BACKGROUND: Work-related upper limb amputations account for a significant number of occupational injuries. This study analyzes the nationwide incidence of work-related amputations of upper limbs in Taiwan by using the workers' compensation database. METHODS: A total of 2,950 cases were retrieved and analyzed from the database of workers' compensatory disability claims between the years 1999 and 2001. RESULTS: The cases were workers with an average age of 39.1 years, of whom 23.0% were female, the highest percentage of cases was in the 30-49 age group, but the incidence based on insured population was the highest for those under 20 years old. About 96% of the cases involved finger amputation, and 85% of all amputations were related to common powered machines. The average annual incidence of disability claims due to occupational amputation of upper extremities was 12.5 per 100,000 workers. Claims were most prevalent among manufacturing workers and male workers. The male to female risk ratios ranged from 2.62 for manufacturing workers to 9.05 for transport, storage and communication workers by the Poisson regression analysis. CONCLUSIONS: Young male manufacturing workers were at high risk of occupational amputation of upper extremities in Taiwan. These results provide an epidemiological base for developing a prevention strategy.  相似文献

To investigate occupational determinants of bladder cancer in the urban area of Shanghai, occupation and industry information for 1,219 incident bladder cancer cases diagnosed during the period 1980 to 1984 were compared with 1982 census data on employment. Standardized incidence ratios (SIR) for bladder cancer were estimated for occupation and industry classifications. Significant excess risks were observed for plastic products workers (male: SIR = 432; female: SIR = 368); textile bleachers, dyers, and finishers (male: SIR = 169); metal refining and processing workers (male: SIR = 139; female: SIR = 197); petroleum refining workers (male: SIR = 2152); railway engine drivers and firemen (male: SIR = 683); and workers employed in industries of apparel and other textile products manufacturing (female: SIR = 204); paper processing (male: SIR = 146; female: SIR = 226); organic chemical manufacturing (male: SIR = 186); plastic product manufacturing (male: SIR = 218; female: SIR = 272); and metallurgy (male: SIR = 107; female: SIR = 561). This study indicates that many of the industries and occupations that are responsible for increased risk throughout the world are also associated with occupational bladder cancer in Shanghai.  相似文献

A cohort study was conducted to evaluate the mortality pattern among female and male workers in a cigarette factory. The study followed 972 female workers and 761 male workers with at least 6 months of cumulative employment from 1 January 1962 through 1 July 1996. Among women, mortality from all causes of death [standardized mortality ratio (SMR) = 0.9; 95% confidence interval (CI) = 0.8-1.1] and mortality from all malignant neoplasms (SMR = 1.1; CI = 0.9-1.3) were consistent with reference rates. Male workers had a significantly reduced overall mortality (SMR = 0.8; CI = 0.7-0.9), while mortality from all malignant neoplasms was as expected (SMR = 0.9; CI = 0.7-1.0). Among female workers the frequencies of deaths from diseases of nervous system (SMR = 2.0; CI = 1.1-3.4) and from non-Hodgkin's lymphoma (NHL) (SMR = 2.7; CI = 1.0-5.6) were elevated at a statistically significant level. No association between duration of employment and diseases of nervous system was observed. A higher risk for NHL, based on three deaths, was reported among female workers with 15 or more years of employment (SMR = 8.1; CI = 2.2-21.0). Although based on small numbers, the excess of NHL here reported suggests that potential exposure to foliar residues of pesticides should be thoroughly considered in tobacco manufacturing.  相似文献

BACKGROUND: The health of UK petroleum industry workers has been monitored for many years. AIM: To identify any long-term adverse health outcomes from occupational exposures in this industry. METHODS: The mortality (1951-2003) and cancer morbidity (1971-2003) experienced by cohorts of 28,555 oil refinery workers and 16,477 petroleum distribution workers has been investigated. Study subjects were all those males first employed in the period 1946-74 at one of eight UK oil refineries or 476 UK petroleum distribution centres; all subjects had a minimum of 12 months employment with some employment after 1 January 1951. Observed numbers of cause-specific deaths and site-specific cancer registrations were compared with expectations based on national mortality and cancer incidence rates. RESULTS: Standardized mortality ratios (SMRs) were significantly <100 for all causes both in oil refinery workers (Obs 11,156, SMR 89) and in petroleum distribution workers (Obs 7320, SMR 96). Significantly elevated SMRs were shown in oil refinery workers for cancer of the pleura (mesothelioma) (Obs 64, SMR 261) and melanoma (Obs 48, SMR 168). Significantly elevated SMRs were not found in petroleum distribution workers for any site of cancer. Significantly elevated standardized registration ratios (SRRs) were only shown in oil refinery workers and for cancer of the pleura (mesothelioma) (Obs 115, SMR 274), melanoma (Obs 85, SMR 129) and other skin cancer (Obs 983, SRR 117). CONCLUSIONS: The only findings that showed clear evidence of an occupational cancer hazard were those for mesothelioma in oil refinery workers.  相似文献

Background: Construction workers are potentially exposed to many health hazards, including human carcinogens such as asbestos, silica, and other so-called "bystander" exposures from shared work places. The construction industry is also a high risk trade with respect to accidents.

