摘    要:目的:探讨应用Amplatzer蘑菇伞器治疗小儿动脉导管未闭(PDA)的临床疗效及价值。方法:本组共68例,年龄8个月-12岁(平均6.4岁),体重6.5-30(平均18.2)kg,PDA直径为3-8mm,术中行右心导管检查取血,测压,术后5-15min行隆主动脉造影,测肺动脉压,主动脉压。术后1月,3月,6月分别行X线及超声心动图检查,了解有无残余分流及封堵器位置。结果:本组68例均应用AmplatzerPDA蘑菇伞器治疗,成功率100%,疗效满意,无并发症,术后3d行超声心动图检查显示无1例分流。结论:应用Amplatzer蘑菇伞器治疗PDA创伤小,操作简单,使用广泛,成功率高,临床疗效满意。

Abstract:AIM To assess the safety and efficacy of the Amplatzer ductal occluder in the treatment of patent ductus arteriosus(PDA) in children. METHODS: A total of 68 patients underwent transcatheter occlusion of PDA at a median age of 6.4 years (range 8 month~12 years) and at a median weight 18.2 kg (range 6.5~30 kg). The diameters of ductus arteriosus were 3~8 mm. Aorta and pulmonary pressures were measured during and post implantation, and descending aorta imaging was performed 5~15 min post implantation. X ray and 2D echocardiogram examinations were performed 1,3 and 6 months after implantations to reveal the shunt and devices position. RESULTS: Complete closure was achieved in all patients (100% closure rate) and no complications were observed. No residual shunt was found 1 month after implantation by 2D echocardiogram. CONCLUSION:Transcatheter occlusion of PDA using Amplatzer ductal occluder is safe,effective and easy to perform. It is a satisfactory alternative method in the treatment of PDA in children.
