检测血清中BCG—A60抗体对结核病诊断的临床意义   总被引:1,自引:0,他引:1  
BGC-A60是从牛型结核分支杆菌的胸浆中提纯出来的抗原,为了解用聚合酶联免疫法检测血清中抗BCG-A60抗体对结核诊断的临床意义,对734例健康人和非结核病人,以及560例结核病患者作了抗BCG-A60抗体的检测。结果证实,ELISA-A60.IgG诊断结核病的特异为89.3%,敏感性为84.5%,获得较满意的效果,认为可用于临床对结病的辅助诊断,还发现在活动性原发型肺结核中IgM的阳性率明显高  相似文献

目的评估脂阿拉伯甘露糖─IgG(LAM-IgG)和包膜蛋白─IgG对活动性结核的诊断价值。方法两种抗原分别采用快速和常规酶朕免疫吸附法(ELISA)进行检测。选择108例活动性结核(其中97例活动性肺结核、11例肺外结核),69例非结核肺部疾患(其中肺癌16例,其它肺部疾患53例)血清平行检测结核分支杆菌IgG抗体。结果97例活动性肺结核涂阳敏感性分别为71.8%(28/39)和92.3%(36/39),涂阴为60.3%(35/58)和77.6%(45/58)。69例非结核肺部疾患组两法阳性分别为4例和14例,假阳性率分别为5.8%(4/69)和20.3%(14/69);特异性分别为94.2%(65/69)和79.7%(55/69)。结论提示快速ELISA血清LAM-IgG测定操作简便、快速、不需任何仪器设备且特异性较高,对活动性结核病有较高的临床辅助诊断价值。  相似文献

目的 评价RapidflTM试剂盒检测血清抗脂阿拉甘露酯IgG抗体对结核病的诊断价值。方法 采用快速斑点免疫金渗滤法检测活动性肺结核60例,骨结核6例,胸膜炎3例,脑膜炎2例,肺癌30例及30例健康人血清中LAM-IgG。结果 活动性结核病组LAM-IgG阳性率69%,肺癌及正常对照组LAM-IgG假阳性率5%,结论血清LAM-IgG测定对活动性结核病有较好的辅助诊断价值。  相似文献

本文采用快速ELISA法检测了肺结核、肺癌和健康人血清中LAM-IgG,以观察其在结核病诊断的吉核组阳性率82.2%,涂培组阳率96.4%,涂培组则为68.6%,而肺癌组及健康组假阳性率各为7.5%、3.4%。结果提示血清中LAM-IgG检测对活动性肺结核有辅助诊断意义。  相似文献

用ABC-ELISA法检测55例结脑和37例非结脑患者脑脊液抗PPD-IgG并与ELISA法比较。结果两法阳性率分别为96.4%和83.6%,ABC-ELISA法的敏感性高于ELISA法,P<0.05,而两法特异性均为97.3%。同时检测结脑患者脑脊液和血清抗PPD-IgG,脑脊液阳性率为96.4%高于血清阳性率为85.5。结果表明本法可为结脑诊断的一种辅助方法。  相似文献

目的 评估血清抗脂阿拉伯甘露聚糖38kDa抗体(LAM-38kDa-IgG)检测对涂阴肺结核和肺外结核的诊断价值。方法 采用斑点免疫金渗滤法检测57例涂阴肺结核,52例肺外结核,32例涂阳肺结核,29例肺癌患者及33例正常人血清中的LAM-38kDa-IgG。结果 涂阴肺结核组LAM-38kDa-IgG阳性率为73.7%,其中痰结核杆菌涂(-)培(+)组为84.6%,涂(-)培(-)组为70.5%;肺外结核组阳性率为71.2%;涂(+)肺结核组阳性率93.8%;肺癌组假阳性率31%;健康组假阳性率9.1%。结论 提示血清LAM-38kDa-IgG测定对涂阴肺结核和肺外结核有一定的辅助诊断价值。  相似文献

用聚合酶链反应(PCR)技术、单克隆抗体(McAb)TB15-C_3经酶联免疫吸附试验(ELISA)及抗酸染色等3种方法,对10种抗酸杆菌及2种非分支杆菌进行检测。PCR仅对结核分支杆菌复合体扩增出245bp特异性条带,McAb-ELISA检测除人型结核分支杆菌外,还与BCG、鸟型及瘰疬分支杆菌反应阳性,抗酸染色则所有分支杆菌均呈阳性。用PCR、McAb-ELISA及抗酸染色检测了124份临床标本,PCR的检出率高于抗酸染色涂片的阳性率(P<0.05),McAb-ELISA检测阳性率明显高于抗酸染色涂片和PCR的阳性率(P<0.01)。认为三种方法在使用中各自有其优点,不可偏废。  相似文献

