家庭无创正压通气在重度慢性阻塞性肺疾病稳定期的应用

摘    要:目的探讨重度慢性阻塞性肺疾病(COPD)稳定期实施家庭无创正压通气(NPPV)的疗效。方法将30例经住院治疗处于稳定期的重度COPD患者分为观察组16例和对照组14例,观察组给予家庭NPPV治疗,对照组给予长期家庭氧疗(LTOT)。分别观察治疗前和2 a后的肺功能、动脉血气分析、6 min行走距离(6MWD)、西雅图阻塞性肺病调查表(SOLQ)评分、病死率及每年住院次数等指标。结果2 a后观察组死亡2例(12.5%),对照组死亡6例(42.9%),观察组病死率明显低于对照组,差别具有统计学意义(P<0.05);2 a后观察组动脉血二氧化碳分压(PaCO2)下降,每年住院次数明显减少,动脉血氧分压(PaO2)、6MWD、SOLQ评分均增加;而对照组PaCO2、每年住院次数增加,PaO2、6MWD、SOLQ评分均有减少,2组比较差别均有统计学意义(P<0.01);2组肺功能第1秒用力呼气容积占预计值百分比(FEV1%)、第1秒用力呼气容积占用力肺活量百分比(FEV1/FVC%)均有下降,但观察组较对照组下降缓慢,2组比较差别有统计学意义(P<0.05)。结论重度COPD稳定期患者长期实施家庭NPPV是积极有效的治疗措施。

Abstract:Objective To assess the effects of home noninvasive positive pressure ventilation(HNPPV) on patients with chronic obstructive pulmonary disease(COPD) during stable phase. Methods Thirty patients with COPD during stable phase were divided into two groups. Sixteen patients in observation group were treated with HNPPV, long-term domiciliary oxygen therapy was used in control group ( 14 cases ). Puhnonary function, arterial blood gas analysis ; 6rain walk distance ( 6MWD ), Seattle obstructive lung disease questionnaire ( SOLQ), case fatality and the time of hospitalization per year were observed. Results Two years later, two cases (12.5%) died in observation group, 6 cases (42.9%) died in control group, the case fatality was lower in observation group than that in control group ( P 〈 0.05 ). In observation group, the partial pressure of carbon dioxide in artery( PaCO2 ) decreased ,and the time of hospitalization lowed obviously,but the partial pressure of oxygen in artery (PaO2 ) ,6MWD, scores of SOLQ respectively increased. There was a statistically significant difference between two groups( P 〈 0. 01 ). Both FEV1% and FEV1/FVC% decreased in two groups, but observation group was slowly. There was a statistically significant difference between two groups ( P 〈 0.05 ). Conclusion HNPPV is an active and effective measure on patients with COPD during stable phase.
