Hip disorders in childhood

Childhood disorders of the hip are encountered frequently in paediatric orthopaedic practice. They present initially to paediatricians, general practitioners and accident departments, and it is therefore important for both surgeons and generalists to have a good working knowledge of the common presentations. The challenge is to distinguish between disease processes that are benign and self-limiting (e.g. transient synovitis), acute and joint threatening (e.g. septic arthritis) or chronic and disabling (e.g. Perthes' disease). This review primarily considers the epidemiology and aetiology of childhood hip diseases and provides a diagnostic framework, based upon age and risk factors. The important investigations and treatment options for each of the key differential diagnoses are also considered.     

Renal phosphate--wasting disorders in childhood

The purpose of this review paper is to provide a summary of recent developments in the childhood disorders of renal phosphate-wasting, with particular emphasis on two of the hereditary conditions, X-linked hypophosphatemia (XLH) and autosomal dominant hypophosphatemic rickets (ADHR), as they are distinguished from the paraneoplastic syndrome, oncogenic hypophosphatemic osteomalacia (OHO). An overview of the clinical manifestations, pathogenesis, diagnosis, and treatment of these conditions, with attention to newly-discovered genetic and hormonal signatures, will be discussed. The information may be invaluable in the complex diagnosis and successful treatment of OHO and hereditary hypophosphatemias of childhood.     

Blistering disorders in childhood.

Many conditions present in childhood with vesicles and bullae, ranging from benign conditions to life-threatening diseases. This article reviews selected blistering disorders, with emphasis on new information that is helpful to the practicing physician. Entities that are discussed in this article include childhood zoster and fetal varicella syndrome, urticaria pigmentosa, scabies, bullous bug bites, lichen sclerosus et atrophicus, epidermolysis bullosa, and erythema multiforme.
