Abstract:Objective To investigate the association between N-terminal osteocalcin (N-MID)and osteoporosis in elderly male patients with type 2 diabetes mellitus (T2DM). Methods The clinical data of 100 male T2DM outpatientsor inpatients ≥60 years old who weretreated in Dezhou People's Hospital from January 2015 to December 2019 wereretrospectively analyzed. They were divided into a non-osteoporotic group (62cases) and an osteoporotic group (38 cases) according to their bone mineraldensities (BMD). The BMD was measured in all the patients, and the serum levelsof leptin, 25(OH)D3, bone alkaline phosphatase (BAP), procollagen type I carboxy-terminal peptide (PICP), pyridinoline cross linked carboxyterminal telopeptideof type I collagen (ICTP), β-CTX, and N-MID were also measured. The correlationanalysis between N-MID and other indicators was conducted. The risk factors forosteoporosis were analyzed by logistic regression analysis, and receiveroperating characteristics curve (ROC) were drawn to determine the diagnosticvalue of N-MID for osteoporosis. The measurement data were compared between thetwo groups by the independent-sample t test. Results The thoracic BMD,lumbar BMD, hip BMD, leptin, 25(OH)D3, BAP, and N-MID in the osteoporotic groupwere lower than those in the non-osteoporotic group (all P<0.05). The PICP, ICTP, β-CTX, and HbA1c in the osteoporoticgroup were higher than those in the non-osteoporosis group (all P<0.05). N-MID was positively correlatedwith thoracic BMD, lumbar BMD, hip BMD, leptin, 25(OH)D3, and BAP (r=0.374, 0.501, 0.428, 0.672, 0.691, and0.807; all P<0.05), and negativelywith PICP, ICTP, β-CTX, and HbA1c (r=-0.419,-0.293, -0.313, and -0.405; all P<0.05).Binary logistic regression analysis showed that N-MID was an independent riskassessment factor for the osteoporosis patients (OR=2.85, P<0.05). Thearea under the ROC for the diagnosis of osteoporosis by serum N-MID was 0.851(95%CI 0.724-0.943), so it hadcertain diagnostic value for osteoporosis (Z=4.073, P<0.001). Taking the N-MIDconcentration of 12.26 μg/ml as the diagnostic cut-off value, the sensitivityand specificity of N-MID in the diagnosis of osteoporosis were 83.93% and72.61%, respectively. Conclusion The serum N-MID levelin elderly male patients with T2DM and osteoporosis decreases, and is anindependent risk factor for osteoporosis, so it may serve as a diagnosticindicator for T2DM combined with osteoporosis in elderly men.
