四川省泸州市2017年心脑血管疾病死亡特征及疾病负担分析

摘    要:目的 分析泸州市心脑血管疾病死亡特征、对期望寿命的影响及其潜在寿命损失,评估心脑血管疾病所致的疾病负担。方法 收集2017年泸州市心脑血管疾病死亡资料,计算死亡率、平均期望寿命、去死因期望寿命、潜在减寿年数、减寿率、平均减寿年等指标。结果 2017年泸州市心脑血管疾病死亡率为219.96/10万,标化率为150.58/10万,男性死亡率(231.77 /10万)高于女性(197.32/10万);0~14岁组、15~44岁组、45~64岁组、65岁以上组死亡率分别为1.02/10万、13.02/10万、133.10/10万以及1 370.20/10万。2017年泸州市居民期望寿命为77.41岁,去除心脑血管疾病后期望寿命增加4.18岁,其中男性增加3.76岁,女性增加4.61岁;去除脑血管病与心脏病后,期望寿命分别增加2.52岁与1.97岁。心脑血管疾病死亡PYLL为70 524人年,PYLLR为14.59‰,AYLL为11.77年/人,男性PYLL、PYLLR与AYLL(48 220人年、18.93‰、12.43年/人)均高于女性(22 304人年、9.56‰、10.56年/人),脑血管病导致了最多的寿命损失(37 988人年、7.86‰、11.16年/人)。结论 泸州市居民心脑血管疾病导致的疾病负担重,在男性中导致了更高的期望寿命损失与潜在寿命年损失,65岁以上老年人是心脑血管疾病所致死亡的主要人群,脑血管病的疾病负担最为严重。

Abstract:Objective To explore the death characteristics of cardio-cerebrovascular diseases, and their impact on life expectancy and potential years of life lost(PYLL), in order to assess the disease burdens. Methods Based on the data of cardio-cerebrovascular death in Luzhou in 2017, we calculated death rate, average life expectancy, cause eliminated life expectancy, PYLL, PYLLR, and AYLL. Results From the death data in cardio-cerebrovascular diseases, the crude death rate was 219.96/105, the standardized mortality was 150.58/105, and the male death rate(231.77 /105) was higher than female (197.32/105). The cardio-cerebrovascular death rates of group 0-14 years, 15-44 years, 45-64 years and 65 years above were 1.02/105, 13.02/105, 133.10/105, and 1370.20/105, respectively. The average life expectancy was 77.41 years. With the elimination of cardio-cerebrovascular cause, life expectancy could increase by 4.18 years; male and female life expectancy could increase by 3.76 years and 4.61 years. With the elimination of cerebrovascular and cardiovascular causes, life expectancy could increase by 2.52 years and 1.97 years, respectively. From the cardio-cerebrovascular data, PYLL was 70 524 person-years, PYLLR was 14.59‰, AYLL was 11.77 years per capita, and the PYLL, PYLLR and AYLL of male (48220 person-years, 18.93‰, 12.43 years per capita) were all higher than these of female (22304 person-years, 9.56‰, 10.56 years per capita). Cerebrovascular disease caused the highest loss. Conclusion Cardio-cerebrovascular disease caused heavy disease burden in Luzhou, and it caused higher life expectancy lost and PYLL in males, especially in cerebrovascular disease and the elderly aged above 65.
