PURPOSE: To study the effectiveness and safety of selective laser trabeculoplasty (SLT) on primary open-angle glaucoma and ocular hypertension in Chinese eyes. METHODS: This was a prospective randomized controlled clinical study in which 58 eyes of 29 patients with primary open-angle glaucoma or ocular hypertension were included. One eye of each patient was randomized to receive SLT (Group 1) and the fellow eyes received medical treatment (Group 2). Patients were evaluated after laser treatment at 2 h, 1 day, 1 week, 2 weeks, 1 month, 3 months, 6 months, and then yearly. RESULTS: All patients (13 male, 16 female) were Chinese. The mean age was 51.9 +/- 14.7 years. The mean baseline intraocular pressure was 26.8 +/- 5.6 mmHg in group 1 and 26.2 +/- 4.2 mmHg in group 2 (P = 0.62). The failure rate, defined as intraocular pressure >21 mmHg with maximal medications, was 17.2% in group 1 and 27.6% in group 2 at 5-year follow-up (P = 0.53). Eight eyes (27.6%) in group 1 required medications to control the intraocular pressure to below 21 mmHg. There was no statistically significant difference in the intraocular pressure reductions between the two groups at all time intervals (P > 0.05). The mean number of antiglaucoma medications was significantly lower in the SLT than the medical treatment group up to 5 years of follow up (P < 0.001). Transient post-SLT intraocular pressure spike >5 mmHg was observed in three eyes (10.3%). CONCLUSION: With fewer medications, SLT gives similar intraocular pressure reduction to medical therapy alone in Chinese patients with primary open-angle glaucoma or ocular hypertension.  相似文献

In glaucomatous eyes refractory to medication, laser techniques and conventional drainage surgery, intraocular pressure is often high, and visual loss rapid. In this situation a reliable, robust artificial outflow system is required. Molteno has evolved a plastic tube and plate device combined with a fibrosis suppression medication regimen. Thirty-eight eyes of 32 patients with uncontrolled glaucoma were treated with the Molteno system. Six months after operation mean intraocular pressure had been reduced from 41.0 +/- 13.6 to 16.2 +/- 5.6 mmHg. Eighteen eyes had pressures of 20 mmHg or less on no hypotensive therapy, 17 on reduced treatment. Three eyes had a pressure of 21 to 35 mmHg on treatment at six months. The 13 aphakic eyes responded as well as 25 phakic eyes. Five eyes with rubeotic glaucoma demonstrated pressures of less than 20 mmHg without therapy, four eyes with traumatic glaucoma required continuing medication with three having pressures below 22 mmHg. Of the seven eyes with uveitic glaucoma, one was lost, two required maintenance therapy; five of six surviving eyes had pressures below 20 mmHg. Fifteen eyes with congenital or juvenile glaucoma achieved pressures below 20 mmHg, three of these with timolol drops, three with timolol and acetazolamide, and nine with no treatment. While seven of seven eyes with refractory primary open-angle glaucoma attained pressures below 20 mmHg, all seven needed continuing mild hypotensive therapy. Eleven eyes underwent a one-stage procedure, while 27 eyes required a two-stage operation. Twenty-eight eyes received fibrosis suppression medication after the second stage, and 24 maintained or improved their preoperative visual acuity. Results have been encouraging: in general the Molteno system is recommended as the second drainage operation in all glaucomatous eyes in which conventional therapy has failed, and as the primary surgical procedure (after laser techniques) in eyes with rubeotic and uveitic glaucoma. Ciliary body destructive procedures should be restricted to control of symptoms in blind eyes.  相似文献

In 93 phakic eyes--52 eyes with primary open-angle and 41 eyes with exfoliation glaucoma--with increased intraocular pressure, argon laser trabeculoplasty was performed. Fifty to sixty burns were made over 180 degrees of the lower trabecular meshwork. During the follow-up period, visual acuity, intraocular pressure, medication changes and the need for surgery were studied. In 25 eyes a second laser treatment was performed over 180 degrees of the upper trabecular meshwork. Intraocular pressure control was obtained in 81% of the eyes with primary open-angel glaucoma and 71% of the eyes with pseudoexfoliation glaucoma. The average reduction of pressure obtained after laser trabeculoplasty was 9.22 mm Hg for the group of eyes with primary open-angle glaucoma and 13.42 mm Hg for the group of eyes with pseudoexfoliation glaucoma (p less than 0.001). Reduction in medical treatment was obtained in 27 eyes from the 71 eyes with successful reduction of the intraocular pressure. In 6 eyes with successful results after argon laser trabeculoplasty, cataract extraction was performed. The intraocular pressure remained normal in all eyes after the operation.  相似文献

