临床药师要用药学知识参与临床合理用药

一例肺癌术后复发病人拟口服盐酸厄洛替尼片进行化疗,该药由CYP3A4酶代谢,当与CYP3A4酶的抑制剂或诱导剂联合应用时,应适当调整盐酸厄洛替尼的剂量。该病人同时合并使用了其他6种药物,临床医师就此问题咨询临床药师,临床药师查询资料后给予解答,并由此引发了作者对临床药学工作的思考:临床药师在临床工作中是有用武之地的,但是必须要注重自身专业能力的提高。同时作者发现,国内临床用药的参考资料,如药品说明书和专著的内容有待更新和完善。     

临床药师的知识储备

2002-01-21，卫生部与国家中医药管理局联合颁布了《医疗机构药事管理暂行规定》，其中明确提出：“药学部门要建立以病人为中心的药学管理工作模式，开展以合理用药为核心的临床药学工作，参与临床疾病诊断、治疗，提供药学技术服务，提高医疗质量”，并要求医疗机构“逐步建立临床药师制”。开展临床药学工作，可以说是“箭在弦上，不得不发”，全国多家医院不同程度地开展了此项工作。     

Clinical pharmacists: Bridging the gap between patients and physicians

The clinical pharmacy has spread out drastically in terms of its professional services throughout the past few years. The clinical pharmacist become a crucial element of healthcare team and promotes patient care by interacting with physician and patient. The aim of this paper is to highlight the role of clinical pharmacists in various departments. It concludes that the features of interactions occurring between clinical pharmacists and physicians influence the teamwork between pharmacists and physicians and provide better patient care.

The issue of territoriality between pharmacists and physicians in primary care

This paper discusses the utility of the concept of territoriality in order to understand relationships between pharmacists and physicians in the primary care setting. First, we provide background information on the concept of human territoriality. Secondly, we discuss its potential application to the relationships between pharmacists and physicians. Finally, we suggest ways in which understanding and tackling territorial issues may strengthen the pharmacist‐physician link and, in doing so, may improve patient care.     

临床药师参与多种阿片类药物联用致阿片中毒药物治疗1例

本文介绍了临床药师对1例多种阿片类药物联用致阿片中毒的患者进行的药学监护:协助医师为患者制定解救方案,分析患者阿片中毒的可能原因并调整后续镇痛治疗方案,对患者的长期镇痛治疗提供用药教育。在临床药师的配合下,医师成功解救了阿片中毒的患者,并为患者制定了个体化的镇痛方案,保证了药物治疗的有效性和安全性。     

临床药师参与抗菌药物使用管理的经验和体会(论著)

为了促进抗菌药物的合理使用,有效控制细菌耐药,临床药师参与了医院抗菌药物管理的全过程,并从中收获了有益的经验与体会。要想取得实效,医院管理层面的决心与力度、制定具体的调控措施、完善各项相关制度及多部门的有机合作缺一不可。通过努力,首都医科大学附属北京朝阳医院抗菌药物管理取得明显成效,抗菌药物的不合理使用现象明显减少。抗菌药物的管理需持之以恒,合理使用必须常态化,才能从根本上控制细菌耐药性。
