| 摘 要： | 目的:探讨胃癌成骨性骨转移的临床及预后、18F-FDG PET/CT检查价值。方法:回顾分析3例胃癌成骨性骨转移患者的临床特征及18F-FDG PET/CT影像表现，并检索国内外数据库，对同类病例的临床资料进行汇总分析。结果:胃癌成骨性骨转移常伴有ALP、CA19-9、CEA水平升高及低钙血症，预后差，椎体为最常见转移部位（100%），在PET/CT上可表现为FDG高代谢结节或弥漫性骨质硬化伴稍高FDG代谢。结论:胃癌成骨性骨转移少见，首发症状不明显易造成误诊或漏诊，18F-FDG PET/CT有助于明确原发病灶及提高成骨性骨转移的诊断检出率。

| Abstract: | Objective:To investigate the clinical and prognosis of osteogenic bone metastasis of gastric cancer and the value of 18F-FDG PET/CT examination.Methods:The clinical and 18F-FDG PET/CT imaging manifestations of three patients with osteogenic bone metastases from gastric cancer were reviewed and analyzed,and the domestic and foreign databases were searched to summarize the clinical data of similar cases.Results:Osteogenic bone metastases from gastric cancer were often accompanied by elevated levels of ALP,CA19-9,CEA and hypocalcemia with poor prognosis.The vertebral body was the most common site of metastasis(100%),which could appear as FDG hypermetabolic nodules or diffuse osteosclerosis with slightly FDG hypermetabolismon PET/CT.Conclusion:Osteogenic bone metastases from gastric cancer are rare and may be misdiagnosed or missed if the first symptoms are not obvious.18F-FDG PET/CT can help to clarify the primary lesion and improve the diagnostic detection rate of osteogenic bone metastases. |
