金黄色葡萄球菌耐药性及毒素分析

摘    要:摘要:目的 分析100株金黄色葡萄球菌的耐药谱、耐药基因、毒力基因分布及其相关性。方法 收集2014年10-12月江苏大学附属人民医院临床分离的100株金黄色葡萄球菌,采用VITEK 2 compact 进行细菌鉴定及药敏试验,聚合酶链反应技术(PCR)检测常见的9种耐药基因及10种毒力基因。结果 共检出耐甲氧西林金黄色葡萄球菌(MRSA)52株,检出率52%,未检出替加环素、万古霉素、利奈唑烷及喹努普汀/达福普汀耐药株,对庆大霉素、左氧氟沙星、红霉素、利福平、环丙沙星、莫西沙星、克林霉素、四环素和复方新诺明的耐药率分别在12%~64%之间。实验共检出7种耐药基因和7种毒力基因,其中最常见的耐药基因组合为aacA-aphD+ ermC+ tetM,另外,检出的耐药基因与其表型并不完全一致。毒力基因hla、RNAⅢ及psm α在所有的菌株中均表达,而eta、etb和edin C在所有菌株中均未表达。pvl、seb、sec和tst-1毒力基因阳性率分别为7%、47%、64%和4%。其中MRSA与甲氧西林敏感金黄色葡萄球菌(MSSA)相比,MRSA多重药物的耐药率明显增高,且部分毒素基因分布存在差异。结论 临床标本分离的金黄色葡萄球菌存在多重耐药性,且部分菌株毒力较强,需加强监控。

Drug resistance and virulence genes of Staphylococcus aureus

Abstract:Abstract: Objective To investigate the distribution and association of drug resistance, drug resistance genes and virulence genes of 100 Staphylococcus aureus (S.aureus) isolates. Methods 100 strains of S.aureus were collected from the patients hospitalized in the Affiliated People's Hospital of Jiangsu University during Oct 2014 to Dec 2014. Then, the identification and antimicrobial susceptibility testing of the 100 S.aureus isolates were determined by VITEK 2 Compact automated microbial System. The 9 kinds of drug resistance genes and 10 kinds of virulence genes were detected by polymerase-chain-reaction (PCR). Results 52 isolates of Methicillin-resistant Staphylococcus aureus (MRSA) have been detected and the detection rate was 52%. The resistance rates of gentamicin, levofloxacin, erythromycin, rifampicin, ciprofloxacin, moxifloxacin, clindamycin, tetracycline and sulfamethoxazole/trimethoprim were between 12% and 64%. None of the isolates displayed resistance to tigecycline, vancomycin, linezolid and quinupristin/dalfoprsifin. Totally 7 kinds of drug resistance genes and 7 kinds of virulence genes have been detected. aacA-aphD+ ermC+ tetM were the most common resistance genes. The resistance genes were somewhat inconsistent with phenotype. All strains were positive for hla、RNAⅢ and psm α with negative to eta、etb、edin C genes. The positive rates of pvl、seb、sec、tst-1 were 7%, 47%, 64%, 4%, respectively. Compared to methicillin-sensitive Staphylococcus aureus (MSSA), MRSA strains had high rates of drug resistant and the profile of virulence genes was different between them. Conclusion The S. aureus isolated from clinical specimens with multiple drug resistances and virulence potential are epidemic in the hospital. More surveries and measures should be taken to prevent the infection.
