Ethics

This article is from the 1989 CONTEMPO issue of the Journal of the American Medical Association, the purpose of which is to keep physicians informed of recent developments in different areas of medicine through brief overviews by specialists in each field. In his article on ethics, Pellegrino focuses on the issues of euthanasia and fetal research. The practice of active, voluntary euthanasia raises questions about the difference between killing a terminally ill patient and withholding or withdrawing life-prolonging treatment, the limits of patient autonomy, the compatibility of active euthanasia with professional ethics, and the social consequences of legalizing euthanasia. The debate over the use of fetal tissue for research and treatment centers on the issue of induced abortion.     

Ethics

In the annual Contempo issue, the editors of the Journal of the American Medical Association publish updates by experts in selected specialties of major issues and innovations in various subfields of medicine. Pellegrino's Contempo '90 essay on ethics focuses on the ethical impact of recent AIDS research, particularly the availability of early treatment with drugs that, while not curative, may prolong and enhance the quality of life for persons with AIDS. Among the issues Pellegrino highlights are screening and confidentiality, access to care and clinical trials, the design of clinical trials, the obligations of health personnel and of persons with AIDS, resource allocation, and needle exchange programs.

Ethics: A Theory of Medical Ethics

This book review characterizes Robert Veatch's A Theory of Medical Ethics as a "third-generation" treatise that looks beyond case- and issue-oriented analysis to develop the theoretical bases of a "true system of medical ethics." Veatch proposes a "draft medical ethical covenant" based on a "triple contract" model, in which the moral principles of contract keeping, autonomy, honesty, avoiding killing, and justice govern the physician's relationship to both individual patients and society.
