标准大骨瓣减压并颞肌贴敷术治疗大面积脑梗塞

目的探讨外伤标准大骨瓣减压并脑组织表面颞肌贴敷术治疗大面积脑梗塞患者的效果。方法25例大面积脑梗塞患者采用大骨瓣减压及脑组织表面颞肌贴敷术进行治疗,同期22例患者行内科治疗,并比较其效果。结果在中线回复、患者死亡率、痊愈率三方面,手术治疗组优于常规内科治疗组,但病残率无明显差异。结论标准大骨瓣减压并颞肌贴敷术可降低大面积脑梗塞患者的死亡率,改善预后。

Application of Standard Large Trauma Craniotomy and Roofing of Temporal Muscle to Treatment of Massive Cerebral Infarction

Objective To explore the curative effects of surgical decompression with the standard large trauma craniotomy(SLTC) and roofing of temporal muscle (RTM) on the patients with massive cerebral infarction (MCI). Methods Twenty-five patients with MCI were treated by SLTC and RTM, and twenty-two patients with MCI receiving conservative treatment serve as control group. The out-comes of both the group were compared each other. Results The shift of midline structures was significantly improved after the surgical treatment compared to the deterioration after the conservative treatment (P<0.05). The mortality was 40.90% in the conservative group and 12.00% in the surgical group. The curative effect was better in the surgical group than that in the conservative group. Conclusions A decrease in the mortality and improvement in the prognosis can be reached by SLTC and RTM in the patients with MCI.