PURPOSE: To investigate the efficacy and safety of selective laser trabeculoplasty as an initial treatment for newly diagnosed open-angle glaucoma, and its role as adjunctive therapy. PATIENTS AND METHODS: A prospective multicenter nonrandomized clinical trial was performed. Patients with newly diagnosed open-angle glaucoma or ocular hypertension were assigned to the primary (selective laser trabeculoplasty) treatment group or the control (latanoprost) group according to patient choice. Both groups were followed up at 1, 3, 6, and 12 months. A secondary treatment group was also included to study the efficacy of selective laser trabeculoplasty for patients intolerant of medical therapy or in whom such therapy was unsuccessful, with or without a history of previous argon laser trabeculoplasty. RESULTS: One hundred eyes (61 patients) were enrolled, 74 in the primary treatment group and 26 in the control group. The average absolute and percent reductions in intraocular pressure for the primary treatment group were 8.3 mm Hg or 31.0%, compared with 7.7 mm Hg or 30.6% for the control group (P = 0.208 and P = 0.879). The responder rates (20% pressure reduction) were 83% and 84% for the primary and control groups, respectively. There were no differences in intraocular pressure lowering with selective laser trabeculoplasty on the basis of angle pigmentation. A modest contralateral effect was observed in the untreated fellow eyes of patients undergoing selective laser trabeculoplasty. CONCLUSIONS: Selective laser trabeculoplasty was found to be equally efficacious as latanoprost in reducing intraocular pressure in newly diagnosed open-angle glaucoma and ocular hypertension over 12 months, independent of angle pigmentation. Nonsteroidal antiinflammatory therapy had similar efficacy to steroids after laser therapy. These findings support the consideration of selective laser trabeculoplasty as a first-line treatment for newly diagnosed open-angle glaucoma or ocular hypertension.  相似文献

PURPOSE: To determine the additive intraocular pressure reduction of various topical glaucoma agents used adjunctively with latanoprost. DESIGN: Retrospective interventional case series. METHODS: Retrospective evaluation of 73 eyes of 73 patients with glaucoma and inadequate intraocular pressure control on latanoprost alone. Each patient received adjunctive treatment with an additional glaucoma agent (dorzolamide, brimonidine, timolol, or other beta-blockers) for 1 year. RESULTS: When added to latanoprost, dorzolamide lowered intraocular pressure an additional 3.9 mm Hg (19.7%, P <.001); beta-blockers further reduced intraocular pressure by 2.0 mm Hg (12.3%, P <.001), and brimonidine further reduced intraocular pressure by 2.0 mm Hg (9.3%, P =.0011). Dorzolamide dosed twice or three times daily was as effective as adjunctive therapy with latanoprost (P =.92). CONCLUSION: Adjunctive therapy with dorzolamide provided a statistically significant intraocular pressure reduction at 1 year in eyes that were inadequately controlled with latanoprost alone.  相似文献

In a prospective study 35 eyes of 25 patients with coexisting cataract and glaucoma underwent trabeculectomy, phacoemulsification and implantation of a folded polyHema intraocular lens through the trabeculectomy opening. Follow-up ranged from 6 to 27 months (mean 13.3). The mean age was 76.4 (68 to 88 years). 22 eyes suffered from primary open angle glaucoma, 10 eyes from a pseudoexfoliation glaucoma and 3 eyes had a chronic angle closure glaucoma. Preoperatively intraocular pressure was controlled in 10 eyes with a mean medication of 2.1 but uncontrolled in 25 eyes (mean medication: 2.5). The preoperative visual acuity ranged from 20/40 to hand motions. Postoperatively intraocular pressure was controlled (<18 mmHg) in all (100%) eyes and without therapy in 32 (91%) eyes. Three (9%) eyes had to be treated with topical timolol twice a day after surgery. Mean intraocular pressure dropped from 21.2±6.0 mmHg preoperatively to 13.5± 2.1 mmHg postoperatively. Vision improved in all but 4 eyes, 25 (74%) achieving a visual acuity of 20/40 or better. The causes for failed improvement or deterioration of vision were senile macular degeneration in 2 eyes and central retinal vein occlusion and vascular optic nerve atrophy in one eye respectively. Postoperative complications included hyphema in 9 (26%) eyes, fibrin effusion to a various extent into the anterior chamber in 19 (54%) eyes and delayed hypotony (<5 mmHg) with chorioidal effusion in 1 (3%) eye. Fibrin effusion was frequently observed in eyes with intraocular pressure below 10 mmHg, iris surgery and hyphema. Finally the complications did not effect the results regarding visual acuity or glaucoma control. In summary we conclude that the combination of small-incision cataract surgery and trabeculectomy is an effective surgical approach to obtain visual rehabilitation and glaucoma control with one single surgical intervention.  相似文献

