手术松解结合单臂铰链式外固定架治疗创伤后肘关节僵硬的临床效果

摘    要:目的 对比分析传统方法单纯松解术与手术松解联合单臂铰链式外固定架治疗对创伤后肘关节僵硬的临床疗效,为临床治疗提供理论依据。 方法 连续选择2013年2月—2014年2月收入绍兴市人民医院诊断为创伤后肘关节僵硬患者共82例,该研究通过医院伦理委员会批准及获得患者、家属的知情同意权,根据入院先后顺序,采用随机数字法将其分为对照组(n=40例)和观察组(n=42例),其中对照组患者采用单纯松解术,观察组采用松解术联合单臂铰链式外固定架治疗。对比2组患者治疗前后的肘关节功能评分(Mayo)、肘关节优良比例及肘关节活动度的差异性。 结果 2组患者均获8~13个月的随访,平均(11.2±2.1)个月。2组患者术后的Mayo评分、关节优良率及活动度均比术前增加,且观察组的上述指标比对照组增加更明显,差异均有统计学意义,Mayo评分:(81.9±5.6)分比(69.3±4.2)分,t=2.615,P=0.017;关节优良率:92.8%比87.5%,χ2=2.678,P=0.035;活动度:(57.8±4.9)°比(42.2±3.6)°,t=3.812,P<0.001。 结论 采用松解术联合单臂铰链式治疗肘关节僵硬的临床效果可能优于单纯松解术,值得临床推广。

Effect of surgical release combined with unilateral hinged external fixation on posttraumatic elbow stiffness

Abstract:Objective To evaluate the clinical effect of surgical release combined with unilateral hinged external fixation in treatment of posttraumatic elbow stiffness,and provide the basis for the clinical treatment. Methods A total of 82 consecutive cases suitable for the experiment were randomly divided into control group(n=40) and experiment group(n=42);the patients in the control group received releasing combined with common plaster fixation,while the patients in the experiment group received releasing combined with unilateral hinged external fixation;the clinical effects were compared between the two groups. Results The mean follow-up time was (11.2±2.1)months.the Mayo score,the excellent rate of joint and activity in the two groups were all better than before and the patients in the experiment group were all with more improvements as compared with the control group(P<0.05). Conclusion The surgical release combined with unilateral hinged external fixation in the treatment of posttraumatic elbow stiffness could be better than common plaster fixation.