Setting: An ELISA assay based on mycobacterial antigen 60 (A60) for the estimation of specific immunoglobulins in the serum has been used successfully for the rapid diagnosis of tuberculosis in studies done predominantly in Western countries. In a recent Indian study, encouraging results were reported in adult tuberculosis.Objective: To evaluate the utility of this ELISA test for rapid diagnosis of different clinical forms of tuberculosis in Indian children.Design: ELISA test based on mycobacterial A60 was used to estimate specific IgM, IgA and IgG antibodies in the sera obtained from 452 cases of tuberculosis and 161 controls in the paediatric population of Delhi, India.Results: Of the 161 controls, only 7.4% were positive for IgM, 4.3% for IgG, 3.7% for IgA and 8% when a combination of IgM and IgA was considered. Of 58 cases of definite pulmonary tuberculosis, 55.2% were positive for IgM, 32.7% for IgG, 36.2% for IgA and a high positivity of 72.4% was seen when IgA and IgM estimations were combined. The corresponding figures in 150 cases of definite extrapulmonary tuberculosis were 57.3%, 36.6%, 38% and 76.6%. A relatively weak serology was observed in 244 cases of probable tuberculosis. A very high positivity (95%) was seen in acid-fast bacilli-positive cases of tuberculosis.Conclusions: Our findings point to a very good specificity (92%) and a reasonably good sensitivity (75.5%) of the test when combined IgM and IgA antibody titres are considered in the diagnosis of childhood tuberculosis.  相似文献

Abstract An ELISA diagnostic test for tuberculosis antigen A60 (TBA60) IgG/IgM was used in a tertiary referral hospital in Taiwan. From June 1992 to December 1993, serum samples obtained from 907 patients were analyzed for TBA60 IgG and IgM titres. The final diagnosis of these patients was confirmed by microbiological study and clinical follow up for 18–24 months. Among 147 patients with active pulmonary tuberculosis, IgG was positive in 112 (76.2%), IgM was positive in 14 (9.52%). Among 90 patients with active extrapulmonary tuberculosis, IgG was positive in 53 (58.9%), IgM was positive in 9 (10%). Among 153 patients with inactive tuberculosis, IgG was positive in 28 (18.3%), IgM was positive in 1 (0.6%). Among 517 patients with nontuberculous disease, IgG was positive in 50 (9.7%), IgM was positive in 3 (0.6%). In this study population with 26% (237/907) active tuberculous infection rate, the TBA60 ELISA IgG had a diagnostic sensitivity of 69.6% and a specificity of 92.1%. These results indicate a positive predictive value of 67.9% and a negative predictive value of 89.2%. The sensitivity of IgM was 10.5% and specificity, 99.4%. The serum IgG titre had good correlation with the extent of pulmonary disease. Patients with smear-positive pulmonary TB had a higher percentage of IgG seropositivity (83.9%) than those with smear-negative pulmonary TB (70.6%) and extrapulmonary TB (58.9%). In 50 cases with active tuberculosis, follow- up examinations were carried out one month after treatment. In 18 cases with initially negative IgG and IgM titres, 13 showed elevation of serum IgG titres into positive level, one had positive seroconversion of IgM which was the only serological marker indicating active infection. Therefore, 77.8% (14/18) gained diagnostic benefit from follow-up serological examination. It was concluded that TBA60 IgG and IgM ELISA is a useful test when diagnosing tuberculosis. This test also assists in the clinical judgement of tuberculosis when used as an adjunct to symptoms and sputum smear, and for monitoring therapeutic response at the commencement of treatment.  相似文献

