目的 探讨腔内技术治疗尿流改道术后输尿管肠代膀胱吻合口狭窄的临床应用价值.方法 膀胱癌尿流改道术后输尿管肠代膀胱吻合口狭窄患者9例,狭窄段长度1~3 cm,均采用腔内技术治疗,顺行经皮肾处理8例,逆行输尿管镜处理1例;术中使用高压气囊配合筋膜扩张器扩张,术后留置双J管.结果 随访0.5~5.0年.1例吻合口闭锁患者术后3个月仍为重度积水,患者拒绝开放手术而长期留置肾造瘘管;8例患者肾积水减轻,再次逆行扩张狭窄段并留置双J管,其中5例经2~3次扩张换管,拔除双J管后复查肾积水稳定于轻度状态;3例拔除双J管后腰痛不适,需要长期留置双J管.结论 尿流改道术后输尿管肠代膀胱吻合口狭窄腔内技术治疗效果良好,可避免开放手术的风险.  相似文献

PURPOSE: A modified Le Duc procedure with a short submucosal tunnel was applied for ureteroileal implantation in ileal orthotopic neobladder and bladder augmentation with the ileum. We assessed the rate of stenosis and ureteral reflux at the ureteroileal anastomosis after this procedure. MATERIALS AND METHODS: Two women and 22 men underwent radical cystectomy and creation of a Hautmann ileal neobladder for invasive bladder cancer. Another woman underwent ileal bladder augmentation with bilateral ureteral reimplantation into the ileal segment. Ureteroileal anastomosis was performed using the modified Le Duc technique in 48 renoureteral units. Followup in all patients included retrograde cystography done before discharge home and excretory urography, renal ultrasonography or abdominal computerized tomography every 4 to 6 months. Followup was 11 to 39 months in 23 of the 25 cases. RESULTS: Retrograde cystography before discharge home revealed no urinary reflux in any reimplanted ureter. There was no ureteral stenosis or reflux in 20 male and 3 female patients (44 renoureteral units) who voided successfully without catheterization. A unilateral ureteral stricture at the ureteroileal anastomotic site in 1 man who voided successfully was treated with endoscopic surgery. Bilateral slight upper urinary tract dilatation caused by ureteral reflux was present in another man who did not void successfully. CONCLUSIONS: The modified Le Duc technique is simple and safe for forming an ureteroileal anastomosis in ileal orthotopic neobladder creation. It appears to have a low ureteral stenosis and reflux complication rate in patients who successfully void postoperatively.  相似文献

OBJECTIVES: To evaluate the adaptability and the possible advantages of the antireflux mechanism of the serous lined extramural tunnel for ureter re-implantation on a gastrointestinal anastomosis (GIA) Stapler detubularised ileal neobladder METHODS: From April 1998 to July 2002 43 male patients underwent radical cystectomy and the creation of a Camey II ileal neobladder using this antireflux technique. Follow-up in all cases included excretory urography or T.C. scan and a retrograde cystography at 6 months and a renal scintigraphy with DMSA at 1 year follow-up, besides serum creatinine, blood urea and serum electrolytes every three months and renal-neovesical ultrasound every six months. RESULTS: Early and late complications were low. At follow-up 1 case of neovesico-ureteral reflux and 2 cases of ureteroileal anastomotic strictures were found. DMSA scintigraphy showed no further renal scars. CONCLUSIONS: This preliminary experience was favourable due to overall reduced operating time (about 1 hour 45 minutes to create the orthotopic neobladder and the ureteroileal anastomosis), simplicity of execution and a low complication rate (6.9%) at a median follow-up of 38 months (range 12-52 months).  相似文献

目的探讨内镜治疗原位新膀胱术后输尿管末端粘连的临床应用。方法采用改良的全膀胱切除和原位新膀胱术治疗157例浸润性膀胱癌患者,男性144例,女性13例。术后发现11例患者因输尿管末端互相粘连或与新膀胱壁粘连而导致上尿路积水,遂行经尿道内镜下切断粘连带。结果内镜治疗术中术后均无明显并发症,随访9~36个月(中位14个月),10例肾功能和积水程度明显改善,1例稳定。1例在积水缓解后7个月再次复发,发现输尿管肠吻合口狭窄,行开放手术作输尿管新膀胱再植,随访18个月,积水改善。结论原位新膀胱术后输尿管末端粘连是上尿路积水的原因之一,常规膀胱镜检有利于诊断,经尿道内镜下切断粘连带是较为简单有效地治疗方法。  相似文献