Methods: A total of 19 943 male employees from the German construction industry who underwent occupational health examinations between 1986 and 1992 were followed up until 1999/2000.

Results: A total of 818 deaths occurred during the 10 year follow up (SMR 0.71; 95% CI 0.66 to 0.76). Among those were 299 deaths due to cancer (SMR 0.89; 95% CI 0.79 to 1.00) and 312 deaths due to cardiovascular diseases (SMR 0.59; 95% CI 0.51 to 0.68). Increased risk of mortality was found for non-transport accidents (SMR 1.61; 95% CI 1.15 to 2.27), especially due to falls (SMR 1.87; 95% CI 1.18 to 2.92) and being struck by falling objects (SMR 1.90; 95% CI 0.88 to 3.64). Excess mortality due to non-transport accidents was highest among labourers and young and middle-aged workers. Risk of getting killed by falling objects was especially high for foreign workers (SMR 4.28; 95% CI 1.17 to 11.01) and labourers (SMR 6.01; 95% CI 1.63 to 15.29).

Conclusion: Fatal injuries due to falls and being struck by falling objects pose particular health hazards among construction workers. Further efforts are necessary to reduce the number of fatal accidents and should address young and middle-aged, semi-skilled and foreign workers, in particular. The lower than expected cancer mortality deserves careful interpretation and futher follow up of the cohort.

PURPOSE: A retrospective cohort mortality study based on standardized mortality ratios (SMRs) was conducted to investigate the possible association between exposure to chlorinated organic solvents and various types of cancer deaths. METHODS: Vital status and causes of death of study subjects were determined from January 1, 1985 to December 31, 1997 by linking cohort data with the National Mortality Database. Person-year accumulation began on the date of entry to the cohort, or January 1, 1985 (whichever came later), and ended on the closing date of the study (December 31, 1997), if alive; or the date of death. RESULTS: This retrospective cohort study examined cancer mortality among 86,868 workers at an electronics factory in the northern Taiwan. Using various durations of employment and latency and adjusting for age and calendar year, no significantly elevated SMR was found for any cancer in either male or female exposed workers when compared with the general Taiwanese population. In particular, the risk of female breast cancer was not found to be increased. Although ovarian cancer suggested an upward trend when analyzed by length of employment, ovarian cancer risk for the entire female cohort was not elevated. CONCLUSIONS: It is concluded that this study provided no evidence that exposure to chlorinated organic solvents was associated with human cancer risk.  相似文献

This study assesses differences mortality patterns and relative hazard due to fatal occupational injuries between native and immigrant workers in the US. Fatal occupational injury data from 2003 to 2010 were examined using survival analysis based on proportional hazards models controlling for categorical variables of race, gender, occupation, and industry. Workers are stratified based on whether they are native to the US (n?=?31952) or born abroad (n?=?7096). Foreign-born workers are further stratified into region of birth. Foreign-born workers had an adjusted hazard ratio of 1.148 (95?% CI 1.109:1.189) relative to native workers. Stratifying foreign-born workers into region of origin revealed significantly higher adjusted risk of work fatality relative to native workers for most foreign regions. Of fatally injured workers, foreign-born workers have shorter survival before succumbing to traumatic injury during their time of occupational ‘exposure’ in the workforce. Native-born workers tend to incur fatal injuries at older ages after longer ‘exposure’.  相似文献