目的 探讨异种血清抗体检测技术在结核病诊断中的应用价值。方法 采用IgG/IgM抗体试剂盒分别检测102例结核病患者(包括73例肺结核和29例肺外结核)、223例其他肺部疾病患者和100例对照者结核感染情况,以临床诊断为标准评价该方法的敏感度和特异性,同时分别与痰菌培养及痰涂片平行检验的结果作比较,统计学分析采用χ2检验。结果 结核抗体IgG/IgM检测结核病患者的敏感度为74.51%、特异度为91.64%。结核抗体IgG/IgM检测肺结核和肺外结核的敏感度分别为82.19%、55.17%,肺内和肺外结核的敏感度差异有统计学意义(P<0.05),结核抗体lgG/IgM检测结核患者阳性检出率明显高于痰培养法和痰涂片法(P<0.05)。102例结核病患者年龄段分组分析,少年组和老年组检出率分别为58.33%、36%,远低于青年组和中年组的96.15%和89.74%。不同年龄组间进行卡方比较分析显示,P<0.05,差异有统计学意义。425例标本中,共发现8例非结核分枝杆菌,其中6例胞内分枝杆菌, 2例脓肿分枝杆菌,lgG/lgM抗体检测均为阴性。结论 IgG/IgM血清抗体检测肺内、外结核具有快速方便、经济和较高的敏感度,适合用于临床结核筛查。  相似文献

The objective was to apply the purified 38kDa protein antigen of Mycobacterium tuberculosis in ELISA to estimate the IgG, IgA and IgM antibody levels in sera and circulating immune complexes of tuberculosis patients. Sera from smear and culture positive tuberculosis patients were positive for anti 38kDa IgG, IgA and IgM antibodies, with a sensitivity of 61%, 30% and 10%, respectively, and with a specificity of 100% for IgG. The sensitivity of the test improved to a level of 68% for IgG+IgA and of 71.4% for IgG+IgA+IgM without significantly compromising the specificity (IgG of 100%, IgG+IgA of 96%, IgG+IgA+IgM of 90%). Among the smear, culture-negative but X-ray-positive cases, 60% were serum positive for IgG antibody, while in smear-negative but culture-positive cases, 54% were positive for IgG antibody. Measurement of 38kDa antibodies showed a greater than 95% sensitivity in smear and culture-positive, and smear-negative and culture-positive patients, through a combination of assays for serum IgG and circulating immune complex antibodies, while the specificity was 100%.  相似文献

目的 评价胶体金免疫层析法检测血清中结核分枝杆菌特异性IgG/IgM抗体在结核病诊断中的应用价值。方法 收集结核病患者和健康人的血清样本共332份及其背景资料,采用胶体金法检测血清中特异性结核抗体IgG/IgM,与临床诊断和细菌学检测结果比较,使用SPSS 22.0统计软件对结果进行比较分析,以P<0.05为差异具有统计学意义。结果 胶体金免疫层析法检测结核病患者中特异性结核抗体IgG/IgM的灵敏度为41.15%、特异性为91.67%,检测菌阳和菌阴结核病患者的敏感性分别为51.38%和33.77%。在全部结核病患者中,特异性结核抗体IgG/IgM检测检出率(41.15%)显著高于痰涂片法(18.84%)和痰菌培养法(36.15%)(P<0.05)。结核抗体检测、痰涂片法和痰培养法三种方法联合检测阳性率为61.54%,高于单种方法检测或其中两种方法联合检测。结论 胶体金免疫层析法检测血清中结核分枝杆菌抗体具有灵敏、特异、快速和简便等优点,可用于结核病的筛查,同时该方法也具有一定的局限性,敏感度和特异度有待进一步的提高,因此不可单独用于诊断结核病,可配合痰细菌学、影像学和临床表现等进行辅助诊断。  相似文献

目的 探讨结核分枝杆菌融合抗原38F和64F对活动性肺结核患者血清中IgG、IgM和IgA抗体检测的诊断价值。 方法 利用结核分枝杆菌抗原优势肽段融合抗原38F和64F,通过ELISA法对223例活动性肺结核患者进行结核分枝杆菌特异性IgG、IgM和IgA抗体水平检测。223例活动性肺结核患者分为三组,第一组为涂片和培养共同阳性组(86例),第二组为涂片阴性且培养阳性组(51例),第三组为涂片和培养共同阴性组(86例)。 结果223例活动性肺结核病患者,第一组IgG、IgM和IgA的联合检出率为79.07%(68/86);第二组IgG、IgM和IgA的联合检出率为62.75%(32/51);第三组IgG、IgM和IgA的联合检出率为69.77%(60/86)。全部样本血清学方法的检出率为71.75%(160/223)。结论 结核分枝杆菌融合抗原38F和64F对于活动性肺结核具有较高的辅助诊断价值,并且IgG、IgM和IgA抗体联合检测可以提高检出率。  相似文献