BACKGROUND AND PURPOSE: Orthotopic ileal neobladder is currently the preferred continent urinary diversion in suitable patients undergoing radical cystectomy for muscle-invasive bladder cancer. To our knowledge, presented herein is the initial report of laparoscopic orthotopic ileal neobladder following cystectomy that was performed completely intracorporeally in a porcine model. MATERIALS AND METHODS: The laparoscopic technique was developed in seven pigs. Subsequently, a long-term survival study was performed in 12 consecutive animals. Laparoscopic cystectomy was performed, preserving the urethral sphincter. An ileal segment of 35 cm (first three animals), 45 cm (next four), or 55 cm (final five animals) with adequate mesentery was isolated; and ileal continuity was restored intracorporeally by a stapled anastomosis. Ileal detubularization for construction of an ileal neobladder, urethroileal anastomosis, and bilateral stented ileoureteral anastomoses to a tubular Studer limb extension were all created completely intracorporeally using only laparoscopic free-hand suturing and knot-tying. Biochemical data (preoperative and serial postoperative hemoglobin, renal panel, blood gases), radiologic studies (intravenous urogram, retrograde pouchgram), functional measures (neobladder urodynamics, Whitaker pressure-flow study of both ureters), and microscopic evaluation of the neobladder and ureteroileal and urethroileal anastomotic sites were obtained to evaluate the long-term functional and anatomic outcome. RESULTS: Completely intracorporeal laparoscopic construction of an ileal orthotopic neobladder was successful in all 12 animals without intraoperative or early postoperative complications or open conversion. The mean operating time was 5.4 hours (range 4.5-6.5 hours), and the blood loss was minimal. All study pigs survived their predetermined follow-up period, ranging from 1 to 3 months. Late complications occurred in three animals: one port-site abscess and two cases of E. coli pyelonephritis and azotemia, leading to one death at 2 months. The mean serum creatinine concentrations were 1.33 mg/dL, 1.61 mg/dL, and 1.55 mg/dL at 1, 2, and 3 months, respectively. The mean neobladder capacity was 420 mL (range 250-700 mL) with pressures < or = 20 cm H2O (range 17-20 cm H2O). Pre-euthanasia Whitaker testing confirmed excellent drainage in all 24 ureters. No ileoureteral or ileourethral anastomotic strictures or leaks were noted on intravenous urography, retrograde pouchgram, or postmortem physical calibration of the anastomotic sites. Histologic examination confirmed excellent healing without obvious fibrosis. CONCLUSION: Laparoscopic construction of an orthotopic neobladder is feasible. The anatomic and functional outcome is excellent and comparable to that of open surgery. Clinical application is imminent.  相似文献

PURPOSE: To describe the technique of laparoscopy-assisted undiversion of an ileal conduit into a continent orthotopic ileal neobladder performed on a patient with a previous radical cystoprostatectomy and ileal conduit. CASE REPORT: A 57-year-old man presented with a prolapsed stoma and a history of a right radical nephroureterectomy for grade 3 ureteral transitional-cell carcinoma and a radical cystoprostatectomy and ileal conduit urinary diversion for in-situ bladder carcinoma, performed 12 and 8 years ago, respectively. After the ileal stoma was resected, five trocars were placed transperitoneally. Partial resection of the distal ileal conduit was performed, leaving in place the proximal segment with its left ureteroileal anastomosis. Flexible urethroscopy revealed a contracting external sphincter, and random urethral frozen-section biopsies ruled out tumor. A 45-cm segment of ileum was isolated and exteriorized through the stoma site, and an ileal neobladder was created extracorporeally, suturing the proximal ileal-conduit segment, with its ureteroileal anastomosis, to it. The ileal neobladder was reintroduced into the abdomen and anastomosed laparoscopically to the urethral stump with six 2-0 polyglactin sutures. The total operative time was 7 hours with a blood loss of 100 mL. There were no intraoperative complications. The hospital stay was 7 days. At a follow-up of 24 months, the patient had total daytime continence and normal renal function, and intravenous urography revealed an unobstructed urinary tract. CONCLUSION: Laparoscopy-assisted ileal-conduit undiversion into an orthotopic ileal neobladder is technically feasible. It can be considered an alternative to open surgery for patients who have undergone urinary diversion.  相似文献

目的 探讨根治性全膀胱切除术中保留远端的前列腺包膜及精囊对原位新膀胱功能及勃起功能的影响。方法 对24例男性浸润性膀胱癌患者施行改良根治性全膀胱切除及原位回肠膀胱术:保留远端的前列腺外科包膜及精囊,新回肠膀胱与残留前列腺包膜连续缝合;对术后新膀胱的储尿、排尿、控尿功能及患者的勃起功能进行随访和比较。结果 术后病理分期:T2aN0M0 5例,T2bN0M0 9例,T3aN0M0 7例,T3bN1M0 3例。术后随访3—24个月,平均12.7月。无瘤生存22例;带瘤生存2例。新膀胱容量(385±68)mL,最大充盈压(24±16)cmH2O。排尿良好,最大尿流率(18±5)mL/s,剩余尿(35±16)mL;完全控尿22例,夜间尿失禁2例;21例术前勃起功能正常者术后2例发生勃起功能障碍。结论 在改良根治性膀胱全切术中保留远端的前列腺外科包膜及精囊,可明显改善患者术后的储尿、排尿、控尿功能和勃起功能,同时可有效防止新膀胱一尿道吻合口狭窄的发生。  相似文献

