血清锌α2糖蛋白与2型糖尿病患者空腹C肽、血尿酸水平的相关性研究

摘    要:目的 观察探讨血清锌a2糖蛋白(ZAG)与2型糖尿病(T2DM)患者空腹C肽、血尿酸(SUA)水平的相关性,为预防T2DM患者高尿酸血症(HUA)及并发症的发生提供理论依据。方法 选取2019年5月至2021年5月在武汉科技大学附属孝感医院内分泌科住院的2型糖尿病患者110例为研究对象,根据是否合并HUA分为两组,观察组:T2DM并HUA组54例,年龄(55.2±6.68)岁,男28例,女26例;对照组:T2DM尿酸正常组56例,年龄(55.8±5.80)岁,男28例,女28例。均测定空腹血清ZAG、C肽、血糖、血脂、糖化血红蛋白及其他相关临床指标。数据采用SPSS 17.0软件,组间比较采用独立样本t检验、χ2检验和非参数检验,ZAG与临床其他指标分别进行偏相关分析和多元性逐步回归分析。结果 观察组患者空腹血清ZAG水平明显高于对照组[(60.01±24.56)mg/L比(44.35±17.20)mg/L,P<0.05]。T2DM患者空腹血清ZAG水平分别与SUA(r=0.543)、体质量指数(r=0.240)、C肽(r=0.437)、三酰甘油(r=0.372)呈正相关,与高密度脂蛋白胆固醇呈负相关(r=-0.447),差异均有统计学意义(均P<0.05)。结论 T2DM并HUA患者较T2DM尿酸正常者血清ZAG表达水平升高;ZAG可能诱导T2DM患者糖脂代谢紊乱,参与胰岛素抵抗过程,增加SUA表达,可能成为预防及治疗HUA及其相关代谢性疾病的新靶点。

Abstract:Objective:To investigate the correlation of zinc-a2-glycoprotein with C-peptide and serum uric acid (SUA) in patients type 2 diabetes mellitus (T2DM), and to provide theoretical references in the prevention of hyperuricemia and its complications in patients with T2DM.Methods:From May 2019 to May 2021, 110 patients with T2DM treated at Department of Endocrinology, Affiliated Xiaogan Hospital of Wuhan University of Science and Technology were chosen, and were divided into an observation group (54 cases,including 28 males and 26 females), the ones complicated with hyperuricemia who were (55.2±6.68) years old, and a control group (56 cases, including 28 males and 28 female), the ones not complicated with hyperuricemia who were (55.8±5.80) years old, cases, according to whether they were complicated with hyperuricemia. The fasting serum levels of ZAG, C-peptide, and plasma glucose (FPG), glycosylated emoglobin A1c (HbA1c), blood lipid, and related clinical indicators were detected. The data were analyzed by the software SPSS 17.0. The independent-sample t , χ2, and nonparamertric tests were used for the comparison between these two groups. Partial correlation analysis and multiple stepwise regression analysis were used to analyze the ZAG and other clinical indicators. Results:The fasting serum level of ZAG in the observation group was higher in than that in the control group [(60.01±24.56) mg/L vs. (44.35±17.20) mg/L, P<0.05]. The fasting serum level of ZAG in the patients with T2DM was positively correlated with SUA ( r=0.543), body mass index ( r=0.240), C-peptide ( r=0.437), and triacylglycerol ( r=0.372), and negatively with high density liptein cholesterol ( r=-0.447), with statistical differences (all P<0.05). Conclusions:The serum level of ZAG in the T2DM patients with hyperuricemia is higher than that in the ones with no hyperuricemia. ZAG may induce lipid metabolism disorder in T2DM patients, participate in insulin resistance, and increase SUA, so it could be a new target in the prevention and treatment of hyperuricemia and other metabolism diseases.
