高压氧对缺氧缺血性脑病新生儿血清神经元特异性烯醇化酶的影响

摘    要:目的探讨高压氧(HBO)治疗新生儿缺氧缺血性脑病(HIE)的临床疗效及对血清神经元特异性烯醇化酶(NSE)的影响。方法临床诊断为HIE的新生儿103例,随机分为HBO治疗组(55例)和对照组(48例)。对照组采用常规治疗,包括吸氧、止惊、脱水等,HBO组在常规治疗基础上给予高压氧治疗。分别检测两组治疗前(生后3d)和治疗后(生后14d)血清NSE的含量;以临床症状消失时间比较近期疗效;随访至1岁,以Gesell发育诊断量表进行智能发育最终评估,以精神运动发育商(DQ)比较远期疗效。结果治疗后HBO组血清NSE明显低于对照组(P〈0.01)。HBO组近期和远期疗效均优于对照组,治疗总有效率和1岁时DQ值均高于对照组(P〈0.05)。结论HBO对HIE新生儿具有脑保护作用,能减轻临床症状,改善预后。

Abstract:Objective To explore the efficacy of hyperbaric oxygen ( HBO ) in the treatment of newborns with hypoxicischemic encephalopathy (HIE) and its effect on plasma neuron - specific enolase (NSE). Methods 103 newborns diagnosed as HIE were randomly divided into two groups : HBO treatment group ( n = 55 ) and control group ( n = 48 ). Neonates in control group received conventional treatment including oxygen inhalation, anti - convulsion, dehydration and etc. , while those in HBO treatment group were given HBO besides conventional treatment. The contents of plasma NSE in both groups were measured in pre - treatment (3 days after birth) and post - treatment (14 days after birth), and short - term curative effects were compared according to extinction time of clinical symptom between two groups. All newborns were followed up till one year old . Their intellectual developments were evaluated by Gesell development diagnosis scale and long - term curative effects were compared according to psychomotor development quotient (DQ). Results The level of plasma NSE was significantly lower in HBO treatment group after treatment than that in control group ( P 〈 0. 01 ). Moreover, both short - term and long - term curative effects as well as total effective rate and DQ value in one year old in HBO treatment group were better than those in control group ( P 〈 0.05 ). Conclusion HBO can protect brain, alleviate clinical symptoms and improve the prognosis in newborns with NIE.