Objectives: To investigate and compare Wallace direct ureteroileal anastomosis with Le Duc anti‐reflux procedure in modified Studer orthotopic neobladder reconstruction after radical cystectomy. Methods: A total of 72 consecutive patients who underwent modified Studer orthotopic bladder reconstruction after a radical cystectomy for bladder cancer were investigated. They were examined for vesicoureteral reflux, hydronephrosis, and pyelonephritis at 6 months after surgery according to the type of ureteroileal anastomosis. Results: Vesicoureteral reflux occurred in 29 ureters (38.2%) after the Wallace procedure compared to six ureters (9.6%) with the Le Duc (P < 0.05). Hydronephrosis was detected in 12 ureters (18.8%) in the Le Duc patients compared to seven (9%) in the Wallace patients (P > 0.05). Six months after the operation, all three patients with vesicoureteral reflux‐related hydronephrosis improved using clean intermittent catheterization in the Le Duc patients; five of seven patients were cured by clean intermittent catheterization and two improved without any treatment in the Wallace patients. Seven of nine cases of ureteroileal anastomosis stenosis causing hydronephrosis were cured without any treatment but one case resulted in a non‐functional kidney despite treatment of the stenosis. Conclusions: Direct ureteroileal anastomosis using the Wallace method is effective for minimizing ureteroileal anastomosis stenosis and it represents a simple surgical procedure when combined with a modified Studer procedure.  相似文献

目的:探讨腔内钬激光治疗输尿管结石并发狭窄的处理策略.方法:收集36例输尿管镜下钬激光切开治疗输尿管结石并发狭窄患者的临床资料.结果:35例手术成功,1例单纯壁间段狭窄者予钬激光切开后仍无法上行,手术终止.术后共随访12~45个月,平均(23.8±9.5)个月.30例治愈或好转,有效率为83.3%(30/36).6例无效(5例术后1~3个月结石复发,复行腔内激光手术,随访18~24个月,其中4例治疗有效,1例效果不佳.结论:经尿道钬激光碎石术(TUL)在治疗上尿路结石时可同时处理输尿管狭窄,是一种安全有效的手段.钬激光切开治疗狭窄的疗效(85.2%,23/27)要好于硬性扩张(77.8%,7/9),但两者比较无统计学意义(P>0.05)根据术中具体情况实施操作可能更具实用价值,不仅能够减少操作所需时间,且预后理想.我们倾向性认为中下段狭窄的预后(93.1%,27/29)较上段(57.1%,4/7)较好(P<0.05),但其处理方法选择标准及处理策略仍有待进一步探讨和完善.  相似文献

目的:通过根治性膀胱全切术后回结肠和回肠正位膀胱替代术的临床观察,重点评价两种术式的尿控及其对上尿路的影响及机制。方法:从2002~2006年期间行标准的根治性膀胱全切术后行正位膀胱替代术中选取获得随访的膀胱癌患者31例,其中17例采用回结肠新膀胱重建术(Le Bag术),14例采用回肠新膀胱重建术(Studer术)。术后3个月内(近期)和3个月后(远期)观察相关并发症和尿控情况,并行尿动力学检查和静脉肾盂造影或B超检查。结果:回结肠新膀胱组手术后并发症率为11.8%(2/17);回肠新膀胱组并发症率为14.3%(2/14)。虽然回结肠新膀胱组3个月后白天完全获得尿控76.4%(13/17).低于回肠新膀胱组92.8%(13/14),但统计学差异不显著;而夜间社会性尿控回肠新膀胱组7I.4%(10/14),显著高于回结肠新膀胱组35.3%(6/17)。尿动力学结果表明两组间的尿流率、功能尿道长度、最大尿道压和新膀胱压力无明显差异,但回结肠新膀胱组容量高于回肠新膀胱组,而顺应性则低于回肠新膀胱组。此外。回结肠新膀胱组的上尿路扩张发生率为11.8%(2/17),略高于回肠新膀胱组0(0/14)。结论:虽然回结肠新膀胱在近期能获得较大容量,但由于回结肠新膀胱顺应性较回肠新膀胱低,因而在获得满意的尿控和对上尿路的保护上,回肠新膀胱更具优势。  相似文献
