摘    要:摘要:目的 掌握医院感染流行病学特征,分析其原因,为预防控制医院感染,制定相关防控策略提供依据。方法 查阅2011-2013年度大连市某三甲院所有出院病人的病历,填写摘录表,对医院感染率进行回顾性分析。结果 共70 169例患者,696例发生医院感染,总体感染率为0.99%。男、女感染率分别为0.73%和1.28%,差异有统计学意义(χ2=53.42,P<0.01)。60岁以上年龄组感染率最高,为1.19%,14岁及以下年龄组感染率最低,为0.30%,差异有统计学意义(χ2=34.89,P<0.01)。四个季度感染率分别为0.86%、0.77%、1.06%和1.26%,差异有统计学意义(χ2=25.72,P<0.01)。各科室中感染率居前三位的是ICU,肿瘤科、内科,感染率分别为27.19%、1.42%和1.15%。感染部位以上呼吸道、下呼吸道和泌尿系统为主,感染率分别为36.78%、31.61%和13.80%。结论 加强重点科室和关键环节的监测,严格规范无菌操作。

Abstract:Abstract: Objective To grasp the epidemiologic feature and explore the underlying reasons for hospital infection. To provide a basis for hospital managers to prevent and control hospital infections effectively, and improve the quality of medical care and ensure medical safety. Methods We retrospectively reviewed and analyzed the medical records of the patients who were discharged from the hospital during 2011 to 2013, and also calculated the nosocomial infection rates and filled the excerpt tables. Results 70169 cases were collected, 696 belong to infection in hospital, the prevalence rate was 0.99%.Hospital infection rate of male patients was 0.73% and hospital infection rate of female patients was 1.28%, and the difference was statistically significant (χ2=53.42, P<0.01). The age group of 60~ has the highest infection rate of 1.19% in all age groups, and the lowest rate was the age group of ≤14 with the rate of 0.30%. The difference was statistically significant (χ2=34.89, P<0.01). From the first quarter to the forth quarter, the prevalence rates were 0.86%, 0.77%, 1.06% and 1.26%, respectively, and the difference was statistically significant (χ2=25.72, P<0.01). The top three infected rates of all departments were in ICU (27.19%), oncology department (1.42%) and medical department (1.15%). Nosocomial infections occurred mainly in the upper airway (36.78%), lower respiratory tract (31.61%) and urinary system (13.80%). Conclusion We need to focus on the monitoring of the departments at high risk of infections and the key links, and carry out the aseptic technology strictly.
