| 摘 要： | 本文通过对３０例颞叶癫痫病人进行手术治疗的实践经验总结，作者提出，颞叶癫痫可按致痫灶部位和痫性病变部位分为内侧型、外侧型和混合型三种，并据此临床分类分别进行选择性海马杏仁核切除术、选择性皮质痫灶切除术，前颞叶大部切除术。结果显示，本组病例手术治疗有效率为８９％，不同手术方法有效率比较无显著性差异。结论认为，寻找可能的结构性病变并采用分类手术的原则可能是治疗颞叶癫痫的有效途径。

| Abstract: | Bymeansofsummarizingsurgicalpracticewith30casesoftemporallobeepilepsy(TLE)patients,authorsdemonstratedaclinicalclassificationofTLEinthisarticle,whichdistinguish-esTLEintothreetypes,namelytemporo-medial,temporolateralandmixedtype.Accordingtothedif-ferenttypes,Weselectedanygdalohippocampectomy,corticalresectionandanteriortemporallobecto-my,respectively.Theresultsshowedtheefectiverateofsurgicaltreatmentis89%therewerenosig-nificantdiferencesbetweentheefectiverateandthevariousoperations.Wesuggestitisacomprehen-siveandefetivewayforsurgicaltreatmentofTLE.thatcombinetosearchthestructurallesionswiththeseclassifiedoperations. |
