摘    要:目的:利用导航系统进行手术,探讨神经导航手术中脑组织移位影响因素及处理。方法:使用导航显微镜的记忆功能,分别对路径方向及平面方向两种移位进行研究;同时对术中脑室开放组与未开放组的移位大小进行比较。结果:所有病灶在导航显微镜下全切除。病变体积及手术时间长短对脑组织在手术路径方向的移位有相关性;手术入路的角度及病变的深度与路径方向的移位有相关性;脑室开放后大量脑脊液的丢失对皮层在路径方向及平面方向的移位均有影响。结论:重力作用是脑组织移位的重要因素。对于术中脑组织的移位,可以采用相应的措施减少移位的程度。

Abstract:Objective:To discusses the correlated factors of brain shift and methods of reducing and avoiding these errosrs in neuronavigation operation.Method:All the operation process was done under the direction of navigation system.Record the volume,area of craniotomy,depth of disease,operation time and angle of operation path.Use these data study the correlation with the movement of brain depress and shift,meanwhile analyze the effect of ventricle on these two directions.Result:The volume of mass and the operation time have correlation with the movement of brain depress.The angle and the depth of disease have correlation with the shift of brain.The opening of ventricle has effect on the movement both depress and shift of brain.Conclusion:Gravity may be the main factor of brain shift,several methods can be used to reduce the error and enhance operation quality.
