喉鳞状细胞癌中人类错配修复蛋白2与多药耐药蛋白表达及相关性

摘    要:目的研究人类错配修复蛋白2(human muts homolog 2,hMSH2)在喉鳞状细胞癌(简称喉鳞癌)中的表达及与多药耐药基因蛋白P-糖蛋白(P-glycoprotein,P-gp)表达的相关性,两者表达与临床病理特征的关系。方法采用免疫组化En-ViSion法检测喉鳞癌及声带息肉组织中hMSH2和P-gp的表达情况,同时检测hMSH2阳性表达和阴性表达组中P-gp的表达情况。结果 hMSH2蛋白在喉鳞癌组织中阳性表达率为59%,明显低于声带息肉90%(χ2=12.741,P〈0.05),P-gp在喉鳞癌中阳性表达率为76%,显著高于声带息肉阳性表达率45%(χ2=13.260,P〈0.05)。hMSH2阳性的喉鳞癌中P-gp阳性率为69%,hMSH2阴性的喉鳞癌中P-gp阳性率为87%,差异有统计学意义(χ2=4.479,P〈0.05)。hMSH2表达与喉鳞癌分化程度、临床分期、淋巴结转移相关。P-gp表达与肿瘤分化程度、临床分期、淋巴结转移及T分级相关。结论 hMSH2与P-gp在喉鳞癌发生及恶性演进中起一定作用,hMSH2可能通过影响P-gp表达导致喉鳞癌多药耐药的产生。

Abstract:OBJECTIVE To investigate the expression and relationship of mismatch repair protein hMSH2 and multidrug resistance protein P-gp in laryngeal squamous cell carcinoma(LSCC),and analyze their correlation with the clinical pathologic features.METHODS Immunohistochemical En-ViSion method was used to measure the expression of hMSH2 and P-gp in 110 specimens of LSCC and 40 specimens of vocal cord polyps.RESULTS The positive expression rate of hMSH2 in LSCC(59%)was significant lower than that in vocal cord polyps(90%,χ2=12.741,P0.05).The positive rate of P-gp in LSCC and benign lesions were 76% and 45%,respectively.The difference had statistical signifcance(χ2=13.260,P0.05).The positive rate of P-gp in hMSH2 positive group was 69%,and in hMSH2 negative group was 87%,with significant difference(χ2=4.479,P 0.05).The expression of hMSH2 was related to pathological grade,clinical stage and lymph node metastasis(all P0.05).The positive expression of P-gp was correlated with pathological grade,clinical stage,lymph node metastasis and T stage(all P0.05).CONCLUSION Abnormal expression of hMSH2 and P-gp may play a role in the development and progression of LSCC.hMSH2 may induce multidrug resistance in LSCC by infuencing expression of P-gp.
