2013年某三甲医院血培养污染菌的特征分析

摘    要:摘要:目的 回顾某院2013年度血培养标本的实验室数据,了解该院血培养污染率及污染菌种类。方法 收集该院2013年1-12月共38879份血培养标本的微生物培养结果,并回顾分析临床病例资料,分离出397株污染菌并进行分析。结果 38879份血培养标本中,阳性标本2819份,阳性率为7.25%(2819/38879);分离出污染菌的标本397份,污染率为1.02%(397/38879);导管血、骨髓和静脉血标本的污染率分别为2.34%(23/981)、3.63%(21/578)和0.95%(353/37320);各科室污染率介于0%~2.52%。污染菌以凝固酶阴性的葡萄球菌(CNS)为主(303/397,76.32%),其次为G+杆菌和链球菌属。结论 2013年度该院血培养污染率为1.02%,污染菌的种类主要是CNS;当血培养结果疑为污染菌时,应结合实验室检查结果与临床症状综合判断是否为血流感染。

Abstract:Abstract: Objective The study retrospectively analyzed the laboratory data of blood culture specimens at a hospital in 2013, and assessed the contamination rate and the strains for bacterial contamination. Methods Results of bacterial culture of a total of 38879 blood culture specimens collected at the hospital in 2013 were obtained, and were used to retrospectively analyze the clinical cases. 137 strains of contaminating bacteria were isolated and analyzed. Results Among 38879 blood culture specimens, 2819 were tested positive with a positive rate of 7.25% (2819/38879), and 397 were isolated contaminating bacteria with a contamination rate of 1.02% (397/38879). The contamination rates of catheter-contacted blood, bone marrow, and venous blood were 2.34% (23/981), 3.63% (21/578), and 0.95% (353/37320), respectively; and the contamination rates at different departments ranged from 0% to 2.52%. The main bacteria for contaminating bacteria was coagulase-negative Staphylococcus (CNS) (303/397, 76.32%), followed by gram-positive Bacillus and Streptococcus. Conclusion The contamination rate of blood culture at the hospital is 1.02% in 2013, and the main contaminating bacteria is CNS. While blood culture results of patients indicate presence of contaminating bacteria, suspected bloodstream infection should be diagnosed according to both the laboratory results and clinical symptoms of the patients.