The records of 100 patients (161 eyes) with uveitis were reviewed retrospectively to determine the prevalence of increased intraocular pressure, the forms of uveitis most commonly associated with glaucoma, and the forms that require specific glaucoma therapy. Secondary glaucoma was present in 23 patients (31 eyes): three of 24 patients with acute uveitis (three eyes, 12% of acute uveitis patients) and 20 of 76 patients with chronic uveitis (28 eyes, 26% of chronic uveitis patients). Eighteen patients (26 eyes, 78% of glaucoma patients) with chronic uveitis required long-term medical therapy to control intraocular pressure. Three patients (three eyes, 12% of glaucoma patients) with acute uveitis required short-term therapy to control intraocular pressure. The remaining two patients had transient increases in intraocular pressure that did not require treatment or that responded to treatment of intraocular inflammation alone. One patient with chronic uveitis (two eyes, 4% of glaucoma patients) required filtering surgery. At least five patients (eight eyes, 22% of glaucoma patients) had glaucomatous visual field defects. The results of this survey are consistent with the concept that secondary glaucoma is a management problem in patients with anterior segment inflammation and chronic rather than acute uveitis. An algorithm for the management of increased intraocular pressure in patients with uveitis is proposed.  相似文献

During a 7-month period 33 patients (20 with primary open-angle glaucoma and 13 with suspected glaucoma) were treated with guanethidine 3% and adrenaline 0.5% in 1 eyedrop twice daily. The previous therapy was discontinued and the aim of the trial was to treat the patients with GA alone. There was an average decrease in intraocular pressure of 10.8 mmHg or 37.5% for the whole group (including 5 patients with additional therapy). In eyes with an average IOP in a day-curve without medication equal to or higher than 28 mmHg we found a decrease of 44.6% or 14.4 mmHg, and in eyes with an average IOP without medication between 21 and 28 mmHg a decrease of 30.4% or 7.6 mmHg. With GA alone the IOP was 3.3 to 3.9 mmHg lower than on the previous therapy (P less than 0.05); 46% of the eyes without additional therapy had all IOPs lower than 22 mmHg and 74% of the eyes had IOPs lower than 22 mmHg except 1 with a peak lower or equal to 25 mmHg 3 hours after application. This peak 3 hours after application indicates that GA has a biphasic action and was significant at the 0.5% level. Red eyes and slight ptosis were no problem for most patients. Patients found it very convenient to administer GA only twice daily.  相似文献

Sixty glaucomatous eyes of 60 patients treated with laser trabeculoplasty as primary therapy were reviewed retrospectively. There were 42 eyes with capsular glaucoma and 18 with simple glaucoma. The mean prelaser intraocular pressure (IOP) was 35.2 (SD = 6.5) mmHg. Success was defined as IOP less than or equal to 22 mmHg without medication. The probability of success was 0.73 at 1 year, 0.66 at 2 years, 0.57 at 3 years, and 0.50 at 4 years. Three eyes experienced progressive visual field loss or disc damage in spite of an intraocular pressure below 22 mmHg without medication. High prelaser pressure and the severity of the visual field defects were significant predictors of treatment failure.  相似文献

