超声造影对胆囊腺肌症的诊断价值

摘    要:目的探讨超声造影对胆囊腺肌症的诊断价值。 方法选取在深圳市南山区西丽人民医院行常规超声及超声造影检查并经术后病理诊断确诊为胆囊腺肌症的患者23例,观察及分析病变的常规超声及超声造影图像特征,并对比常规超声及超声造影对胆囊腺肌症诊断的准确性。 结果胆囊腺肌症的常规超声典型表现为胆囊壁局部呈扁平状增厚,内可见散在微小或无回声,部分伴有强回声及其后方"彗星尾"征;以此特征为诊断标准,诊断准确率为69.6%(16/23)。超声造影典型表现为动脉相及静脉相均呈不均匀增强,内见散在细小无增强区;以此特征为诊断标准,诊断准确率为95.7%(22/23)。超声造影诊断准确性明显高于常规超声(χ2=5.447,P<0.05)。 结论胆囊腺肌症在常规超声及超声造影具有特征性表现,超声造影的诊断效能明显优于常规超声。

Abstract:ObjectiveTo explore the value of contrast enhanced ultrasound in the diagnosis of gallbladder adenomyomatosis. MethodsTwenty-three patients with pathologic diagnosis of gallbladder adenomyomatosis were included in this study. The features of conventional ultrasound and contrast enhanced ultrasound were observed and the accuracy rates of conventional ultrasound and contrast enhanced ultrasound were compared. ResultsThe typical conventional ultrasound features for gallbladder adenomyomatosis included the localized thickness of gallbladder, intramural small anechoic area, and intralumenal echogenic foci with comet tail. The accuracy for conventional ultrasound was 69.6% (16/23). The typical contrast enhanced ultrasound features were the heterogeneous enhancement with scattered small non-enhanced areas on both arterial phase and venous phase. The accuracy of contrast enhanced ultrasound was 95.7% (22/23), which was significantly higher than that of conventional ultrasound (χ2=5.447, P<0.05). ConclusionsBoth conventional ultrasound and contrast enhanced ultrasound had typical features for gallbladder adenomyomatosis. The diagnostic efficiency of contrast enhanced ultrasound was significantly better than that of conventional ultrasound.