This investigation was undertaken to assess the effectiveness of an enzyme-linked immunosorbent assay (ELISA) using A60 antigen in ascertaining diagnosis in hospitalized patients suspected to have pulmonary tuberculosis (TB) but with negative sputum stains. Cultures were performed to confirm active or inactive disease. IgG and IgM antibody activity was determined by adding a 1:100 dilution of serum to plates coated with A60 antigen. After addition of peroxidase-conjugated antihuman IgG or IgM and color development, optical density (OD) was determined. A total of 83 patients was studied, taking into account their current disease status and prior history. Using as a cutoff value the mean value +/- 2 SD measured in the negative culture, no TB history group, that is, OD = 0.50 for IgG measurements and 0.43 for IgM measurements, the sensitivity, specificity, and positive predictive value of IgG measurements were equal to 48, 71, and 50%, respectively. Using IgM measurements, these parameters were equal to 76, 98, and 95%, respectively. Combining the results of IgG and IgM measurements, sensitivity, specificity, and positive predictive value were equal to 68, 100, and 100%, respectively. Thus, the ELISA described here can greatly facilitate the diagnosis of TB in patients with negative smears.  相似文献

An ELISA method for detecting serum antibodies against A60, an antigen prepared from the cytoplasm of Mycobacterium bovis BCG, has been applied to 385 subjects, namely 197 controls (neonates, healthy adults, and tuberculin negative, nontuberculous patients), and 188 subjects at various stages of tuberculous infection and disease. Most IgM determinations gave negative results. While the neonates and normal adults had titers of IgG anti-A60 antibodies below the cut off value, wide variations in antibody titers were observed among the various types of subjects infected by M. tuberculosis. The results obtained with nontuberculous subjects were: 100% negative IgG in neonates and healthy adult individuals and 6.4% "false positive" cases among 124 non-tuberculous patients. The percentage of serologically positive cases of tuberculosis was: 5.9% in latent active primary forms, 42.8% in patent active primary forms, and 82.8% in active postprimary forms. Tuberculous infections had a positively rate of 14.7%, while inactive postprimary tuberculosis had a positivity rate of 50%. The results obtained with A60 can favourably be compared with other serum ELISA tests for tuberculous antibodies against purified or semipurified mycobacterial antigens. Anti-A60 ELISA IgG antibody test can be useful to monitor the kinetics of humoral immunological response during tuberculous infection, disease and chemotherapy. A positive IgG ELISA test may support the diagnosis of active tuberculous disease.  相似文献

目的 评估结核分枝杆菌IgG和IgM抗体检测试剂的临床价值。方法 评估实验分2次进行:(1)于2014年5月选择于解放军第三〇九医院就诊的患者,经病历回顾性分析分为肺结核组(92例)和非结核对照组(99例),同时,选取94名健康对照者。(2)于2016年12月至2017年3月共选取解放军第三〇九医院就诊的118例患者血清标本,经病历回顾性分析分为结核病组(62例)和非结核对照组(56例)。对痰和支气管灌洗液进行涂片检查,收集研究对象血清标本应用结核分枝杆菌IgG和IgM抗体检测试剂盒(胶体金免疫层析法)进行检测。结果 第1次实验:应用结核分枝杆菌IgG和IgM抗体检测试剂盒检测研究对象血清中抗结核抗体的敏感度为64.1%(59/92),特异度为89.1%(172/193),阳性预测值为73.8%(59/80),阴性预测值为83.9%(172/205),一致率为81.1%(231/285)。92例肺结核患者中,IgG抗体检测的阳性率为64.1%(59/92),其中涂阳肺结核患者的阳性率为73.8%(31/42),涂阴肺结核患者的阳性率为56.0%(28/50),差异无统计学意义(χ 2=3.15,P=0.076);而IgM检测均为阴性。第2次实验:应用结核分枝杆菌IgG和IgM抗体检测试剂盒检测118例研究对象血清中不同抗结核抗体的敏感度为45.2%(28/62),特异度为78.6%(44/56),阳性预测值为70.7%(29/41),阴性预测值57.1%(44/77),一致率为61.9%(73/118)。62例结核病患者中,IgG抗体检测的阳性率为45.2%(28/62),其中,涂阳肺结核患者的阳性率为56.3%(18/32),明显高于涂阴肺结核患者的29.2%(7/24),差异有统计学意义(χ 2=4.07,P=0.044);而IgM检测只有1例阳性,阳性率为1.6%(1/62)。 结论 结核分枝杆菌IgG和IgM抗体检测试剂中,IgG检测对活动性结核病具有较好的辅助诊断价值,而IgM检测的临床价值尚需进一步评估。  相似文献
