多层螺旋CT胰腺灌注参数与急性胰腺炎严重指数相关性的初步研究

摘    要:目的:探讨急性胰腺炎(AP)的CT灌注值与AP CT严重度指数(CTSI)和临床严重程度的关系。方法:应用16层螺旋CT对79例患者进行胰腺CT灌注成像,胰腺正常组(对照组)16例,AP组63例。使用ADW 4.2工作站perfusion 3软件包进行图像处理,测得血流量(BF)、血容量(BV)、平均通过时间(MTT)和表面通透性(PS)。分析AP各灌注值与对照组的差别以及与AP的CTSI和临床APACHEⅡ评分相关性。结果:AP和对照组各灌注值的差异有统计学意义(P<0.01),BF和BV与AP的CTSI和APACHEⅡ评分呈负相关(r=-0.865,r=-0.703;r=-0.817,r=-0.703;P<0.01),MTT和PS与CTSI和APACHEⅡ评分无相关性(r=-0.185,r=0.094;r=-0.192,r=0.141;P>0.05)。结论:AP的BF和BV较对照组显著减少,BF和BV与AP的CTSI和APACHEⅡ评分呈明显相关性,可作为判定AP严重程度的可靠指标。

Abstract:Objective: To study the correlation of blood perfusion values with CT severe index (CTSD and clinical severity of acute pancreatitis (AP). Methods: Seventy-nine patients and in which 63 cases bad AP and 16 normal cases as control were enrolled in this study and all of them had undergone MSCT perfusion imaging. The mean values of blood flow(BF), blood volume(BV), mean transit time(MTT) and permeability surface(PS) were measured on ADW 4.2 workstation with perfusion 3 software package. Using statistical software was to emphatically analyze the difference between the AP and control group and the correlation of perfusiou values with CTSI and APACHE Ⅱ respectively. Results: There was a significant difference between the AP and control group and a negative correlation of MSCT perfusion values of BF and BV with the CTSI and APACHE Ⅱ scores(r=-0.865, r=-0.703; r=-0.817, r=-0.703; P〈0.01), but MTT and PS values did not have correlation with CTSI and APACHE Ⅱ scores(r=-0.185, r=0.094; r=-0.192, r=0.141; P〉0.05). Conclusion: BF and BV are markedly decreased in AP. BF and BV have a significant correlation with the CTSI and clinical severity of AP and can be the reliable indexes in assessing the severity of AP.
