糖尿病并齿龈肺部侵袭性曲霉菌细菌混合性感染病例分析

摘    要:目的探讨糖尿病并齿龈及肺部侵袭性曲霉菌、细菌混合性感染的诊断治疗。方法对1例糖尿病并齿龈及肺部侵袭性曲霉菌、细菌混合性感染患者的病因、诊断与治疗进行回顾性分析。结果本例患者经抗真菌药物全身及局部治疗,肺部病灶有明显吸收好转,齿龈病灶无吸收,28 d后体温降至正常。于治疗42 d后因突然咳嗽大咯血死亡。结论侵袭性曲霉菌主要发生在免疫受损的人群,合并糖尿病时更易加重病情和误诊,而遭致混合性感染。早期诊断治疗,控制血糖水平,加强支持疗法及重要器官的检测有利于病情控制,及时手术清除病灶是提高治愈率和减少病死率的关键。

Abstract:Objective To investigate the diagnosis and treatment of diabetes with pulmonary and dental mixed infectation with invasive Aspergillus and bacteria.Methods The etiopathogenisis,diagnosis and treatment of one patient with diabetes and pulmonary and dental mixed infections with invasive Aspergillus and bacteria was retrospectively analyzed.Results After vein and local application of antifungal drugs,the focus in pneumoniae was obvicusly absorbed,but the focus in gums of teeth had no absorption.The temperature decreased to normal 28 days later.The paitent suddenly died of large hemoptysis in the 42th day.Conclusion The people with immune function impaired often infected with invasive Aspergillus,when incorporate with diabetes mellitus,the condition often worsened and easily misdiagnosed,and can result in mixed infectation.Early diagnosis and treatment,control the level of blood sugar,strengthen of maintenance therapy and detection of important organs are beneficial to control patient,s condition,Promptly removing the focus by operation is the important turning point to increase recovery rate and reduce fatality ratio.