PURPOSE: To determine the long-term success rate after trabeculectomy without antimetabolites in patients with uveitis. METHODS: Review of data from all patients with uveitis who underwent trabeculectomy for uncontrolled intraocular pressure secondary to intraocular inflammation between May 1990 and December 1994. Results were compared with those from a group of patients with primary open-angle glaucoma matched for sex and surgeon. RESULTS: The uveitis group consisted of 32 eyes (20 patients). Maximum control of intraocular inflammation was achieved for a minimum of 2 months before surgery. Mean (+/-SD) age was 40.0 +/- 12.5 years (range, 14 to 67 years), with a median (+/-SE) follow-up of 53.0 +/- 1.8 months (range, 33 to 84 months). The primary open-angle glaucoma group consisted of 33 eyes (23 patients), with a mean age of 62.0 +/- 13.7 years (range, 26 to 81 years) and a median follow-up of 63.0 +/- 12.0 months (range, 34 to 299 months). The overall 5-year success rate (intraocular pressure < or =21 mm Hg with or without topical medication) for the uveitis group was 78% compared with 82% for the primary open-angle glaucoma group (P = .7). The absolute 5-year success rate (intraocular pressure < or =21 mm Hg with no medication) for the uveitis group was 53% compared with 67% for the primary open-angle glaucoma group (P = .87). CONCLUSIONS: In the absence of other risk factors, such as Afro-Caribbean race and previous intraocular surgery, and with adequate preoperative control of inflammation, trabeculectomy without antimetabolites may be considered the primary surgical procedure for increased intraocular pressure in patients with uveitis.  相似文献

Laser trabeculoplasty as primary therapy in chronic open angle glaucoma   总被引:1,自引:0,他引:1  
Sixty-three phakic eyes of 54 patients with early simple (21 eyes) or capsular (42 eyes) glaucoma treated with laser trabeculoplasty (LTP) as the primary therapy were followed retrospectively 12 to 18 months after LTP. In 59 of 63 eyes followed up for 12 months and 46 of 52 eyes followed up for 18 months the intraocular pressure (IOP) was constantly maintained below 22 mmHg with no medication. After a good initial response for 6 to 12 months, 6 eyes required medical therapy because of uncontrolled pressure or progression in the damage of the optic disc or visual fields. The mean pressure reduction of all eyes 18 months after LTP was 28% in simple and 37% in capsular glaucoma. No severe complications were seen. LTP seems to be an effective and safe procedure as the primary therapy for open angle glaucoma.  相似文献

目的 探讨高眼压下原发性闭角型青光眼复合式小梁切除术的临床疗效.方法 从60例(60只眼)联合应用药物24~48h眼压控制不佳的原发性闭角型青光眼患者中,随机选择38例(38只眼)施行复合式小梁切除术(观察组);22例(22只眼)行单纯小梁切除术(对照组),比较两组术后前房形成、眼压、滤过泡及并发症等情况.结果 60只眼手术顺利,未引发爆发性脉络膜出血等严重并发症,术后追踪8~15个月,术后第1天浅前房发生率观察组为5.3%,明显低于对照组22.7%(P<0.05).术后随访8个月时,观察组34只眼眼压控制在6-21mmHg(89.5%),对照组15只眼压控制在6-21mmHg(68.2%),两组间比较差异有统计学意义(x2=4.22 P<0.05),观察组功能性滤过泡占86.9%,较对照组功能性滤过泡63.6%为多(x2=4.42 P<0.05).结论 原发性闭角型青光眼持续高眼压状态下的复合式小粱切除术是安全有效的,对青光眼眼压持续24~48h不降者,应及时复合式小粱切除术,以防止视功能进一步损害,但必须掌握急症高眼压下的手术技巧才能提高成功率.  相似文献

BACKGROUND: In a collaborative study, patients with untreated normal-tension glaucoma were randomly assigned to a marked intraocular pressure reduction group or to a no therapy group. It was anticipated that medical therapy and laser trabeculoplasty would generally not achieve adequate pressure lowering and that fistulizing surgery would be required. This hypothesis was examined using current observations in the study. METHODS: Patients randomized to the therapy group had a pressure reduction of at least 30% from their last prerandomization level. This was achieved within 6 months by means of fistulizing surgery or with pilocarpine and/or laser trabeculoplasty. Beta-blockers and adrenergic agonists were excluded from both eyes. RESULTS: Of 30 patients with documented stable 30% pressure reduction, 17 (57%) achieved this with topical medication and/or laser trabeculoplasty: 8 with pilocarpine alone, 2 with laser trabeculoplasty alone, and 7 with laser trabeculoplasty after initial topical medication. The remaining 13 (43%) patients required a single fistulizing procedure. There was no statistically significant difference between the mean follow-up time for the nonfistulized group (533.8 +/- 437.6 days) and for the fistulized group (502.7 +/- 344.7 days). Both treatment groups had similar baseline profiles. CONCLUSION: Marked pressure reduction can be achieved and maintained on a long-term basis by means other than fistulizing surgery in a large proportion of patients with untreated normal-tension glaucoma.  相似文献

