脑钠肽对维持性血液透析患者心功能不全诊断价值的临床分析

摘    要:
目的:对MHD(维持性的血液透析)患者BNP(B型脑钠肽)于透析前后其血浆内的浓度进行观察,同时评价BNP诊断患者心功能不全的临床价值。方法:回顾我院2008年10月~2010年12月,我科收治的100例MHD患者临床资料,对患者透析前后其血浆的BNP水平进行检测,并应用超声心动图对患者透析前后其心脏结构与功能的变化进行测定,同时测量患者下腔静脉的宽度,以及记录其当次透析的脱水量。结果:患者透析后其血浆BNP的中位数(124.9pg/ml)较透析前(381.1pg/ml)具有显著性差异(P<0.01),并且透析后的血BNP,较LVDd与LVDs呈现正相关,较LVEF则呈现负相关,较LV与RV以及RVD和IVS不具有相关性。透析前后其血BNP水平减少比值,较下腔静脉其宽度的减少比值呈现正相关,较透析前后其体重减少的比值呈现正相关;21例LVEF(BNP 680.5pg/ml)小于50%,79例LVEF(BNP 110.9pg/ml)大于50%,对两者比较,差异显著(P<0.01);左心扩大者29例(BNP 522.1pg/ml),较左室正常者71例(BNP 104.9pg/ml)有显著性差异(P<0.01)。结论:BNP能够反映出MHD患者其体内血容量所出现的变化以及左心功能状况,是对慢性的心功能不全进行诊断较为敏感的指标。

Abstract:
Objective: Of MHD(maintenance hemodialysis) patients BNP(B-type natriuretic peptide) in plasma before and after dialysis were observed within the concentration,while BNP diagnostic evaluation in patients with heart failure clinical value.Methods: Looking back at my hospital in 2008 October ~December 2010,our department treated 100 cases of MHD patients clinical data for dialysis patients with plasma BNP levels before and after testing,and echocardiography before and after dialysis for patients with cardiac structure of its functional changes were measured at the same time measuring the width of the inferior vena cava of patients,and record the amount of dehydration when the second dialysis.Results: After dialysis patients,the median plasma BNP(124.9pg/ml) compared with pre-dialysis(381.1pg/ml) with significant difference(P<0.01),and post-dialysis blood BNP,LVDs more positively correlated with LVDd,LVEF is related to more negative than the LV and IVS with RV and RVD are not relevant.BNP levels before and after dialysis to reduce the ratio of their blood,compared with the inferior vena cava to reduce the ratio of its width is positively related to their weight loss before and after dialysis compared with the ratio of positive correlation21 cases of LVEF(BNP 680.5pg/ml) less than 50%,79 cases of LVEF(BNP 110.9pg/ml) greater than 50% of the two,the difference was significant(P<0.01);left ventricular expansion of 29 cases(BNP 522.1pg/ml),71 cases compared with normal left ventricular(BNP 104.9pg/ml) are significantly different(P<0.01).Conclusion: MHD patients BNP can reflect the body and blood volume changes arising from the left cardiac function,is chronic heart failure are more sensitive diagnostic indicators.