目的了解涉外电子企业职业伤害患者的性别、年龄、职业、伤害类别、伤害程度以及部位等人群流行病学特征,探讨影响职业伤害的有关因素,为制定有效的干预措施提供理论依据。方法采用分层随机整群抽样方法,在深圳市宝安区西乡镇分层随机抽取12家电子行业的工厂共6041名员工作为样本,调查2003年1月1日-12月31日期间职业伤害情况,对其伤害情况进行描述性统计分析。结果职业伤害发生率为16.44%,标化伤害发生率为17.18%;死亡率为33.11/10万。其中男职业伤害发生率为23.76%,女为14.83%。职业伤害主要以16-20岁组男性的伤害率最高(35.80%);工龄为0-1a和3-4a组的职业伤害发生率居一、二位,分别为25.87%、19.32%;职业伤害发生率的前3位依次是:搬运工(39.85%)、切割工(23.22%)、啤压工(21.43%);伤害类型前2位是:刺割伤(36.66%)、灼烫(32.62%)。结论涉外电子行业职业伤害发生率较普通人群高,应引起相关部门的重视并采取相应的预防及控制措施。  相似文献

A cohort of 537 workers employed for less than one year between 1900 and 1964 in the Geneva perfumery industry was followed up from entry to the end of 1983. During the period of study, 251 workers died and 41 (8%) were lost to follow up. The standardised mortality ratio (SMR) was significantly above 100 for all causes (SMR = 120), all cancers (SMR = 127), lung cancer (SMR = 186), and violent death (SMR = 179). The highest SMR from all causes was associated with the shortest period of employment (less than two months) and it decreased significantly with longer duration. Such mortality excesses had not been recorded among the 1168 workers of the same industry employed one year or more, previously studied in similar fashion. Interviews among a random sample of 52 workers employed for less than two months seem to indicate that the prevalence of smoking, exposures to asbestos, and occupational accidents in other hazardous industries were higher for these workers than for the reference population. Furthermore, unmarried men were overrepresented among the study cohort. These findings support previous observations indicating that short term workers share atypical features related to high mortality from various causes. It is suggested that mortality in this subgroup should be analysed separately in occupational studies.  相似文献

The objective of the study was to determine if female workersin a heavy manufacturing environment have a higher risk of injurycompared with males when performing the same job and to evaluatesex differences in type or severity of injury. By use of humanresources and incident surveillance data for the hourly populationat 6 US aluminum smelters, injuries that occurred from January1, 1996, through December 21, 2005, were analyzed. Multivariatelogistic regression, adjusted for job, tenure, and age category,was used to calculate odds ratios and 95% confidence intervalsfor female versus male injury risk for all injuries, recordableinjuries, and lost work time injuries. The analysis was repeatedfor acute injuries and musculoskeletal disorder-related injuriesseparately. Female workers in this industry have a greater riskfor sustaining all forms of injury after adjustment for age,tenure, and standardized job category (odds ratio = 1.365, 95%confidence interval: 1.290, 1.445). This excess risk for femaleworkers persisted when injuries were dichotomized into acuteinjuries (odds ratio = 1.2) and musculoskeletal disorder-relatedinjuries (odds ratio = 1.1). 目的了解金属制造业非致死性职业伤害流行特征,为有效控制职业伤害的发生提供科学依据。方法采用随机整群抽样方法,抽取7家金属制造工厂中的2487名工人进行问卷调查,分析非致死性职业伤害发生特征和影响因素。结果调查人群非致死性职业伤害的年发生率为15.08%;事故类型以机器伤害(16.80%)、物体打击(13.60%)和热/火烫(11.47%)为主;伤害部位主要为手指/脚趾(78.16%)。多因素L0gistic回归分析显示,个性维度中的N分较高(情绪稳定性差)、年龄小、文化程度及技术等级低、采用计件/计时方式计算工资等因素。可使职业伤害发生的危险性升高。结论职业伤害已成为重要的公共卫生问题。应引起社会的广泛关注。  相似文献

Cancer mortality in a cohort of male German iron foundry workers   总被引:1,自引:0,他引:1  
BACKGROUND: Observations of an increased incidence of cancers of the upper aero-digestive tract (pharynx, esophagus, larynx, lung) among workers of local German foundries gave rise to concern about a potentially elevated occupational risk of those cancer sites. The purpose of the study was to examine whether occupational exposure in iron foundries increases the risk of cancer. METHODS: A historical cohort study of 17,708 male German production workers in 37 iron foundries who were first employed in 1950-1985 with a minimum employment period of 1 year was initiated. Employment and occupational histories were collected. Mortality was compared with that of the German general population during 1950-1993 using a new method for computing the SMR when not all causes of death are available (called SMR*). RESULTS: Mortality from all causes was elevated to SMR = 115.4 (95% confidence interval (CI) = 111.9-119.1), as was for total cancer (SMR* = 123.8, CI = 102.1-152.6), especially cancers of the lung (SMR* = 163.9, CI = 123.9-223.0) and liver (SMR* = 322.5, CI = 149.5-844.8), and diseases of the respiratory system (SMR* = 147.6, CI = 100.4-221.5). Non-significant elevations of mortality were also found for cancers of the mouth and pharynx (SMR* = 153.5, CI = 82.3-359.8) and larynx (SMR* = 173.1, CI = 85.5-550.5). Mortality from various causes of death was higher among workers with shorter exposure periods than among long-term employees. The elevated mortality persisted for years and decades after termination of employment. CONCLUSIONS: The results provide further evidence for an increased risk of lung cancer and possibly other cancers of the upper aero-digestive tract among foundry workers. Special attention should be paid to the strongly increased mortality from liver cancer and the mortality pattern among employees having terminated work.  相似文献

