COVID-19无症状感染者的转归及临床特征分析

摘    要:目的 分析某市无症状感染者的转归及临床特征。方法 以2020年1月24日至4月13日,在本市集中隔离点核酸筛查呈阳性的无症状感染者为研究对象,分析其转归及临床特征。结果 全市集中隔离境外回国人员、国内疫区返回人员及密切接触者共2816人,经核酸筛查呈阳性的无症状感染者21例,阳性检出率为7.5‰。在21例无症状感染病例中,52.4%(11/21)为持续无症状感染者,47.6%(10/21)转化为确诊病例,转化的中位数时间为2.5天。确诊病例中,90%的患者肺部CT影像有改变,50%的患者出现临床症状,两者改变的中位数时间分别为2.0天和6.0天,肺部影像改变早于临床症状出现的时间。21例病例末次暴露至确诊的平均时间为11.6天,核酸转阴的平均时间为14.1天,其中持续无症状组比确诊病例组早4.1天。全部病例平均住院时间为20.5天,实际无症状组比确诊病例组减少4.1天。全部病例白细胞、中性粒细胞和淋巴细胞的变化均在正常范围。结论 无症状感染者转化为确诊病例的比例较高。尽早临床症状识别和肺部CT检查,动态筛查出确诊病例,实施分层隔离治疗措施,是预防疫情蔓延的关键。

Abstract:To analyze the outcome and clinical characteristics of asymptomatic infect in a city. Methods From January 24 to April 13, 2020, asymptomatic infected persons who tested positive for nucleic acid screening in the concentrated isolation points in this city were selected as the study subjects, and their outcomes and clinical characteristics were analyzed. Results A total of 2 816 person were concentrated isolated, including oversea returnees, return from domestic epidemic areas and close contacts in the whole city. Twenty-one asymptomatic infected persons were tested positive by nucleic acid screening, and the positive detection rate was 7.5‰. Of the 21 asymptomatic infections, 52.4%(11/21) were persistently asymptomatic, and 47.6%(10/21) were converted to confirmed cases, with a median conversion time of 2.5 days. In the confirmed cases, 90% of the patients had changes in lung CT images, and 50% of the patients had clinical symptoms. The median time of the changes was 2.0 and 6.0 days, respectively, and the changes in lung images were earlier than the onset of clinical symptoms. The average time from the last exposure to the diagnosis of 21 cases was 11.6 days, and the average time of nucleic acid negative conversion was 14.1 days, among which the duration of asymptomatic group was 4.1 days earlier than the confirmed case group. The average length of hospital stay for all cases was 20.5 days, which was actually 4.1 days less in the asymptomatic group than in the confirmed case group. The changes of leukocytes, neutrophils and lymphocytes in all cases were in the normal range. Conclusion The ratio of asymptomatic infected persons to confirmed cases is high. Early recognition of clinical symptoms and pulmonary CT examination, dynamic screening of confirmed cases and stratified isolation treatment are the keys to prevent the spread of the epidemic.
