泸州市18~60岁体检人群血尿酸水平与非酒精性脂肪肝关系研究

摘    要:目的 了解四川省泸州市18~60岁体检人群血尿酸(SUA)水平与非酒精性脂肪肝(NAFLD)的关系,评价SUA对NAFLD的影响,为NAFLD的防控提供依据。方法 选取2021年1—12月在西南医科大学附属中医医院健康管理中心参与健康体检的年龄18~60岁体检者,进行面对面问卷调查、体格检查、血液生化检测及肝脏B超检查,根据诊断标准,将入选者分为NAFLD组和对照组,采用多因素logistic回归模型分析SUA水平与NAFLD患病的关系。结果最终2 684名调查对象纳入研究(男2 163名,女548名),NAFLD检出率为39.45%(1 059/2 684),其中男性检出率为46.49%(993/2 136),高于女性的12.04%(66/548),差异具有统计学意义(χ2=216.604,P<0.001);NAFLD组SUA水平为(400.89±86.17)μmol/L,高于对照组SUA水平的(339.82±84.28)μmol/L(t=-18.185,P<0.001);按SUA水平进行四分位数分组,SUA最低四分位数、第二四分位数、第三四分位数、...

Abstract:Objective To explore the association between serum uric acid (SUA) and non-alcoholic fatty liver disease (NAFLD) in physical examination population aged 18 to 60 years in Luzhou, Sichuan Province, and to evaluate the impact of uric acid on NAFLD, so as to provide evidences for the prevention and control of NAFLD. Methods We selected participants aged 18 to 60 years in the Health Management Center of the Affiliated Hospital of Traditional Chinese Medicine of Southwest Medical University and conducted face-to-face questionnaire interview, physical examination, blood biochemical test, and liver B-ultrasound examination from January through December, 2021. Statistical analyses were performed to assess the change trend of each index of different SUA levels and the association of SUA with NAFLD. According to the diagnostic criteria, the participants were divided into the NAFLD group and the control group, and multivariate logistic regression model analysis was performed to assess the association of SUA with NAFLD. Results Finally 2 684 participants were included in the study (2 163 males and 548 females). The detection rate of NAFLD was 39.45% (1 059/2 684), with higher detection rate of NAFLD in males (46.49%) (993/2 136) than in females (12.04%) (66/548), and the difference was statistically significant (χ2=216.604, P<0.001). The SUA of the NAFLD group was significantly higher than that of the control group [(400.89±86.17)μmol/L vs. (339.82±84.28) μmol/L, t=-18.185, P<0.001]. The detection rates of NAFLD were 26.18% (178/680), 36.09% (244/676), 41.58% (274/659), and 54.26% (363/669) for the participants in the lowest, second, third, and the highest quartile of SUA content, respectively. With the increase of SUA level, BMI, systolic blood pressure, diastolic blood pressure, waist circumference, triglycerides, total cholesterol, low-density lipoprotein cholesterol, aspartate aminotransferase, and alanine aminotransferase while age, high-density lipoprotein cholesterol, and fasting blood glucose decreased (P<0.001). Unconditional multivariate logistic regression analysis revealed that compared to those in the lowest quartile of SUA content, the participants in the highest quartile were at increased risk of NAFLD, with the odds ratios (95% confidence interval) of 2.431(1.841-3.211) after adjusting for gender, age, drinking, smoking, lack of exercise, dietary behavior, BMI, central obesity, and other mixed factors. Conclusion High serum uric acid increases the risk of non-alcoholic fatty liver disease in people aged 18 to 60 years old in Luzhou.