BACKGROUND: Occupational eye injuries have been recognized as a serious health risk to workers and are in need of further investigation to develop effective interventions. METHODS: Rhode Island workers' compensation claims of ocular injury between 1998 through 2002 (n=8,877) were examined. The Current Population Survey was used to estimate occupational employment levels as a baseline for rate calculations. RESULTS: The estimated ocular injury claim rate was 32.9 per 10,000 workers (95% CI=32.3-33.6), with the cost of claims totaling $1,514,666 and averaging $171 per claim. The highest estimated claim rate of all occupations was found for construction laborers of 373.7 per 10,000 workers (95% CI=267.1-480.3). Relative to the durable manufacturing industry, the highest risk of injury resulting in disability indemnification was the wholesale trade industry (OR=2.18, 95% CI=1.19-4.01, P<0.05). CONCLUSIONS: Many of the eye injuries reported were likely preventable. Greater diligence, training, and safety precautions are needed to reduce the risk of eye injury to employees.  相似文献

Workers in the dye industry are exposed to various chemicals, of which reactive dye is a well-known occupational asthmagen. This study examined the relationship between asthma mortality and occupational exposure in the dye industry. The cohort comprised 66,089 male workers, including 904 workers in the dye industry, who underwent medical examinations from 1995 to 2003 at a medical service institute located in Incheon, Korea. Deaths were also observed during the 1995 to 2003 period. The mortality was analyzed using the standardized mortality ratio (SMR) compared to the Korean general population, and the mortalities from asthma in dye manufacturing workers and other workers were compared using the standardized rate ratio (SRR). The all-cause mortality in dye industry workers was significantly lower than in the general population (SMR=0.40, 95% CI 0.24-0.63), while the asthma mortality (SMR=9.03, 95% CI 1.86-26.39) was significantly higher. Deaths from non-malignant respiratory diseases were higher in dye industry workers, but were not statistically significant (SMR=2.0, 95% CI 0.41-5.86). In the internal comparison, the all-cause mortality was the same in both groups, while the mortalities from non-malignant respiratory diseases (SRR=6.04, 95% CI 4.00-9.44) and asthma (SRR=23.29, 95% CI 11.05-58.83) were higher in dye industry workers than in workers in other industries. Because asthma is a life-threatening disease, special consideration and preventive measures must be taken for workers in the dye industry.  相似文献

Although workers in meatpacking facilities in the U.S. experience high rates of occupational injury, their injury experiences have received limited research attention. Prior research indicates underreporting in injury rates in this industry as well significant variation in injury rates among facilities. To add detail to the rates and circumstances surrounding occupational injury among meatpacking workers, we conducted a cross-sectional study of workers employed at an industrial beefpacking plant in Nebraska (n = 137) and interviewed workers about recent injury experiences. We assessed frequency, cause and nature of self-reported injury. We estimated annual incidence rates of self-reported injuries using the OSHA formula and compared these rates to industry-wide data. We also evaluated psychological distress in this workforce as measured by the Kessler-6 scale to assess whether distress was associated with recent occupational injury. In this study, 15.1% of workers experienced occupational injuries that required time off work, job transfer, or restriction during the past three months. The estimated annual incidence rate was 15.2 injuries per 100 full-time workers for these injuries at this plant. Rushing was identified as the cause of nearly 50% of injuries, and repetitive work as the cause of an additional 20% of injuries. Use of metal mesh sleeves (POR: 0.10 (p = 0.008)) and metal mesh gloves (POR: 0.41 (p = 0.05) were associated with reduced risk of injury. Use of a carbon steel for knife sharpening (POR: 5.2 (p = 0.02)) was associated with elevated risk of moderate and severe injury. There were no associations between self-reported occupational injury and overall measures of psychological distress. Self-reported incidence rate of severe injury in this plant was more than twice official industry estimates. Worker self-reports may illustrate key areas for injury prevention.  相似文献

