ARFI联合BI-RADS分类对伴钙化乳腺肿块的临床应用价值

摘    要:目的:探讨声辐射力脉冲弹性成像(ARFI)各指标及其联合常规超声乳腺影像报告和数据系统(BI-RADS)分类对伴钙化的乳腺肿块的临床应用价值。方法:选取收治我院的70位患者共72个伴钙化的乳腺肿块为研究对象,分别进行常规超声及ARFI检查,用声触诊组织成像(VTI)记录VTI图像与常规超声图像上病灶的面积比(AR)和VTI图像评分,用声触诊组织量化(VTQ)记录病灶内部剪切波速度(SWVi),病灶周边剪切波速度(SWVb),对所有参数绘制ROC曲线。结果:AR、VTI评分、SWVi和SWVb四种ARFI指标在伴钙化乳腺肿块良恶性之间差异有显著意义(均P<0.05)。SWVi、AR和VTI评分在对伴钙化的乳腺肿块诊断价值上没有明显差异(均P>0.05),但均高于SWVb,诊断价值有显著差异(均P<0.05)。SWVi、AR、VTI评分三种ARFI指标联合常规超声BI-RADS分类与单独超声BI-RADS分类诊断相比,诊断价值均有显著差异(均P<0.05),但三种联合诊断之间差异不显著(均P>0.05),对于BI-RADS 4a类肿块,SWVi和AR能够避免94.7%~100%良性病变的活检。结论:ARFI技术及其联合BI-RADS分类对伴钙化的乳腺肿块有较高的诊断价值,且能减少不必要的活检。

Abstract:Objective:To investigate the clinical application value of acoustic radiation pulse elastography (ARFI) and its combined conventional ultrasound breast image report and data system (BI-RADS) classification for breast masses with calcification.Methods:Seventy patients with 72 breast masses with calcification in our hospital were selected for the study.Routine ultrasonography and ARFI were performed.The area ratio (AR) of the lesions in the virtual touch tissue imaging (VTI) and conventional ultrasound images was recorded by VTI,and the VTI images were scored,the internal shear wave velocity (SWVi) of the lesion,the shear wave velocity (SWVb) around the lesion were recorded by acoustic palpation tissue quantification (VTQ),and the ROC curve was plotted for all parameters.Results:There were significant differences in AR,VTI score,SWVi and SWVb between benign and malignant breast masses with calcification (all P<0.05).There was no significant difference in the diagnostic value of SWVi,AR and VTI scores in breast masses with calcification (all P>0.05),but they were higher than SWVb,and the diagnostic value was significantly different (all P<0.05).The diagnostic value of SWVi,AR and VTI scores combined with conventional ultrasound BI-RADS classification was significantly different from that of single ultrasound BI-RADS classification (all P<0.05),but there was no significant difference among the three combined diagnoses (all P>0.05).For BI-RADS 4a masses,SWVi,AR and VTI score were able to avoid biopsies from 94.7% to 100% benign lesions.Conclusion:ARFI technology and its combined BI-RADS classification have a high diagnostic value for breast masses with calcification and can reduce unnecessary biopsy.
