经胼胝体穹隆间入路切除第三脑室区肿瘤(附10例报告)

摘    要:目的探索经胼胝体穹隆间入路切除第三脑室区肿瘤的手术方法、优缺点及术后并发症。方法总结分析2004年1月至2006年5月间经胼胝体穹隆间入路切除10例第三脑室区肿瘤患者的病例资料。结果肿瘤全切7例,近全切3例;术后出现多饮多尿2例,电解质紊乱6例,癫痫2例,经治疗后得到控制。随访时间2周~12个月,病情平稳8例,肿瘤复发1例,死亡1例。结论该入路是切除第三脑室区肿瘤较理想的入路。利用自然脑裂的分离,手术视野、操作角度好、盲区少,有利于显露重要血管、神经,肿瘤全切率高,疗效满意。

Abstract:Objective To investigate the method,advantage,disadvantage and complications of microsurgery through the transcallosal and interfornical approach for the tumors in the third ventricles.Method The clinical data of 10 patients with tumors in the third ventricles,who underwent microsurgery through the transcallosal and interfornical approach for the tumors from January,2004 to May,2006,were analyzed retrospectively.Results Of 10 patients with tumors in the third ventricles,7 received total resection of the tumors and 3 subtotal.The polydipsia-hyperdiuresis occurred in 2 patients,the water-electrolyte disorder in 6 patients and epilepsia in 2 after the operation.The postoperative complications were controlled after the therapy.The follow-up from 2 weeks to 12 months showed that 8 patients were recovered well,1 experienced recurrence of the tumor,and 1 died.Conclusions The tumors in the third ventricles may be satisfactorily resected by microsurgery through the transcallosal and interfornical approach,which has the advantage including the ideal exposure of the important blood vessels and nerves,good visual field of operation,little operative side injury,high rate of total removal of the tumors and so on.
