| 摘 要： | 目的 探讨Friedman分型与Z型腭咽成形术(ZPPP)联合颏舌肌前移舌骨悬吊一期手术(GAHM)治疗重度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)手术疗效的相关性。方法 选择存在腭咽及舌咽平面阻塞的重度OSAHS(AHI＞40)患者26例，术前所有患者根据Friedman分型分为Friedman Ⅱ型和Friedman Ⅲ型，同期行ZPPP联合GAHM手术，术后至少6个月复查。采用卡方检验分析手术前后各相关参数的变化。结果 FriedmanⅡ型患者的手术成功率为100%，而Friedman Ⅲ型患者的手术成功率仅22.2%，差异有统计学意义(P<0.01)。结论 通过Friedman分型，Friedman Ⅱ型患者可以取得良好的手术疗效。

| Abstract: | Objective To explore the relationship between Friedman OSA staging and genioglossus advancement and hyoid suspension(GAHM) plus Z-palatopharyngoplasty(ZPPP). Methods 26 patients with severe OSAHS (AHI>40)were performed GAHM plus ZPPP. The obstructive sites were identified in both oropharynx and hypopharynx. The follow up lasted at least 6 months postoperatively. The Chi square test was used to compare the preoperative and postoperative results. Results Satistying clinic results for the patients with Friedman OSA stages Ⅱ were 100%, while the overall success rate of Friedman OSA stages Ⅲ was 22.2%. Conclusion ZPPP plus GAHM is an effective method for patients with Friedman OSA stages Ⅱ. |
