乳腺癌组织中IGF1R、ER表达与新辅助化疗疗效的关系

摘    要:目的:探讨乳腺癌组织中胰岛素样生长因子受体1(insulin-like growth factor receptor 1,IGF1R)、雌激素受体(estrogen receptor,ER)蛋白的表达对患者新辅助化疗疗效的影响。方法:对我院接受新辅助化疗的114例乳腺癌患者(2015年1月至2015年12月)进行回顾性分析,所有患者于化疗前均接受免疫组织化学检测,根据新辅助化疗结果将患者分为完全缓解(pCR)组25例、未完全缓解(nCR)组89例,采用非条件Logistic回归模型探讨IGF1R、ER表达与新辅助化疗疗效的关系。结果:IGF1R阳性表达患者的pCR率为19.23%,阴性表达患者的pCR率为24.19%,差异无统计学意义(P>0.05);ER阳性表达患者的pCR率为14.08%,低于阴性表达患者的34.88%,差异具有统计学意义(P<0.05);发生淋巴结转移、ER阳性表达患者会降低新辅助化疗的疗效(P<0.05),分化程度越高,患者的新辅助化疗疗效越好。结论:ER阳性表达患者新辅助化疗疗效较差,IGF1R表达程度与患者新辅助化疗疗效无关。

Abstract:Objective:To investigate the effect of insulin-like growth factor receptor 1 (IGF1R) and estrogen receptor (ER) protein expression in breast cancer tissues on neoadjuvant chemotherapy in patients with breast cancer.Methods:114 cases of breast cancer patients in our hospital receiving neoadjuvant chemotherapy (January 2015 to December 2015) were retrospectively analyzed.All patients before chemotherapy underwent immunohistochemical detection,according to the results of neoadjuvant chemotherapy were divided into complete remission (pCR group with 25 cases) and incomplete remission (nCR group with 89 cases).By non-conditional Logistic regression model to explore the relationship between expression of ER,IGF1R and the effect of neoadjuvant chemotherapy.Results:The pCR rate in positive expression of IGF1R was 19.23%,lower than that of the 24.19% in negative patients,but the difference was not statistically significant (P>0.05).The pCR rate in positive expression of ER was 14.08%,lower than that of the 34.88% in negative patients,the difference was statistically significant (P<0.05).Lymph node metastasis,the positive expression of ER patients decreased the efficacy of neoadjuvant chemotherapy (P<0.05),the higher the degree of differentiation,the better the effect of neoadjuvant chemotherapy.Conclusion:The positive expression of ER was disadvantageous to the neoadjuvant chemotherapy,and the expression of IGF1R was not related to the effect of neoadjuvant chemotherapy.
