经单鼻孔蝶窦直接入路切除垂体腺癌

目的：报告经单鼻孔蝶窦直接入路切除垂体腺瘤32例显微手术经验，并与其他经蝶入路进行比较。万法：扩张器插入手术侧鼻孔直抵蝶窦前壁进行扩张，显露蝶窦开口，凿开前壁，进行手术。结果：31例肿瘤全切除，1例大部切除。1例术后脑脊液鼻漏。结论：此入路与其他经蝶切除鞍区病变的术式相比，对鼻部组织损伤小，不必分离鼻中隔和两侧粘膜，省时，简单，并发症少，值得临床推广。

A direct transnasal transsphenoidal approach to remove pituitary tumors

Object 32 cases of pituitary adenomas were resected via direct transnasal transsphenoidal approach, which compared with the others sphenoidal approachs. Method The speculum was straighily through nostrile to the anerior wall of the sphenoidal sinus. As expanding the speculum, the middle septum was pressed to the opposite side. Under the operating microscope, the anterior wall mucosa was dissected away, the bony wall drill off and then the sphenoidel sinus approached. other steps were as usual. Result Total resection was achievsed in 31 patients, and partial resechon in 1, CSF leaks occured in one patient, but no operative mortality. Conclusion Compared with the others, the advantages of this approach are less destructive, less time, less complications and easy to perform concurrently.