Argon laser iridotomy (ALI) was performed in 50 eyes for prophylactic treatment of anatomically narrow iridocorneal angles and in 50 eyes for therapy of chronic angle-closure glaucoma. Intraocular pressure was increased 6 mmHg or more 1 to 2 hours after ALI in 19 of 50 eyes with anatomical narrow iridocorneal angles and in 23 of 50 eyes with chronic angle-closure glaucoma. Increases greater than 20 mmHg over baseline value occurred in 5 of 50 eyes with narrow iridocorneal angles and in 7 of 50 eyes with chronic angle-closure glaucoma. A clinically significant increase in intraocular pressure (defined as a pressure 30 mmHg or greater and 40% or more increased over the pre-laser value) occurred 1 to 2 hours after ALI in 11 of 50 eyes with narrow iridocorneal angles and in 17 of 50 eyes with chronic angle-closure glaucoma. There was no statistical difference (chi square P greater than 0.3) in the incidence of this complication in the two groups. Additional medical therapy was effective in lowering the acute laser-induced elevation in intraocular pressure. Patient diagnosis, patient demographics, preoperative glaucoma medication and laser treatment parameters did not predict which eyes would develop this complication. Eyes which did not have a clinically significant elevation in intraocular pressure 1 to 2 hours after ALI did not show a later increase at 24 hours.  相似文献

Argon laser trabeculoplasty was performed in 33 eyes with primary open-angle glaucoma whose intraocular pressures were poorly controlled on medical therapy. Five eyes (three patients) were failures and underwent trabeculectomy within three months. The remaining 28 eyes were followed up for 18 months and received a complete examination at periodic intervals. Beginning three months after trabeculoplasty, an attempt was made to decrease medications in a prospective manner in order to determine the least treatment required to maintain a pressure of less than 22 mmHg. No medication could be discontinued in 39% of eyes, whereas 18% were "cured" by trabeculoplasty and required no further medical therapy for control. Nine of 14 eyes that had been treated with carbonic anhydrase inhibitors were controlled without these drugs after trabeculoplasty. In 57% of eyes, the miotic could be discontinued. A total of 82% of cases still required some medication for pressure control after laser trabeculoplasty.  相似文献

A controlled, randomised, prospective trial of Argon laser trabeculoplasty (ALT) was carried out on 48 eyes of 30 black Jamaican patients with primary open-angle glaucoma. All eyes had uncontrolled intra-ocular pressures (22mm Hg) despite medical therapy.Treatment was successful in controlling intraocular pressure in 68% of eyes after 12 months follow-up (p = 0.004). The average drop in intraocular pressure attributable to ALT was 6.6 mmHg. Argon laser trabeculoplasty is an effective adjunct in managing to control intraocular pressure in black Jamaican glaucoma patients.  相似文献

In this paper we present our experience of the last three years in the surgical treatment of eyes with closed angle glaucoma. We have performed an extracapsular lens extraction and posterior chamber intraocular lens implantation on 34 eyes of 34 patients. All of them were affected by closed angle glaucoma with variable control after a Yag laser iridotomy: 6 eyes had high I.O.P. notwithstanding maximal therapy, 11 eyes had I.O.P. under control (less than 21 mmHg) without therapy, 9 with I.O.P. controlled with topical therapy, 8 with I.O.P. controlled with maximal therapy (C.A.I, included).The cases with well controlled glaucoma were operated on because of the presence of a more or less significant lens opacities. After a follow-up of up to 40 months (mean=20.3, range=1–40), all eyes show satisfactory intraocular pressure and no eye needed a filtering procedure. The results of our studies are as follows (values are mean±SD). In the group of 6 eyes with high I.O.P., the mean pre-operative intraocular pressure was 29.7±5.6 mmHg and the mean post-operative I.O.P. was 15.1±1.4mmHg. The mean reduction was 14.5±6.6mmHg (p<0.005). In the 28 eyes with pre-operative I.O.P. under control (17.5±1.6), the mean post-operative I.O.P. was 14.4±2.3 mmHg, with a mean reduction of 3.1±3.1 mmHg (p< 0.005). Before the E.C.C.E., 11 eyes had I.O.P. less than 21 mmHg without anti-glaucoma medication, whereas after the E.C.C.E. 28 eyes did not need such a medication. No significant correlation was found between pre-operative and post-operative extension of peripheral anterior synechiae (when assessable) and post-operative intraocular pressure control. The difference between pre- and post-operative anterior chamber depth measured by ultrasonic biometry was remarkable, i.e. M=1.93+0.36mm and 3.42±0.2mm (pre-operative and post-operative, respectively; p<0.005).  相似文献

