relationship between PTSD symptoms and aggression.  相似文献

It is well established that stressful life events (e.g., family bereavements or moving to a new country) are damaging to psychological health and well‐being. Indeed, social relationships are often noted as an important factor that can influence well‐being and buffer the negative effects of stress. However, the quality and source of these relationships, particularly for adolescents, are often overlooked. Using the Growing Up in Ireland Survey, a population‐based study of 13‐year‐old Irish adolescents (N = 7,525; 51.1% female), the current study examines the quality of both parent and peer relationships as potential mechanisms explaining the association between stressful life events and psychological well‐being indices in adolescents. As expected, results showed that stressful life events negatively impacted the psychological well‐being of adolescents. Parallel mediation analyses indicated that both parent and peer relationship quality mediated this association. Further exploratory analyses found that for girls, greater numbers of stressful life events were associated with poorer quality relationships with both their parents and peers, and in turn, these were linked to lower levels of psychological well‐being. For boys, this effect was only evident for parental relationship quality, but not peers. The implication of these findings for adolescent's psychological well‐being, particularly for girls, is discussed.  相似文献

Posttraumatic stress disorder (PTSD) is associated with significant individual and relationship impairment for people with PTSD and their romantic partners. Conjoint treatments, such as cognitive behavioral conjoint therapy for PTSD (CBCT), are designed to address individual and relationship factors, yet significant barriers impede accessing in-person therapy. Couple HOPES (i.e., Helping Overcome PTSD and Enhance Satisfaction) is a coach-guided, online couple intervention for PTSD based on CBCT that was designed to address these barriers. Previous investigations have found preliminary efficacy of Couple HOPES for improving PTSD symptoms, relationship functioning, and some individual functioning domains for the partner with probable PTSD. However, no study to date has tested individual outcomes for romantic partners, which is needed to fully evaluate the intervention's promise. The current study tested these partner outcomes in a combined, uncontrolled sample of 27 couples. Intent-to-intervene analyses found significant improvements at postintervention in four of eight tested outcomes, including ineffective arguing, g = 0.74; anger, g = 0.32; perceived health, g = 0.67; and quality of life, g = 0.56. Depressive symptoms, generalized anxiety, alcohol misuse, and work functioning did not significantly change, gs = 0.17–0.42. Among participants who completed a 1-month follow-up assessment, generalized anxiety, g = 0.43, and perceived health, g = 0.73, significantly improved over follow-up, whereas anger, g = -0.48, lost gains previously made. Results were largely consistent in the completer sample. These findings show the potential of Couple HOPES to have broad benefits not only for individuals with probable PTSD but also for their romantic partners.  相似文献

目的了解社区老年留置胃管患者口腔健康状况并分析其影响因素。方法采用一般资料调查问卷、日常生活活动能力量表、改良版Beck口腔评分表和流行病研究中心抑郁调查量表,对127例社区老年留置胃管患者及其主要照护者进行调查,并对患者口腔pH值进行测量。结果社区老年留置胃管患者Beck口腔得分为(10.02±2.39)分,96.85%患者存在不同程度的口腔功能受损,71.65%患者口腔pH值呈酸性。多重线性回归分析结果显示,患者胃管留置时间、清洁口腔频率、口腔pH值,以及照护者与患者关系、刷牙频率、抑郁程度为老年留置胃管患者口腔健康状况的影响因素(P0.05,P0.01)。结论社区老年留置胃管患者口腔健康状况不容乐观,主要照护者应重视老年患者留置胃管的口腔清洁,家属和社区工作者应提供正确的指引并关爱照护者,培养其正确的刷牙习惯。  相似文献

