摘    要:
目的了解医院多重耐药菌(MDROs)的感染情况及分布特征,为临床合理用药和医院感染防控提供依据。方法采用目标性监测方法,对某医院住院患者的MDROs感染与耐药情况进行了调查与分析。结果在2016—2018年期间共检测住院患者送检标本56 680份,监测出细菌感染患者10 228例,发生MDROs感染患者1 626例,MDROs感染构成比为15.9%。MDROs以多重耐药鲍曼不动杆菌和产超广谱β-内酰胺酶大肠埃希菌为主,构成比分别为34.69%和24.11%。MDROs主要分离自痰液、尿液和血液,构成比依次为56.64%、15.88%和8.29%。MDROs感染患者主要来自重症监护病房,占MDROs感染患者总数的46.19%。结论该医院MDROs感染率仍处于较高水平,应加强高危人群的监控及细菌耐药监测,规范抗菌药物的合理使用。

Abstract:
Objective To understand the infection status and distribution characteristics of multidrug-resistant organisms( MDROs) in a hospital,so as to provide a basis for clinical rational drug use and nosocomial infection prevention and control. Methods The MDROs infection and drug resistance of inpatients in a hospital were investigated and analyzed by targeted surveillance method. Results A total of 56 680 specimens from hospitalized patients were tested from 2016 to 2018,of which 10 228 patients with bacterial infection were monitored and 1 626 patients with MDROs infection were detected,with MDROs infection composition ratio of 15. 9%. MDROs were mainly multidrug-resistant Acinetobacter baumannii and extended spectrum β-lactamase-producing Escherichia coli,with composition ratios of 34. 69% and 24. 11%,respectively. MDROs were mainly isolated from sputum,urine and blood,with composition ratios of 56. 64%,15. 88% and8. 29%,respectively. MDROs infected patients mainly came from intensive care unit,accounting for 46. 19% of the total number of MDROs infected patients. Conclusion The infection rate of MDROs in this hospital is still at a relatively high level. The monitoring of high-risk groups and bacterial resistance should be strengthened,and the rational use of antibiotics should be standardized.
