65岁以上住院患者焦虑情绪与营养风险的关系

摘    要:目的探讨65岁以上住院患者焦虑状态与存在营养风险的现况及两者的关系。方法采用方便抽样的方法,选择2019年3-8月期间在宁夏回族自治区人民医院65岁以上住院患者为研究对象,共145例,记录入院后72 h内空腹静脉血检测的营养相关指标结果。采用营养风险筛查量表NRS2002评估患者的营养状态,以NRS2002评分≥3分的99例患者为存在营养风险组,以选择年龄和性别相匹配的NRS2002评分<3分表示无营养风险组,共55例;采用独立样本t检验和χ2检验比较存在营养风险组与无营养风险组的组间均衡性,两组均行Zung氏焦虑自评量表(SAS)评分,比较两组患者的焦虑检出率情况。采用Pearson相关性和多因素logistic回归分析焦虑与存在营养风险的关系。结果65岁以上住院患者存在营养风险和焦虑的检出率分别为62.17%和为45.5%。SAS平均评分为(49.56±11.71)分,其中不同年龄和不同居住地的患者SAS评分差异均有统计学意义(t=2.379, P=0.020)和(t=3.007, P=0.003)。存在营养风险组患者SAS评分为(55.6±10.19)...

Abstract:To explore the current state of anxiety and nutritional risk of hospitalized patients over 65 years old andtheir relationship. Methods Using a convenient sampling method, patients over 65 years old and hospitalized in the People’sHospital of Ningxia Hui Autonomous Region from March to August 2019 were selected as the study subjects. Within 72hours after admission, 145 cases of fasting venous blood nutrition related index test results were recorded. Using theNutritional Risk Screening Scale (NRS2002) to assessed nutritional status of the patients, 99 patients with a score of ≥3(NRS2002) were included in the malnutrition risk group. The age- and gender-matched (NRS2002) scores <3 indicated nonutritional risk group with 55 patients. The independent sample t test and the chi -square test were used to compare theintergroup equilibrium between the nutritional risk group and the non-nutritional risk group. The Zung’s Anxiety Self-ratingScale (SAS) score was averaged. Comparing the anxiety detection rate of the two groups of patients, using Pearson correlationand multiple factors logistic regression analysis of the relationship between anxiety and nutritional risk. Results Theincidence of nutritional risk and anxiety in hospitalized patients over 65 years old were 62.17% and 45.5%. The average SASscore was (49.56±11.71), and the SAS scores of patients with different ages and different places of residence were statisticallysignificant (t=2.379, P=0.020 and t=3.007, P=0.003). The SAS score of the patients in the nutritional risk group was (55.6±10.19), which was significantly higher than that of the non-nutrition risk group (39.68±5.87), and the difference was statisticallysignificant (t=-11.926, P<0.001). Pearson correlation analysis showed that (NRS2002) scores were negatively correlated withnutrition-related indicators (r<0, P<0.001). Multivariate logistic regression analysis showed that the risk of anxiety in hospitalizedpatients with nutritional risk increased by 34.9% compared with the non-nutrition risk group (OR =34.929, 95% CI: 9.738 -125.285). Conclusions Hospitalized patients over the age of 65 are prone to nutritional risks during treatment, which may beone of the most important factors in the development of anxiety. Therefore, early nutritional risk screening and nutritionalintervention for inpatients may help reduce the occurrence of negative emotions such as anxiety disorder
