炎症细胞因子在膝关节骨性关节炎滑液中的表达及临床意义

摘    要:
目的 探讨炎症细胞因子(IL-1、TNF-α、IL-6)在膝关节骨性关节炎(KOA)滑液中的表达及其与KOA临床分期的关系。方法 选择行膝关节镜和人工膝关节置换术的50例KOA患者为研究对象,并以30例单纯半月板损伤患者为对照组,采用酶联免疫吸附(ELISA)方法分别测定滑液中IL-1、TNF-α、IL-6含量。结果 与对照组比较,KOA组滑液中IL-1、TNF-α、IL-6水平显著上升,相比较有显著性差异(P<0.05)。IL-1、TNF-α水平随着临床分期的增加而逐渐升高,滑液中IL-1、TNF-α水平为:晚期>中期>早期,相比较均有显著性差异(P<0.05);IL-6水平随着临床分期的增加而下降,滑液中IL-6水平为:早期>中期>晚期,相比较均有显著性差异(P<0.05)。IL-1、TNF-α与KOA临床分期呈正相关性(r=0.567,0.583,P<0.05),IL-6与KOA临床分期呈负相关性(r=-0.486,P<0.05)。结论 KOA患者滑液中存在IL-1、TNF-α、IL-6水平的高表达,IL-1、TNF-α随着临床分期的增加其含量逐渐上升,而IL-6随着临床分期的增加其含量逐渐降低。IL-1、TNF-α与骨性关节炎病变有密切关系,而KOA患者关节液中IL-6的含量不能直接反映出骨性关节炎的严重程度,但并节液中IL-6水平在诊断早期病变有一定的参考价值。

Abstract:
Objective To explore the inflammatory cytokines (IL-1, TNF-α , IL-6) in knee osteoarthritis (KOA) expression in synovial fluid and its relationship with clinical KOA staging.Methods 50 cases of KOA patients underwent arthroscopic knee and knee arthroplasty as the objects of study, and 30 patients with simple meniscus injury patients as control group,using enzyme linked immunosorbent assay (ELISA) method measuring the content of IL-1,TNF-α and IL-6 in synovial fluid.Results Compared with the control group,the content of IL-1,TNF-α and IL-6 in synovial fluid increased significantly in KOA group,and the data have significant differences(P<0.05).IL-1 and TNF-α level increased with clinical stage,and the order were: advanced stage>middle stage>early stage,which has statistically significant difference(P<0.05);the level of IL-6 decreased with clinical stage,the order was:early stage>middle stage>advanced stage,which has statistically significant difference(P<0.05).IL-1,TNF-α was positively correlated with clinical stage in KOA group(r=0.567, 0.583,P<0.05),while IL-6 was negatively correlated (r=-0.486,P<0.05).Conclusion In synovial fluid,IL-1,TNF-α and IL-6 exit high expression in KOA patients.The level of IL-1、TNF-α gradually increased with the clinical stage,while IL - 6 gradually reduced.IL-1、TNF-α has close relationship with osteoarthritis pathological changes,the content of IL-6 in joint fluid of KOA patients cannot directly reflect the severity of osteoarthritis, but the level of IL-6 in section and liquid has certain reference value for early diagnosis of lesions.
