Lichen planus is an inflammatory dermatosis affecting the skin and mucous membrane. In oral lichen planus, the gingival tissue is often involved. Treatment is symptomatic and depends mostly on the administration of corticosteroids and tranquilizers. This report presents the case of a patient with gingival lichen planus who was treated with free gingival graft. Followup after 3 1/2 years showed that the lesions had completely disappeared.  相似文献

In the oral cavity, masticatory mucosa covers the hard palate and gingiva. The epithelium of masticatory mucosa is relatively thick compared to other oral tissues. This results in firmness and immobility necessary to withstand the forces of mastication. In the anterior aspect of the hard palate, long papillae within the lamina propria and thick dense collagenous tissue are found under the transverse ridges or rugae. Though palatal rugae are generally avoided during soft tissue grafting, there are few literature references detailing the consequences of transplanting tissue from the anterior palate which contains rugae. The following case report describes retained palatal rugae in a free gingival graft, which had been transplanted to an edentulous ridge 9 years earlier.  相似文献

BACKGROUND: There have been few cases reported of exostoses following a free gingival graft. In 1980, a free gingival graft was placed on the facial level of 33-34, developing over the years a significant enlargement. In 1999, since the patient felt progressively uncomfortable with the enlarged area, its surgical reduction was proposed. METHOD: Under local anesthesia, the hard tissue developed under the previously-grafted area, was significantly reduced. The specimen, together with a fragment of the covering soft tissue, was sent for histological analysis. RESULTS: The surgical wound healed uneventfully, and the patient was satisfied with the results. The histology showed the presence of mature bone surrounded by a dense connective tissue, whereas the gingival tissue showed acanthosis and fibrosis. CONCLUSION: The development of exostoses following a free gingival graft can be considered an unpredictable, albeit infrequent side-effect of this procedure. The fact that most of these exostoses appear in the cuspid-premolar area, deserves further consideration.  相似文献

Sinus bone grafting with autogenous bone is routinely performed to allow placement of endosseous dental implants. Although numerous maxillary sinuses have been successfully grafted, some complications of this procedure have been reported. These include maxillary sinusitis, resorption, infection and possible failure of grafts, loss of implants after 1-stage surgery, and oroantral fistulae. Only one case of postoperative maxillary cyst developing in the graft has been reported in the literature. Here, the authors report a similar case that necessitated grafting of the cyst with autologous iliac bone.  相似文献

A technique designed to utilize the autogenous free gingival graft without the use of sutures was demonstrated. Necessary objectives were maintained but achieved without unnecessarily piercing the graft, and in less time. The only variance in results would be due to the underlying physical properties of the recipient site. This new approach to grafting avoids the cyanoacrylate from getting between the graft and recipient site, therefore acting as a foreign body. Also shown was the complete coverage of a denuded root surface by this technique using a free autogenous gingival graft. This technique enables the surgeon to achieve the results of a predictable technique (autogenous free gingival graft) even more predictably simple.  相似文献

A rare case of large idiopathic gingival fibromatosis of right maxilla and mandible with extensive osseous destruction is presented. Following examination and definitive diagnosis the patient was treated by gingivectomy and tooth extraction. A denture was substituted at a subsequent time. A variety of systemic drugs can lead to adverse effects in the oral environment. This article reports the case of a 61-year-old man who had a severe drug-induced gingival overgrowth (DIGO) caused by nifedipine. DIGO is relevant due to severe gingival enlargement, which causes disfigurement and blocks physiological and social functions such as mastication and speaking. Management of DIGO is always a challenge due to the patient's systemic condition. This article shows, step-by-step, how the treatment was executed and how the DIGO was reversed.  相似文献

BACKGROUND/AIMS: Hereditary gingival fibromatosis is characterized by various degrees of attached gingival overgrowth. It usually develops as an isolated disorder but can be one feature of a syndrome. A case of a 38-year-old female is reported who presented a generalized severe gingival overgrowth, involving the maxillary and mandibular arches and covering almost all teeth. The clinical differential diagnosis included drug-induced overgrowth as well as idiopathic gingival fibromatosis. TREATMENT: Excess gingival tissue was removed by conventional gingivectomy. As the gingival enlargement was generalized to all quadrants, on both sides, the surgery was carried out under general anaesthesia. The postoperative course was uneventful and the patient's appearance improved considerably. Post-surgical follow-up after 20 months demonstrated a slight recurrence CONCLUSIONS: Hereditary gingival fibromatosis is a rare disorder characterized by the proliferative fibrous overgrowth of the gingival tissue. Resective surgery of the excess tissue is the treatment available. However, recurrence is a common feature.  相似文献
