鼻内镜下蝶窦手术

摘    要:目的探讨鼻内镜下蝶窦病变的处理方法及疗效。方法采用鼻内镜手术对37例蝶窦病变进行手术治疗,根据不同病变情况,采用不同的入路及处理方法。结果所有病变均获得痊愈。2例真菌性蝶窦炎病例蝶窦口出现反复狭窄闭锁。经多次换药处理后治愈。1例蝶窦癌患者术后3年肿瘤复发。其它患者病变均无复发。结论根据蝶窦病变的不同临床特点,采用不同鼻内镜手术入路和处理方法,既可有效治愈蝶窦病变,同时可最大限度地减少手术创伤。

Abstract:Objective To explore the surgical techniques for and to observe the therapeutic effect of nasal endoscopic therapy on sphenoidal sinuous lesions based on a clinical trial. Methods Included in this study were 37 cases with sphenoidal sinuous lesions treated under nasal endoscope via various approaches determined according to the individual status of lesions among these cases. Results All these cases were cured as evaluated after the surgery. The sinuous opening in 2 cases with fungus infection in sphenoidal sinus became narrowed, and was cured after several times of simple appropriate treatment. However, one case with a cancerous lesion in sphenoidal Sinus relapsed 3 years after the operation. Conclusions The choice of approaches for performing endoscopic surgery to treat sphenoid sinus lesion can be nimbly decided according to the actual status of lesion in the sinus, Then, such a surgery can not only treat the local lesion effectively, but also reduce the surgical trauma in the operating field at the maximal extent.
