| 摘 要： | 目的 探讨分析连续性血液净化对脓毒症患者凝血功能及免疫功能的影响。方法 选取2014年1月至2018年3月深圳市人民医院重症医学科收治的78例脓毒血症患者随机分为观察组和对照组,观察组患者给予连续性血液净化,对照组患者给予常规治疗,比较两组患者治疗前后凝血功能、外周血免疫细胞、炎性指标。结果 治疗前两组患者凝血功能、外周血免疫细胞、炎性指标差异无统计学意义(P＞0.05),治疗后两组患者PT、APTT、TT、FIB、INR、TNF-α指标均显著下降,CD3+、CD4+、CD4+/CD8+、 IL-1、IL-10指标均显著上升,且观察组患者CD3+、CD4+、CD4+/CD8+指标上升更显著,IL-1、IL-10上升程度低于对照组,PT、APTT、TT、FIB、INR、TNF-α指标降低程度高于对照组患者,差异均具有统计学意义(P＜0.05),治疗前后两组患者CD8+指标差异无统计学意义(P＞0.05)。结论 连续性血液净化治疗脓毒症患者可改善其凝血功能、免疫功能,促进疾病的恢复,值得推广。 |

| Abstract: | Objective To investigate the effect of continuous blood purification on coagulation and immune function in patients with sepsis.Methods From January 2014 to March 2018,78 patients with sepsis admitted to the Department of Critical Care Medicine were randomly divided into observation group and control group.Patients in the observation group were given continuous blood purification,and patients in the control group were given routine treatment.The coagulation function,peripheral blood immune cells and inflammatory indexes were compared before and after treatment. Results There was no significant difference in coagulation function,peripheral blood immune cells and inflammatory index between the two groups before treatment (P＞0.05).After treatment,the indexes of PT,APTT,TT,FIB,INR and TNF-α were significant in both groups.The levels of CD3+,CD4+,CD4+/CD8+,IL-1 and IL-10 all increased significantly.In the observation group,the levels of CD3+,CD4+ and CD4+/CD8+ increased more significantly.The levels of IL-1 and IL-10 increased less than the control group.The scores of PT,APTT,TT,FIB,INR and TNF-α were significantly lower in the control group than in the control group (P＜0.05).There was no significant difference in CD8+ index between the two groups before and after treatment (P＞0.05).Conclusion The continuous blood purification treatment of sepsis patients can improve their coagulation and immune function,and promote the recovery of the disease.It is recommended that it be popularized in clinical practice. |
