脑梗死患者氧化型低密度脂蛋白与颈动脉粥样硬化的相关性研究

摘    要:目的 研究脑梗死患者氧化型低密度脂蛋白(oxidized low-density lipoprotein,oxLDL)与颈动脉粥样硬化之间的相关性.方法 入选脑梗死患者68例,根据彩色多普勒检查颈总动脉平均内中膜厚度,将其分为内膜正常组30例和内膜增厚组38例.并取33例年龄相当的健康受试者作为对照组.采用酶联免疫吸附法测定血清oxLDL的含量.比较不同组别间oxLDL水平的差异以及oxLDL与颈动脉粥样硬化的关系.结果 脑梗死组患者的oxLDL水平明显高于健康对照组(P<0.01).脑梗死患者中颈总动脉内中膜厚度(intima-media thickness,IMT)增厚组的oxLDL水平明显高于IMT正常组(P<0.01);Spearman等级相关分析提示,oxLDL的水平与颈动脉粥样硬化呈正相关(r=0.572,P<0.01);Logistic回归分析提示,oxLDL可认为是脑梗死患者颈动脉粥样硬化的可能独立危险因素(OR=3.896,P=0.002).结论 脑梗死患者的血清oxLDL水平显著升高,并与颈动脉粥样硬化密切相关.

Abstract:Objective To study the relationship between oxidized low-density lipoprotein and atherosclerosis of carotid artery in patients with cerebral infarction. Methods According to average intima-media thickness (IMT) of common carotid artery, 68 patients with cerebral infarction were grouped as 30 eases of normal IMT group and 38 cases of thick IMT (IMT≥1. 0 mm) group. At the same time, 33 healthy individuals were included as a control group. Their oxLDL in serum were measured by Enzyme Linked Immuno Sorbent Assay (ELISA) , to compare the difference of oxLDL among these three groups ; and to study the relationship between oxidized low-density lipoprotein and atheroselerosis of carotid artery. Results The level of oxLDL was significantly higher in cerebral infarction patients than that of the control group (P 〈0.01 ). oxLDL was also significantly higher in cerebral infarction patients with thick IMT than that in patients with normal IMT (P 〈 0.01 ). Spearman rank correlation analysis showed that oxLDL level was positively correlated( r = 0. 572 ,P 〈 0.01 )with the degree of atheroselerosis of carotid artery. Logistic regression analysis showed that oxLDL level was a probably independent risk factor of atherosclerosis of carotid artery( OR = 3. 896, P = 0. 002) in cerebral infarction patients. Conclusion The level of oxLDL is significantly higher in patients with cerebral infarction, and it is also highly associated with atherosclerosis of carotid artery.