These workers were all male operatives first employed in any one of the 10 participating foundries in 1946-65; all had worked in the industry for a minimum period of one year. Compared with the general population of England and Wales, statistically significant excesses relating to cancer mortality were found for cancer of the stomach (E = 77.4, O = 106, SMR = 137) and cancer of the lung (E = 229.2, O = 441, SMR = 147). A statistically significant deficit was found for cancer of the brain (E = 19.4, O = 10, SMR = 51). Involvement of occupational exposures was assessed by the method of regression models and life tables (RMLT). This method was used to compare the duration of employment in the industry, in "dust exposed" jobs, in "fume exposed" jobs, in foundry area jobs, in fettling shop jobs, and in foundry area or fettling shop jobs, of those dying from cancers of the stomach and lung with those of all matching survivors. The RMLT analyses provided evidence of an occupational involvement in the risk of death from lung cancer from work in the foundry area or fettling shop, and weaker evidence of an occupational involvement in the risk of death from stomach cancer from work in the foundry area.  相似文献

STUDY OBJECTIVE: In the past decade, Spain has experienced dramatic growth of its immigrant population. Available information on the occupational conditions of foreign workers is scarce. This study aims to add to this information by describing occupational injuries in foreign workers in Spain. Design, setting, PARTICIPANTS: Data were analysed from the 2003 Ministry of Labour and Social Issues registry of non-fatal and fatal occupational injury in insured workers. The population at risk was estimated from the Social Security Affiliation Registry as of 31 December 2003. Comparing Spanish with foreign workers and also considering age and sex, incidence rates and relative risks, and their confidence intervals at 95%, were calculated within each population group. MAIN RESULTS: In women and in men, and in every age group, foreign workers had an increased risk of non-fatal and fatal occupational injury compared with Spanish workers. The differences were especially notable in foreign women workers and in older workers. CONCLUSIONS: Many factors probably combine to cause the differences found in this study. Better data collection on the situation of foreign workers is needed to understand these facts and apply appropriate public health solutions.  相似文献

OBJECTIVES: To evaluate the risk of cancer and other diseases among workers engaged in aircraft manufacturing and potentially exposed to compounds containing chromate, trichloroethylene (TCE), perchloroethylene (PCE), and mixed solvents. METHODS: A retrospective cohort mortality study was conducted of workers employed for at least 1 year at a large aircraft manufacturing facility in California on or after 1 January 1960. The mortality experience of these workers was determined by examination of national, state, and company records to the end of 1996. Standardised mortality ratios (SMRs) were evaluated comparing the observed numbers of deaths among workers with those expected in the general population adjusting for age, sex, race, and calendar year. The SMRs for 40 cause of death categories were computed for the total cohort and for subgroups defined by sex, race, position in the factory, work duration, year of first employment, latency, and broad occupational groups. Factory job titles were classified as to likely use of chemicals, and internal Poisson regression analyses were used to compute mortality risk ratios for categories of years of exposure to chromate, TCE, PCE, and mixed solvents, with unexposed factory workers serving as referents. RESULTS: The study cohort comprised 77,965 workers who accrued nearly 1.9 million person-years of follow up (mean 24.2 years). Mortality follow up, estimated as 99% complete, showed that 20,236 workers had died by 31 December 1996, with cause of death obtained for 98%. Workers experienced low overall mortality (all causes of death SMR 0.83) and low cancer mortality (SMR 0.90). No significant increases in risk were found for any of the 40 specific cause of death categories, whereas for several causes the numbers of deaths were significantly below expectation. Analyses by occupational group and specific job titles showed no remarkable mortality patterns. Factory workers estimated to have been routinely exposed to chromate were not at increased risk of total cancer (SMR 0.93) or of lung cancer (SMR 1.02). Workers routinely exposed to TCE, PCE, or a mixture of solvents also were not at increased risk of total cancer (SMRs 0.86, 1.07, and 0.89, respectively), and the numbers of deaths for specific cancer sites were close to expected values. Slight to moderately increased rates of non-Hodgkin''s lymphoma were found among workers exposed to TCE or PCE, but none was significant. A significant increase in testicular cancer was found among those with exposure to mixed solvents, but the excess was based on only six deaths and could not be linked to any particular solvent or job activity. Internal cohort analyses showed no significant trends of increased risk for any cancer with increasing years of exposure to chromate or solvents. CONCLUSIONS: The results from this large scale cohort study of workers followed up for over 3 decades provide no clear evidence that occupational exposures at the aircraft manufacturing factory resulted in increases in the risk of death from cancer or other diseases.
