急性缺血性脑卒中患者认知功能改变与血清ICAM-18-iso-PGF2α表达水平的相关性分析

摘    要:目的探讨急性缺血性脑卒中患者认知功能改变与血清ICAM-1、8-iso-PGF2α表达水平的相关性分析。方法选取2017年8月至2019年10月我院收治的急性缺血性脑卒中患者100例,按MoCA评分将其分为A组(60例,MoCA评分<26分)和B组(40例,MoCA评分≥26分)。采用酶联免疫吸附测定两组患者ICAM-1、8-iso-PGF2α表达水平,采用Spearman分析血清ICAM-1、8-iso-PGF2α的表达与MoCA评分的相关性,采用Logistic回归分析急性缺血性脑卒中患者认知功能改变的影响因素。ROC曲线分析血清ICAM-1、8-iso-PGF2α的表达对急性缺血性脑卒中患者认知功能的诊断价值。结果A组血清ICAM-1、8-iso-PGF2α表达均显著高于B组(P<0.05);血清ICAM-1、8-iso-PGF2α的表达水平与梗死灶位置、颈动脉粥样硬化、颈动脉狭窄情况、NIHSS评分、MoCA和年龄显著相关(P<0.05),而与受教育程度无关(P>0.05)。梗死灶位于丘脑、颞叶和额叶、NIHSS评分、糖尿病和高血压为急性缺血性脑卒中患者认知功能障碍的独立危险因素(P<0.05)。ICAM-1、8-iso-PGF2α诊断急性缺血性脑卒中患者认知功能改变的灵敏度和特异度为93.00%和90.00%(P<0.05)。结论血清ICAM-1和8-iso-PGF2α的表达与急性缺血性脑卒中患者认知功能改变密切相关。

Abstract:Objective To explore the correlation analysis between the changes of cognitive function and the expression levels of serum ICAM-1 and 8-iso-PGF2αin patients with acute ischemic stroke.Method A total of 100 patients with Acute ischemic stroke admitted to our hospital from August 2017 to October 2019 were divided into group A(60 cases,MoCA score<26 points)and group B(40 cases,MoCA)according to the MoCA score Score≥26 points).Enzyme-linked immunosorbent assay was used to determine the expression levels of ICAM-1 and 8-iso-PGF2αin the two groups.Spearman was used to analyze the correlation between serum ICAM-1 and 8-iso-PGF2αand MoCA scores.Logistic regression was used to analyze Acute ischemic stroke.Influencing factors of patients’cognitive function changes.ROC curve analysis of serum ICAM-1,8-iso-PGF2αexpression for the diagnostic value of cognitive function in patients with Acute ischemic stroke.Results The expression of serum ICAM-1 and 8-iso-PGF2αin group A was significantly higher than that in group B(P<0.05);Serum ICAM-1 and 8-iso-PGF2αexpression levels were significantly correlated with the location of infarct,carotid atherosclerosis,carotid stenosis,NIHSS score,MoCA,and age(P<0.05),but not related to education level(P>0.05).The infarcts were located in the thalamus,temporal lobe and frontal lobe.NIHSS score,diabetes and hypertension were independent risk factors for cognitive dysfunction in patients with Acute ischemic stroke(P<0.05).The sensitivity and specificity of ICAM-1 and 8-iso-PGF2αin diagnosing the changes of cognitive function in patients with Acute ischemic stroke were 93.00%and 90.00%(P<0.05).Conclusion The expression of serum ICAM-1 and 8-iso-PGF2αis closely related to the changes of cognitive function in patients with Acute ischemic stroke.