Public opinion polls reliably find a disparity between people's views of the state of their own communities and of the nation in general. This discrepancy, frequently called the Optimism Gap, is often attributed to the fact that people have personal knowledge of things around them, but derive their larger world‐view from media sources such as television, which tend to over emphasize the negative, risky aspects of life. This study explores this premise, as well as the possibility that emotional factors such as depression, anxiety, and anger also play a role. Data from 171 participants were analysed. Overall, people rated problems in the nation in general as more severe than those in their own communities. Television exposure was predictive of ratings of problems nationwide, but not of problems in respondents' own communities, while attention paid to the television news was related to ratings of national and community problems. Depression, irrational beliefs, and anger arousal were all related to ratings of problems in the nation in general, while anger arousal and optimism were predictive of ratings of problems in respondents' own communities. BACKGROUND: Although studies suggest that optimism can predict health outcomes, the relationship has not been tested in women with pelvic organ prolapse (POP). This study sought to explore the relationship between optimism, prolapse severity, and symptoms before operation; and examine whether optimism predicts postsurgical functional status, treatment satisfaction, and treatment success. STUDY DESIGN: Data from the randomized Colpopexy and Urinary Reduction Efforts (CARE) study, in which stress continent women undergoing sacrocolpopexy to repair stage II to IV POP completed a baseline assessment of optimism and validated symptom and quality-of-life measures at baseline and 24 months. Relationships between optimism and demographics, clinical status, and functional and quality-of-life outcomes were assessed. RESULTS: Of 322 Colpopexy and Urinary Reduction Efforts study participants, 305 (94.7%) completed 24-month followup interviews. At baseline, there were no notable differences in optimism with respect to POP stage or history of earlier operations for prolapse or urinary incontinence. The purpose of this study was to examine whether information bias associated with dispositional optimism and generalized self‐efficacy can account for the link between general expectations and well‐being. A modified Stroop task was used in this study. Our hypothesis was that individuals with high self‐efficacy expectations or dispositional optimism would show greater bias towards well‐being‐related stimuli, whereas individuals with low self‐efficacy or optimism would exhibit bias towards threat‐related stimuli. A secondary hypothesis was that both self‐efficacy and optimism would act as mediators of the latency, perceived distress relationship. One hundred and two undergraduate students participated in the study. After controlling for daily mood, the results showed that individuals high in optimism and self‐efficacy showed greater informational bias towards well‐being‐related stimuli. The low self‐efficacy group exhibited greater bias towards threat‐related stimuli. Also, consistent with our hypothesis, optimism and self‐efficacy mediated the relationship between the Stroop colour‐naming latencies and perceived distress. These findings suggest that associations, which refer to automatic processes, may form an additional way through which expectations are related to functioning. Traits of optimism and cynical hostility are features of personality that could influence the risk of falls and fractures by influencing risk‐taking behaviors, health behaviors, or inflammation. To test the hypothesis that personality influences falls and fracture risk, we studied 87,342 women enrolled in WHI‐OS. Optimism was assessed by the Life Orientation Test–Revised and cynical hostility, the cynicism subscale of the Cook‐Medley questionnaire. Higher scores indicate greater optimism and hostility. Optimism and hostility were correlated at r = –0. 31, p < 0.001. Annual self‐report of falling ≥2 times in the past year was modeled using repeated measures logistic regression. Cox proportional hazards models were used for the fracture outcomes. We examined the risk of falls and fractures across the quartiles (Q) of optimism and hostility with tests for trends; Q1 formed the referent group. The average follow‐up for fractures was 11.4 years and for falls was 7.6 years. In multivariable (MV)‐adjusted models, women with the highest optimism scores (Q4) were 11% less likely to report ≥2 falls in the past year (odds ratio [OR] = 0.89; 95% confidence intervals [CI] 0.85–0.90). Women in Q4 for hostility had a 12% higher risk of ≥2 falls (OR = 1.12; 95% CI 1.07–1.17). Higher optimism scores were also associated with a 10% lower risk of fractures, but this association was attenuated in MV models. Women with the greatest hostility (Q4) had a modest increased risk of any fracture (MV‐adjusted hazard ratio = 1. 05; 95% CI 1.01–1.09), but there was no association with specific fracture sites. In conclusion, optimism was independently associated with a decreased risk of ≥2 falls, and hostility with an increased risk of ≥2 falls, independent of traditional risk factors. The magnitude of the association was similar to aging 5 years. BackgroundDespite increased life expectancy among patients with cystic fibrosis (CF), few studies have examined coping among adolescents and young adults with CF. Previous research suggests that stigma associated with chronic disease is related to worse physical and psychological health, but optimism may be protective. This study examined stigma and optimism among patients with CF.MethodsSeventy-two patients with CF (ages 14 to 25) completed a self-report questionnaire assessing stigma, distress, CF-specific quality of life (QoL), and optimism. Objective health data were recorded from patient medical records.ResultsGreater stigma was associated with lower pulmonary function, QoL, and optimism. Stigma was positively correlated with distress. Optimism moderated the relationship between stigma and anxiety (p < 0.001), and between stigma and emotional functioning (p < 0.01).ConclusionsStigma is related to worse lung function and psychological health in patients with CF, but higher levels of optimism may act as a protective factor.  相似文献

目的评估脑卒中偏瘫患者照顾者睡眠质量、照顾负担与心理弹性状况,探索脑卒中偏瘫患者照顾者心理弹性在照顾负担和睡眠质量间的中介和调节作用。方法采用匹兹堡睡眠质量指数、照顾负担量表、Connor-Davidson心理弹性量表对306名脑卒中偏瘫患者照顾者进行测评。结果照顾者睡眠质量总分为9.3±3.7,睡眠障碍检出率为57.8%;照顾负担总分为35.9±11.2,心理弹性总分为55.0±16.1。照顾负担得分与心理弹性得分呈负相关,与睡眠质量得分呈正相关;心理弹性得分与睡眠质量得分呈负相关(均P0.01)。心理弹性在照顾负担和睡眠质量间起到部分中介和调节作用。结论增强脑卒中偏瘫患者主要照顾者的心理弹性水平,有利于减轻其照顾负担,改善其睡眠质量。  相似文献