目的评价激光周边虹膜成形术联合激光瞳孔成形术与传统降眼压药物治疗原发性闭角型青光眼急性发作的有效性和安全性。 方法收集2014年1月至2015年7月于邯郸市眼科医院(邯郸市第三医院)就诊的62例(68只眼)确诊为原发性闭角型青光眼首次急性发作并未做降眼压治疗患者的病例资料。其中,男性15例(17只眼),女性47例(51只眼),平均年龄(67.5±8.5)岁,所有患者眼压升高≥40 mmHg(1 mmHg=0.133 kPa)。将入选患者采用数字表法随机分为激光组和药物组。激光组给予激光周边虹膜成形术联合激光瞳孔成形术治疗,药物组给予传统降眼压药物治疗。采用TA03型Icare眼压计测量治疗前、治疗后1 h和2 h的眼压,记录两组角膜及前房情况。以治疗后2 h眼压降至30 mmHg及以下为抢救成功,反之则为治疗失败。激光组和药物组年龄、就诊时眼压和发作时间等资料的描述采用均数±标准差表示,组间比较使用独立样本t检验;两组治疗成功率的描述采用眼数和百分比,比较采用χ2检验;两组治疗前、治疗后1 h和2 h眼压的比较采用两因素重复测量方差分析。 结果治疗后1 h激光组14只眼抢救成功,成功率为41.2%;药物组15只眼抢救成功,成功率为44.2%。治疗后2 h激光组18只眼抢救成功,成功率为52.9%;药物组20只眼抢救成功,成功率为58.2%。治疗后1 h与2 h两组间成功率无统计学意义( χ2=0.06,0.24;P>0.05)。激光组治疗前视力为光感至0.4,治疗后1 h视力为眼前手动至0.6,治疗后2 h视力为眼前手动至0.6。药物组治疗前视力为光感至0.4,治疗后1 h视力为光感至0.8,治疗后2 h视力为光感至0.8。两组主要并发症为前房内炎症反应和角膜内皮皱褶,无眼内感染和脉络膜下暴发性出血等严重并发症。激光组治疗前、治疗后1 h和2 h平均眼压分别为(56.9±8.7)mmHg、(37.9±16.2)mmHg和(32.9±16.4)mmHg,治疗后眼压下降,与治疗前的比较差异有统计学意义(t=6.02,7.76;P<0.05)。药物组治疗前、治疗后1 h和2 h平均眼压分别为(55.8±9.5)mmHg、(37.6±17.7)mmHg和(30.6±18.2)mmHg,治疗后眼压下降,与治疗前的比较差异有统计学意义(t=5.28,7.16;P<0.05)。但两组各时间点的眼压比较均无统计学意义(t=0.89,0.58,0.85;P>0.05)。 结论激光周边虹膜成形术联合激光瞳孔成形术和传统的降眼压药物治疗均能快速降低原发性闭角型青光眼急性发作期的眼压,但激光治疗安全、有效,可避免药物治疗给机体带来的副作用,是治疗原发性闭角型青光眼急性发作期的重要辅助措施。  相似文献

PURPOSE: To evaluate the intraocular pressure (IOP) after cessation of steroid use in steroid-induced glaucoma and its control with medication or surgery. METHODS: Thirty-four eyes of 34 patients having steroid-induced glaucoma were prospectively evaluated after cessation of steroid for IOP, visual acuity, and optic disc status at 3 months, and every 3 months for 18 months. RESULTS: Topical steroid use (73.5%) was the most frequent cause for glaucoma. The baseline IOP was 35.47+/-12.59 mmHg. The baseline vertical cup-disc ratio correlated with duration of steroid use (P=0.014) and the baseline IOP (P<0.0001). In 25 patients (73.5%), IOP could be controlled by topical medications alone, whereas nine patients (26.5%) required surgery. The mean baseline IOP in eyes requiring surgery was 49.67+/-13.28 mmHg and in eyes managed medically, 30.36+/-7.51 mmHg (P=0.002). The vertical cup-disc ratio in surgically treated patient was 0.87+/-0.13:1 as compared to 0.71+/-0.15:1 (P=0.012) in the medically treated group. At 6, 12, and 18 months follow-up, 22 (64.7%), 33 (97.1%), and all 34 (100%) patients were off treatment, respectively. CONCLUSIONS: Patients with steroid-induced glaucoma, who were 相似文献
