| 摘 要： | 目的 了解浙江省宁波市社区慢性阻塞性肺疾病(COPD)患者的直接经济负担情况,为制定减轻宁波地区经济负担的卫生政策提供理论依据. 方法 整群随机抽取宁波市24个县(市、区)中的2个城市和1个城乡结合部的慢性病防控示范区信息系统及医院电子病历中诊断为COPD,沟通无障碍且符合本次调查的入组和排除标准的患者作为调查对象.采用逐户走访填写问卷并结合查阅病案的方法对803名调查对象进行走访,调查内容涉及患者的一般人口学特征,疾病相关内容及直接经济负担相关费用. 结果 宁波市社区COPD患者的人均年直接经济负担费用为4243元,男女之间的差异存在统计学意义(t=2.259,P=0.024),各年龄组之间的差异无统计学意义 (F=2.822,P=0.06).影响COPD直接经济负担的因素有年龄、性别及婚姻状况. 结论 宁波地区社区COPD患者的直接经济负担是巨大的,国家应及时制定有效的卫生政策,对影响COPD直接经济负担的因素进行有效的干预和调节.

| Abstract: | Objective To understand the direct economic burden of patients with chronic obstructive pulmonary disease(COPD) in communities in Ningbo, Zhejiang province. Methods A total of 803 COPD patients in 24 communities in Ningbo were selected through random clustering sampling as study subjects, and a household questionnaire survey was conducted among them to collect the information about their demographic characteristics, COPD suffering and direct economic burden caused by COPD in the past years. Results The median of annual direct economic burden was 4243 yuan RMB for the COPD patients. The difference in direct economic burden between males and females was statistical significant(t=2.259,P=0.024), but differences was not statistical significant among different age groups(F=2.822,P=0.06). The main influencing factors included age, sex and marital status. Conclusion The direct economic burden of community COPD patients was heavy in Ningbo. It is necessary to develop appropriate health police and conduct effective intervention to ruduce the economic burden suffered by COPD patient. |
